Hi, Rachel,
You can upload it through the Modification form (https://edocs.deg.nc.gov/Forms/NCG01-Modification), but you are
also welcome to just email it to me —that's probably easier. I can do the Contact Updates and make sure the change
form is added to the file in the process. Be sure let me know in the email if there are changes to site contact and billing
contact (just an email and phone number) as well.
If you want to send me the form before the payment is applied, the emails will be updated for the payment
confirmation.

1. Where do I upload the Legally Responsible Individual Change Form?

This appears to be a change in legally responsible person. To change the legally responsible person associated with the
permit, the permitted entity must submit a Modification Request. That person will need to upload the attached form
signed by the new legally responsible person (I've attached it for reference).
Also, for the other individual's email addresses you gave:
Please specify which individual is what contact (legally responsible, site contact, or billing contact). For a change in site
contact, please provide name, phone number, email, and mailing address. For a billing contact, we only need an email
and phone number. One can include all this information in the Modification request or upload a document outlining it.
When we process the modification, we will then update contacts in our database and send confirmation.
Please be sure the modification request is completed. We cannot make these changes without the signed form.
