4. Project Description and History
4a.Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
General land use is rural,with moderate development and a fairly even mix of forest and agricultural lands.
Ashe 122 is a 261'long,single lane,wooden bridge in need of replacement.NCDOT Structures Management Unit records indicates No.122 has a sufficiency rating of 26.99 out of a
possible 100 for a new structure,and is considered structurally deficient according to Federal Highway Administration(FHWA)standards.The superstructure and substructure of Bridge
No.122 have timber elements that are 60 years old.Timber components have a typical life expectancy between 40 to 50 years due to the natural deterioration rate of wood.Timber
components of Bridge No.122 are experiencing an increasing degree of deterioration that can no longer be addressed by reasonable maintenance activities;therefore,the bridge is
approaching the end of its useful life.Replacement of the bridge will result in safer traffic operations.

4h.Explain the purpose of the proposed project:*
The purpose of this project is to replace Ashe County Bridge No.122 on SR 1549
(Garvey Bridge Road)over North Fork New River.The existing single lane bridge will be replaced with a two lane bridge.The replacement structure will be a bridge approximately 270
feet long,providing a minimum 27-foot,10-inch clear deck width.The bridge will include two 10-foot lanes and 3-foot,11-inch offsets on new
alignment to the south.The bridge length is based on preliminary design information and is set by hydraulic requirements.The roadway grade of the new structure will be approximately
the same as the existing structure.Traffic will be maintained on the existing bridge during construction and no offsite detour is required.
The approach roadway for the proposed replacement structure will extend approximately 30 feet from the west end of the new bridge and approximately 108 feet from the east end of
the new bridge.The approaches will be widened to include two 10-foot travel lanes and 3-foot shoulders(6-foot with guardrail).SR 1549 has a functional classification of a Local Rural
Route and will be designed using Sub-Regional Tier Guidelines with a 20 mile per hour design speed.Approximately 225 feet of SR 1550(E.Weavers Ford Road)will be widened to
provide two 8-foot
travel lanes and 3-foot shoulders(6-foot with guardrail).
4i.Describe the overall project in detail,including indirect impacts and the type of equipment to be used:*
The new bridge will be constructed while traffic is maintained on the old bridge.As a result,the new bridge will be just upstream of the existing bridge.Temporary causeways will be used
to construct the new bridge and remove the old bridge(135'of temporary impact form the causeways).At no time will the causeways block more than 50%of the flow.Causeways will be
installed when needed,and then immediately moved once the work in that area is done-the phasing of the causeways is shown in the permit drawings.
Following construction of the new bridge,the old bridge will be removed.The new bridge will result in 6'of new permanent fill,from the installation of two instream bents(3'wide each).
An additional 127'of bank stabilization(40'of temporary impacts from dewatering)will be installed to stabilize the banks beneath the new bridge,and where the old bridge abutments
were located.
Standard road and bridge building equipment will be used during the projects such as trucks,track hoes,and cranes.
Although the new bridge will be wider,it will only have two instream bents compared to the 4 currently supporting the old bridge(5 if you count the one on the waters edge).Also,the
banks in the general vicinity of the project are severely eroded.As a result,the new bridge and associated bank stabilization should improve water quality and aquatic habitat at this
location.

1.Avoidance and Minimization
la.Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
The existing bridge will remain in place during construction,preventing the need for a detour.Temporary construction impacts from causeways will be
reduced so that no more than 50%of the river flow is diverted at any time.To help ensure this,the contractor will use 72"x44"CSP arch pipes in the
causeway.Portions of the causeway will only be installed when needed,and removed once that portion of the project is complete.All work will be done
in the dry,and no direct discharge of sediment will occur in the North Fork New River.
1b.Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
Temporary impacts will be in place only as long as needed and then removed as soon as construction at that location is complete.All Best
Management Practices will be adhered to.

National Wild and Scenic-The project is located 1/4 mile upstream of a Nationally Designated Wild and Scenic River-South Fork New River.While the project will divert some flow at
the project site,due to the necessary causeway,it should not reduce the river flow.However,if the project needs to be reviewed for possible impacts to the wild and scenic section and
NCDOT can assist with any additional correspondence,please let me know.
Paddler Safety-While pipes in the causeway pose a potential safety issue with recreational river users,there is very limited traffic on the North Fork New River.The closest public
access is approximately 8 miles upstream and their are no outfitters on the North Fork New River,so usage is low.While the South Fork New River is a popular floating area and is only
1/4 mile below Ashe 122,the likelyhood of paddlers venturing up the North Fork New River is extremely low due to the swift current and several riffles/drops in elevation between the
South Fork New River and Ashe 122.
