In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations, permission is hereby granted to the
City of Hickory
Catawba County
for the construction and operation of approximately 2,763 linear feet of 16-inch gravity sewer,
approximately 475 linear feet of 10-inch gravity sewer, and approximately 5,752 linear feet of 8-inch
gravity sewer as part of the Geitner Basin Sewer System Rehabilitation project, and the discharge of
gallons per day of additional collected domestic/commercial wastewater into the Henry Fork. WWTP
existing sewerage system, pursuant to the application received April 18, 2016, and additional information
received on .April 29, 2016,.and in conformity with 15A NCAC 2T; the Division's Gravity Sewer Minimum
Design Criteria adopted February 12, 1996, as applicable; the Division's Minimum Design Criteria for the
Fast -Track Permitting of Pump Stations and Force Mains adopted June 1, 2000, as applicable; and other
supporting data subsequently filed and approved by the Department of Environment and Natural
Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until rescinded and shall be subject to the specified
conditions and limitations contained therein.

a. The sewer system shall beeffectively maintained and operated atall times bzprevent discharge
to land or surface waters, and to prevent any contravention of groundwater standards or surface
water standards,

c. An operation and maintenance plan including pump station inspection frequency, preventative
maintenance schedule, spare parts inventory and overflow response has been developed and
d. Pump stations that are not connected to a telemetry system shall be inspected every day (Le, 365
days per year), Pump stations that are connected to a 'telemetry system shall be inspected at
least once per week.
e. High -priority sewer lines shall be inspected at least once per every six -months and inspectionsare
documented.
f. A general observation of the entire sewer system shall be conducted at least once per year.
g. Overflows and bypasses shall bereported tnthe appropriate Division regional office |naccordance
with 15ANCAC2B.0505(a), and public notice shall be provided as required bvNorth Carolina
General Statute §143'216.1[
h. A Grease Control Program is in place as follows:
I For public owned collection systems, the Grease Control Program shall include at least
biannual distribution of educational materials for both commercial and residential users and
the legal means to require grease interceptors at existing establishments. The plan shall also
include legal means for inspections of the grease interceptors, enforcement for violators and
the legal means tocontrol grease entering the system from other public and private satellite
sewer systems,
2' For privately owned collection systems, the Grease Control Program shall include mt least bi-
annual distribution of grease education materials to users of the collection system by the
pernmitteeorits representative.
3 Grease education materials shall be distributed more often than required in Parts (l) and (2)
of this Subparagraph if necessary to prevent grease -related sanitary sewer overflows.
Rigand easements shall be maintained in the full easement width for personnel and
equipment accessibility.
� Documentation shall be kept for Subparagraphs (a) through (i) of this Rule for a minimum of three
years with exception of the map, which shall be maintained for the life of the system.

B. Gravity sewers installed greaterthan ten percent below the minimum required slope per the Division's
Gravity Sewer Minimum Design Criteria shall not be acceptable and shall not be certified until
corrected. If there is an unforeseen obstacle in the field where all viable solutions have been
examined, a slope variance can be requested from the Division with firm supporting documentation.
This shall be done through a permit modification with fee. Such variance requests will be evaluated
on a case -by -case basis. Resolution of such request shall be evident priorto completing and submitting

10. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2T; the Division's

applicable; and other supporting materials may subject the Pennitteetm an enforcement action by

The rehabilitation and replacement of sewer lines in the Geitner Basin area will be spread across nine
project areas, each of which was identified in previous studies as having a high priority for rehabilitation. The
portion of the project for which we are seeking a new permit is entirely composed of sewer pipe that is either
replacing smaller diameter pipe, pipe of a different material or is being installed in a different or new location.
The 5,752 |fof8-inch ductile iron pipe we are requesting to be permitted consists of 2,002 If of new
gravity sewer line tobginstalled in acompletely new location inorder toreplace a pump station and force main
which, will be abandoned and 3,750 |fmf0'inCh ductile iron pipe which will replace existing vitrified clay pipe
along the same or similar alignments. An additional 2,395 If of 8-inch sewer line will be trenchlessly rehabilitated
using cured -in -place pipe lining, and is not described in this permit application.
The 475 If Of IO'inch ductile iron pipe, is replacing a mix nfD4nch and 10-inch vitrified clay pipe and
following the same horizontal alignment 3sthe existing pipe, Anadditional Z,]I5|fofl0-inrhsewer line will be
treMChiess|y rehabilitated using cured -in -place pipe lining, and is not described in this permit application.
The 2,763 If of 16-inch ductile iron pipe is replacing 12'inch ductile iron pipenutfaU. Approximately 990
|fofthe 16-inchline will follow the alignment ofthe existing outfoU,but the remaining l,773Kfofl6-inchline
will follow a new alignment, and i5not described in this permit application.
I have attached a new copy of page 5 of the Fast Track Application (FTA 10-14) amending the pipe length
totals to reflect that the amount of lQ-inch line to be permitted is 475 linear feet and to clarify that all of the
sewer lines to be permitted will be composed of ductile iron pipe and not PVC,

and manholes in the Geitner Basin area of the City of Hickory and will collect no new flows. All
lines are connected to the Geitner Basin Outfall and flow to Henry Fork WWTF (NC0040797),
