Please find below, regional comments for the subject minor permit renewal.
Applicant is not requesting modification of thefacility or increasing flow at this time.
A rating sheet is not attached as no modifications were made to this facility during the previous permit cycle.
A review of compliance data did not reveal any significant or repetitive violations.
Based on the last NPDES Compliance Inspection, there are no outstanding repairs or modifications of the
treatment works necessary at this time.
No special conditions, limitations, or monitoring is suggested at this time.
Based on the above information, the Fayetteville Regional Office recommends reissuance in keeping with the
current basin wide strategy.

Enclosed please find a copy of the Compliance Evaluation Inspection form from the inspection conducted on
February 24, 2009. The Compliance Evaluation Inspection was conducted by Trent Allen of the Fayetteville
Regional Office. The facility was found to be in Compliance with permit NC0035904. As a reminder, preservation
of the Waters of the State can only be achieved through consistent NPDES Permit compliance.
