The faifowing are `;rermhted by rude' and do not require an individual permit when constructed in accordance with the
ruler of 154 NBC 02C.020Q (NOTE: This form must he reserved at bust 14 DAYS prior to infection]

f hese wells are used to inject uncontaminated fluid into an aquifer to determine aquifer hydraulic characteristics_
IN SITU REMEDLATION (I5ANNc4c 02C .O2241 or TRACER WELLS Lsit NCAV 02C 0 29):
1) Passive Injection Systems - in -well delivery systems to diffuse iajectants into the subsurface. Examples include ORC
socks, iSOC systems, and other gas infit_sion methods (Note: injection Event Records (MR) do not need to be submitted for
replacement of each sock used in ORC systems),
?) Small-S(ale Injection Operations — Injection wells located within a land surface area not to exceed 10,000 square feet
far the purpose of soil or groundwater remediation or tracer tests. An individual permit shall be required for test Or
treatment areas exceeding 10,000 square Peet.
3) Pilot Tests - Preliminary studies conducted for the purpose of evaluating the technical feasibility of a remediation strategy
in order to develop a full scale remediation plan for future implementation, and where the surface area of the injection zone
+helis are located within an area that does not exceed five percent of the land surface above the known extent of groundwater
contamination. An individual permit shall be reoufred to conduct more than une pilot test on anti separate groundwater
contaminant plume.
4) Air Injection Wells - Used to inject ambient air to enhance in -situ treatment of soil or groundwater.

1. DESCRIPTION OF PROPOSED INJECTION ACl1V1'PLNS —Provide a beef narrative regarding the purpose,
scope, and goals of the proposed injectirm activity. This should include the rate, volume, and duration of injection
over time.
Prowess will inject approximately 5 kilograms of Enviro-BAC mixed with up to 55-Gallons of water into monitoring
well MW 1. The Enyito BAC will be infected into the molter throw,:1i a PVC tremie nine ur Pouring
directly into the monitoring well. The trcmic [lit.e will extend to an aot+roximate depth of ten feet into the
subsurface. Prorress anticipates iniectine Enviro-HAC into the ahalkLw :atuifet.

violations of groundwater quality standards specified in Subchapter 02L result from the injection activity.

to the injection event. The groundwater samples will be anal yzed for volatile org anic com pounds using Standard
