Hi Suzanne -
Attached please find the Recission Request form to remove the stormwater permit from the Caledonian Monroe, NC
facility. A copy will also be mailed to you.
The facility has been closed since May, 2017. All raw material have been removed from the site.
Will the site still be responsible for paying the NPDES fees that were assessed since the facility has been shut down? If
so, please forward me copies of the outstanding invoices that will need paid to close this matter.

Please follow these instructions:
Rescind an Existing Industrial Permit
Complete the Rescission Request Form [files.nc.gov] and mail one hard copy of the form to Suzanne McCoy, 1612 MSC,
Raleigh, NC 27699-1612. In addition, e-mail the form to Suzanne McCoy. There is no fee for industrial permit
rescissions. Please note that it can take 1-2 months to process the request because we cannot rescind the permit until
DEQ staff conduct a site visit.

Hello Suzanne
I have a question on this site. It is owned by Caledonian Alloys but was shut down and all material removed
and the facility cleaned in 2017. There has been no activity since then and none is expected in the near future.
Is the Stormwater Permit still required?

I have attached a Stormwater Permit Summary Report for your reference and looped in my colleague, Michelle, who
placed the call.
Is it possible that this site was sold and a transfer of the permit to the new owner was not made?

I am contacting you in the hopes of discovering what this is about. I am the EHS Manager for Caledonian
Alloys in California and this has been sent to me. We have a storm water permit but through the California
Waterboards.
I am trying to find out what this is so it can be remedied. Please feel free to call me.
