Division of Water Resources' Central and Regional staff has reviewed the application package
received December 12, 2020 and subsequent additional information package received March 18, 2021.
However, additional information is required before the review may be completed. Please address the items
on the attached pages no later than the close of business on June 14, 2021.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through P, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.

the uploaded submittal. We could not find a copy of the well construction records (GW-1 forms)
for the wells listed as installed and active. Have the GW-1 forms been filed with the Groundwater
Resources Section — Well Construction Permitting unit? If the GW-1 forms have not been
submitted, then please submit them. Please provide a copy of the filed GW-1 well construction
records so that we may update the Trillium Links facility in our database.
2. Item IV.10, the longitudes for all four wells are the same. Is this anerror?

the uploaded submittal did not include this document. A similar issue appears to have occurred
with the initial submittal package. Please email a copy of the O&M Manual separately as there
may be an issue with file size or format preventing this document from being uploaded fully through
the electronic submittal site.
O. Residuals Management Plan:
L The residuals management plan refers to a contract between Trillium Links and a local sludge
hauling and land application company. It is unclear whether Trillium Links has been added as a
certified residual source for land application because specific company/related permit information
has not been provided. Please provide additional information, this can be in the form of the permit
for the land application company showing Trillium Links listed as a residual source, or a contract
letter to add the Trillium Links residuals to the land application permit.

dimensions/drawings were not included in the previous response for Comment B.3 in
Additional Information Request 91. Is this grey hatching indicating an expansion of this
treatment zone that needs to be included within the permit under Phase II to be constructed?
Additionally, within the provided specifications it refers to the sludge holding membranes
being served by 25 gallon per hour permeate pumps. Please provide a description of the sludge
holding system so that the facility description within the permit language is accurate (i.e. two
52 square foot (sf) sludge holding membrane modules served by two 25 GPH permeate pumps).
