The results detailed below represent the status for each of your certified parameters and methods to date. A blank in the "PE complete" column indicates no PE yet recorded for this calendar year. A "Pass" entry indicates your lab has satisfied the annual performance evaluation requirement for that parameter and/or method. Other information in this column indicates vendor and Study identification. A "Fail" entry requires a follow up sample to be analyzed and reported to this office within 60 days. An N/A entry represents parameter exemption from the performance evaluation requirement. A date entered beside the "Fail" entry represents the completion deadline for this PE follow up. You are not required to submit additional results to this office for the calendar year 2004. Should a PE provider notify us of a failure on one of your certified parameters, you will be required to analyze add submit a follow up blind PE result. Failure to analyze and submit an acceptable follow up sample result within 60 days or producing a second successive unacceptable result will result in decertification for that parameter.
