Mr. Larry Ammons L&W Farms, Inc. 805 McDaniel Rd Roseboro, NC 28382 Subject: Lagoon Berm Repair Dear Mr. Ammons: RECEIVED MAY 04 2021 NC DEQ/DWR Central Office April 26, 2021 I visited the site at 6719 Old Mintz Hwy, Garland, NC with you on April 24, 2021 to observe the areas of both adjacent lagoons. According to DEQ inspection report from 4/16/21, you are required to submit a plan of action to repair/restore the back slopes in certain areas. As we discussed and I observed, there are areas along the rear east side of both lagoons and along north end of lagoon #2 that require work. There does not appear to be structural impairment at this time that would require liner material or compacting liner material, only to reestablish and stabilize the top surface to prevent further erosion. In addition to back slope erosion, we observed one area of top dike on Lagoon #1 that has settled and appears to be approximately 6" below design top elevation. Please follow the below steps: 1. Add topsoil as required to return slopes and tops of berms to original shape/slope along back slopes of both lagoons in the subject areas as shown on attached aerial photo. 2. Reestablish permanent vegetation by sod or seeding with quick establishing (such as annual rye) and other permanent grass such as hulled Bermuda seed. Roll areas to lightly compact all newly placed soil. 3. Mulch affected areas with straw or hay as available. 4. Low top of dike area: a. Remove existing vegetation in approximately 50' long area of dike b. Compact sandy clay layer along with topsoil above in area to approximately 2" above adjacent current top elevation to allow further settling. c. Establish permanent vegetation with seeding and mulch as listed above. If you have any questions, please don't hesitate to contact me. I have attached pictures to help document the described actions.
