(Blue Ridge) and its subcontracted driller performed remediation activities at the subject site in 2015. On May 18 and 19, 2015 we drilled 19 borings around monitoring wells MW-3, MW-4, MW-5, MW-6, and MW-7 using a Geoprobe and then injected remediation media into the borings through the hollow drill rods. A map showing the boring/ injection point locations is attached . A total of 1,380 gallons of Hydrogen Release Compound (HRC) was injected into the borings / injection points; a table summarizing the information on each injection point is attached. No water was mixed with the HRC. The temporary wells / injection points were abandoned immediately following injection activities; the top of each boring was filled with bentonite pellets/chips to seal the injection point. Copies of the well construction/ abandonment records are attached. On May 20, 2015 we injected media into deep well DW-1 using a tremie pipe. Approximately 180 gallons of water mixed with 20 gallons of 3-D MicroEmulsion (3-DME) was injected/ gravity fed into well DW-1. We plan to drill sail borings to the top of rock (expected at 11 to 21 feet bgs) near wells MW-3, MW-5. MW- 6. and MW-7 and then inject media (HRC) into the groundwater through the hollow rods of the Geoprobe rise (no wells will be installed). As shown on the attached figure. we nlan to inject at approximately 17 locations. The media will be injected from the bottom of boring to the groundwater surface (about 11 to 14 feet vertically in each borehole).
