Facility Summary
This facility is a minor facility (flow < 1 MGD) discharging 100% domestic wastewater. This facility has a
design capacity of 0.0265 MGD but is permitted to expand to a flow capacity of 0.100 MGD upon receiving
an ATC from DWR and submitting an Engineer Certification to DWR. This WWTP utilizes the following
treatment units:
• Bar Screen
• Two Equalization Tanks
• Aeration Basin with Diffused air
• Secondary Clarifier
• Chlorination with Contact Tank
• Dechlorination
• Aerated sludge Holding Tank
• Recording Flow Meter with Totalizer
Permit Flow Conditions
Effluent flow is permitted as Phase 1 and Phase 2 (see Part I), and limited at 0.0265 MGD [Section A. (1.)]
and 0.100 MGD [Section A. (2.)]. Expansion requires the Division's Authorization to Construct (ATC)
permit and subsequent submittal of an Engineer's Certification Form (see Supplement to Permit Cover
Sheet).
Renewal Summary
This renewal contains the following proposed changes:
• The receiving stream has been updated to reflect the correct segment of Hagan Fork (Stream Index:
11-76-5-2-(2)) where the discharge is located, which is currently classified as WS-IV and not C as
previously permitted.

• The language in paragraph 2 on the Supplement to Cover Sheet has been updated to further clarify
the requirements for expansion to 0.100 MGD.
• The map has been updated to reflect the correct discharge coordinates, stream index and waterbody
classification.
Permit History
In the past, the permit has lacked limits and monitoring for fecal coliform due to the receiving stream
providing such dilution to the discharge that they are not required. There has also been no requirement for
disinfection. Per Roberto Scheller and Corey Basinger at MRO, chlorination and de -chlorination was
suggested to be added to the permit to protect the health of downstream users. Monitoring for fecal was also
recommended and a study began Tuesday, November 27th, 2018 and continued with a twice per week
monitoring schedule for three weeks. The samples were then analyzed by a private lab and evaluated by
MRO. No modifications were made at this time; any proposed modifications will be discussed by John
Hennessy and Julie Grzyb at a later time.
Compliance History
Country Valley WWTP has no enforcements on record. Most recent inspection in 2019 shows the facility
has been in compliance.
One violation has been issued since the last permit renewal:
• July 2016 — Permit Condition violation — Failure to register for eDMR and submit eDMRs

SLUDGE MANAGEMENT PLAN
For
Aqua North Carolina, Inc.
No sludge will be treated on any wastewater treatment plant site operated
by Aqua N.C., Western Division
Where practical, sludge removed from an Aqua N.C., Western
Division facility will be transported via a contract hauler to another
Aqua N.C.; Western Division facility for the purpose of "seeding" a
new or under loaded plant.
Unusable (or "dead") sludge will be removed by a contract hauler and
properly disposed of in accordance with NCGS 143-215.1. Contract
haulers used by Aqua N.C., Western Division will be required to
report the quantity of sludge transported and identify the location of
the proposed disposal site if the sludge is not taken to an existing
plant operated by Aqua N.C., Western Division. Aqua N.C., Western
Division has not entered into any agreement to accept sludge into its
facilities from plants not owned by them.
Aqua N.C., Western Division will keep records on the quantity of sludge
removed from each facility, the name of the contract hauler, and the
destination of the sludge (whether used in another plant or disposed of).
The information will be kept on file and will be made available to any
regulatory agency having jurisdiction over sludge treatment or disposal.
Aqua N.C., Western Division includes all of the facilities under the
jurisdiction of the Mooresville Regional Office.
