Progress: - Project received high intensity rainfall the day prior to review as well as approximately 1 inch the Thursday prior to review. - Paving has progressed since previous review South of EB1 of the -Y16- Structure. - Stone placement has progressed along -L- from approximately-Y16RPA- to Permit Site 15. - Supplemental seeding and fertilizer topdressing was completed the week prior to review. The following concerns were noted at the time of review: - As noted on previous review: Recommend pursuing continuous work to allow for permanent stabilization to meet ESA requirements as soon as feasible at Permit Site 13 Inlet, Permit Site 14, Permit Site 16, Permit Site 18 and Permit Site 20. - Noted a significant amount of Annual Rye Grass developing at the locations project wide that were seeded in the Fall of 2020. Please note, Annual Rye Grass is not approved for use by NCDOT. Continue to monitor vegetation development and mow annual rye grass as needed as it will inhibit the development of permanent vegetative cover. - Recommend repairing and seeding and matting washing on the cut slope vicinity -L- Sta 38+/- to 39+/- Lt. - Recommend pursuing stabilization of idle disturbed area in the island that is exceeding the NCG01 stabilization timeframes vicinity -L- Sta 45+/- to 48+/-. - Recommend pursuing stabilization of the idle disturbed area to meet NCG01 stabilization timeframes along - Y7A-, -Y8, and -Y9-. - Noted minor turbid discharge at Permit Site 18 as a result of impounded runoff in the ditchline piping throughout the rip rap bank stabilization. Ensure this is adequately documented on the NPDES inspection records. Material should be retrieved per guidance from Jerry or Della. *This item should be treated as Urgent. - Recommend pursuing routine clean out and maintenance of the ditch line measures and DI protections throughout-Y16RPA-. - Recommend pursuing stabilization to meet NCG01 stabilization timeframes vicinity -Y16- Sta 33+/- to 36+/- Lt. *Noted Urgent clean out and maintenance needed at two SDO SSCF outlets at the inlet of permit site 16. *Noted Urgent SSCF outlet and SF repair needed at the outlet of basin 27.1 -Recommend shifting the slope drain up station to manage roadbed runoff into basin 27.4 as discussed on site. - Recommend pursuing stabilization of the idle disturbed area vicinity-Y16RPA- Sta 12+/- to 20+/- as a result of storm drain installation within the required NCG01 stabilization timeframe. *Noted Urgent SSCF outlet maintenance needed at the inlet of Permit Site 13. - Recommend clean out of basin 15.4 as well as maintaining/re-building the failing weir. - Noted maintenance needed at the pipe inlet protection and ditch line measures at the inlet of the 30 inch RCP vicinity -L- Sta 155+/- Rt. - Noted maintenance needed at the SSCF outlets and ditch line measures at the inlet of the 24 inch pipe vicinity - L- Sta 158+/- Rt. -Ensure idle disturbed area is stabilized within the required NCG01 stabilization timeframes vicinity -L- Sta 150+/- to 160+/- including the roadbed and adjacent to the outlets of the 30 inch and 24 inch pipe vicinity -L- Sta 157+/- Lt. -Recommend pursuing stabilization of the idle erodible area on the ditch front slope and back slope draining to basin 16.1. - Recommend pursuing maintenance of the inlet protections along -Y20- as needed. - Noted concrete washout took place on the road bed and not contained in a designated structure at two locations South of the -Y16- structure. Ensure all future concrete washout is contained in a designated washout structure. - Recommend installing a slope drain to manage road bed runoff into basin 12.4 vicinity -L- Sta 132+/- Lt. - Recommend repairing and re -installing the failing slope drain vicinity -L- Sta 127+/- Lt and repairing and seeding and matting significant wash in the slope caused by the slope drain failure. - Per plan inlet protections need to be installed at the Dls vicinity -L- Sta 111+/- and 112+/- CL. -Recommend pursuing maintenance at the DI protection vicinity -L- Sta 111+/- Lt as it is at failure status. *Noted discharge of a skim of material in the JS at the outlet of the 48 inch pipe at the Permit Site 8. Recommend retrieval per guidance from Jerry or Della as well as removing project material and replacing rip rap as needed in the rip rap ditch at this location. *This item should be treated as Urgent. - As noted during previous reviews: Recommend repairing and seeding and matting the wash vicinity -L- Sta 78+/- Lt adjacent to the front slope of the toe ditch. - As noted during previous reviews: Recommend repairing and seeding and matting washing at various locations in the front slope at the bulb out vicinity -L- Sta 71+50+/- Lt. Recommendations Moving Forward: - It is allowable to remove basin 13.1. Ensure perimeter EC measures are maintained to good working order and the basin footprint is stabilized in a timely manner following removal. - Pursue stabilization of idle disturbed area within the 7 day stabilization timeframes per NCG01 requirements for HOW areas. - Recommend installing/re-installing diversions and TSD's daily and in advance of forecasted rains as grading operations progress. - Recommend pursuing stage seeding efforts as grading continues. - Recommend utilizing and maintaining construction entrances at all access points to prevent tracking material onto the roadways. - In areas noted as ESAs: - Ensure work progresses in a continuous manner to a state of stabilization per Project Special Provisions. - Ensure stage seeding is utilized as fill/cut operations progress per Project Special Provisions. - Ensure Grubbing is not performed until immediately prior to grading operations in any areas noted as an ESA per Project Special Provisions. - NPDES Records were not reviewed. Progress: - Project remains similar progress to previous review. - Contractor was preparing for barrier wall pour at the -Y16- structure the day of review. - Travis indicated the contractor was primarily focused on EC maintenance the day of review as a result of wet conditions on the project. The following concerns were noted at the time of review: - Recommend pursuing continuous work to allow for permanent stabilization to meet ESA requirements as soon as feasible at Permit Site 13 Inlet, Permit Site 14, Permit Site 16, Permit Site 18 and Permit Site 20. Travis indicated grading has been a challenge due to recent wet weather. Temporary groundcover should be re- applied as needed to provide stabilization of idle disturbed areas. - Ensure disturbed area is stabilized within the required NCG01 stabilization timeframes vicinity -Y7A- Sta 27+/- to 28+/- Rt. - Ensure the remaining material is retrieved from beyond the SSCF outlet vicinity -Y7A- Sta 29+/- Rt. Also, ensure the SF is properly trenched in at this location. - Recommend installing a fabric insert inlet protection at the DI vicinity -Y7A- Sta 24+/- Rt. - Ensure idle disturbed area is stabilized within the required NCG01 stabilization timeframes along-DWY1-. *Noted a minor skim of material in the JS vicinity Permit Site 10A. Recommend consulting Jerry or Della regarding retrieval and installing a wattle or straw bales on the down stream end of the loss to prevent material migrating further downstream. Also, consideration should be given to utilizing toe protection to prevent future failure caused by the spring that has surfaced at this location. *This item should be treated as Urgent. -Ensure disturbed area is stabilized within the required NCG01 timeframes as a result of recent grade work vicinity -Y16- Sta 18+/- to 22+/- Rt - Ensure disturbed area is stabilized within the required NCG01 timeframes as a result of recent grade work in the radius of -Y16- and-Y16RPA-. 4/23/2021 6:59 AM Page 3 of 10 - Ensure earthen material stockpiles are stabilized within the required NCG01 timeframes at the end of - Y16RPA-. *Noted a minor amount of project material discharged at the inlet of the pipe at Permit Site 16, vicinity - Y16RPA- Sta 18+/- Rt. Material should be retrieved per guidance from Jerry or Della and the SSCF outlet cleaned out and maintained to good working order. Consideration should be give to installing a piece of geotextile fabric down the slope to prevent future washing until the storm drain network is installed. *This item should be treated as Urgent. - Ensure idle disturbed area at Permit Site #12 is stabilized within the required NCG01 stabilization timeframes. -Recommend repaired and seeding and matting the wash from the outlet of the 24 inch pipe to the add on basin vicinity -L- Sta 157+50 Lt. Also, recommend installing wattles to prevent future washing. - Recommend pursuing stabilization of the idle disturbed area along the median and Northbound roadbed vicinity -L- Sta 148+/- to 165+/-. - As noted on previous review: Recommend repairing washing on bank and stabilizing with native seed and coir fiber matting on the right bank looking upstream at the inlet of Permit Site 20. - As noted on previous review: Ensure idle disturbed area is stabilized within the required NCG01 stabilization timeframes vicinity Permit Site 20, at the intersection of -Y10- and -Y11-. - Recommend installing a TRSC-A in the end of the ditch and extending the SF to provide containment above the inlet of the culvert vicinity -Y11- Sta 10+75 Lt. -Ensure efforts are made to re -work the preformed scour hole to per detail dimensions vicinity -L- Sta 130+50 Lt. Mike indicated this was constructed as a temporary dissipator. - As noted on previous review: Recommend repairing and seeding and matting the wash vicinity -L- Sta 78+/- Lt adjacent to the front slope of the toe ditch. - As noted on previous review: Recommend repairing and seeding and matting washing at various locations in the front slope at the bulb out vicinity -L- Sta 71+50+/- Lt. Recommendations Moving Forward: - It is allowable to remove the remaining temporary SF from the beginning of the project to approximate -L- Sta 35+/- including along the ramps. Ditch line measures should be remain in place until final paving and shoulder reconstruction are completed and shoulders are stabilized with permanent vegetative cover. -Pursue stabilization of idle disturbed area within the 7 day stabilization timeframes per NCG01 requirements for HQW areas. - Recommend installing/re-installing diversions and TSD's daily and in advance of forecasted rains as grading operations progress. - Recommend pursuing stage seeding efforts as grading continues. - Recommend utilizing and maintaining construction entrances at all access points to prevent tracking material onto the roadways. - In areas noted as ESAs: - Ensure work progresses in a continuous manner to a state of stabilization per Project Special Provisions. - Ensure stage seeding is utilized as fill/cut operations progress per Project Special Provisions. - Ensure Grubbing is not performed until immediately prior to grading operations in any areas noted as an ESA per Project Special Provisions. - NPDES Records were not reviewed. Progress: - Contractor is currently working to prepare for traffic switch between -Y6A- and -Y7B- - Stone placement has taken place since previous review from Permit Stie 11 to the -Y16- structure. - Travis indicated the contractor was primarily focused on EC maintenance the day of review as a result of wet conditions on the project. - Significant temporary groundcover efforts have been made since previous review at various locations throughout the project and looks very good. Thank you to all involved in these efforts. The following concerns were noted at the time of review: -Recommend pursuing continuous work to allow for permanent stabilization to meet ESA requirements as soon as feasible at Permit Site 13 Inlet, Permit Site 14, Permit Site 16, Permit Site 18 and Permit Site 20. - Recommend repairing washing on bank and stabilizing with native seed and coir fiber matting on the right bank looking upstream at the inlet of Permit Site 20. - Ensure idle disturbed area is stabilized within the required NCG01 stabilization timeframes vicinity Permit Site 20. - Noted stockpiles within 50 feet of Dls at -Y20-. Recommend relocating to a minimum of 50 feet from Dls, removing, or containing stockpiles with SF and pursuing temporary stabilization per the NCG01 permit requirements for earthen material stockpiles. - Recommend pursuing repair of basin 16.1. At the time of review, the basin was not impounding runoff as designed due to significant leaking at the inlet of the HDPE pipe. - Recommend re -attaching the skimmer at the add on basin at permit site 12 and pursuing stabilization of the adjacent idle disturbed area within the required NCG01 stabilization timeframes. - Recommend re -attaching the skimmer at basin 15.4. - Ensure efforts are made to re -work the preformed scour hole to per detail dimensions vicinity -L- Sta 130+50 Lt. I also spoke with John Hilton on site. Progress: - The mainline from the beginning of the project to -Y26- was not reviewed. - Structure work continues at -Y16- and -Y26- - Earthwork has continued since previous review North of -Y16- -Work has progressed since previous review at -Y20- including EC measures installation. -Construction has also progressed at -Y10- and -Y11- since previous review. -Along-Y16RPA-, shoulder, front slope, and back slope grading, stabilization, and EC re -installation has taken place since previous review and looks really good. See roadbed runoff recommendations below. -Travis indicated that the project has received approximately 3 inches of rainfall the week prior to review. Also, he indicated the contractors forces were predominantly only focusing on EC repairs and maintenance the day of review. The following concerns were noted at the time of review: - Please note, the project received approximately 3 inches of rain the week of review. Ensure Routine maintenance items throughout the project are addressed within the required timeframes in addition to the concerns below. -Recommend pursuing stabilization of disturbed area along -Y7A- to meet the NCG01 stabilization timeframes. Also, recommend routine clean out and maintenance of the ditch line measures and inlet protections as needed. - Recommend pursuing stabilization of disturbed area along -Y10- to meet NCG01 stabilization requirements as well as ESA requirements at Permit Sites 18 and 20. - Recommend installation of a slope drain to manage runoff from the roadbed down the slope vicinity -Y10- Sta 22+/- Lt. 8 4/23/2021 6:59 AM Page 7 of 10 - Recommend installing wattles or TRSC-As to introduce PAM and prevent washing along the toe of slope vicinity -Y10- Sta 21+/- to 23+/- Lt. *Recommend pursuing Urgent maintenance of the failing SSCF outlet vicinity -Y11- Sta 12+/- Rt as well as retrieving the minor amount of project material that discharged beyond the outlet. -Noted significant offsite runoff flowing onto the project vicinity -Y11- Sta 12+50+/- Lt. Recommend efforts are made to install either a CWD along the clearing limits or a temporary slope drain pipe across the grade to manage offsite runoff without impacting the work area. - Recommend adjusting the inlet protection to provide containment at the inlet of the 36 inch pipe vicinity -Y10- Sta 33+50 Lt as discussed with Matt and Travis on site. - Ensure ditch line measures utilize PAM per plans and per spec vicinity -Y20- Sta 25+/- to 32+/- Lt and Rt. - Ensure disturbed area along -Y20- is stabilized within the required NCG01 stabilization timeframes. - Ensure road bed runoff vicinity-Y16RPA- Sta 12+/- to 20+/- is managed to basin 27.1 per plans. - Recommend installing a TSD draining to basin 15.4 to manage road bed runoff into the basin per plans. TDs across the grade will need to be adjusted and additional slope drain installed. - Recommend adjusting TDs and slope drains vicinity -L- Sta 161+/- to 163+/- to manage road bed runoff to basin 15.1 per plans. - Recommend clean out of the basin at site 12. Also, efforts should be made to return the skimmer to functionality. - Noted concrete washout occurred not in a washout structure at the -Y16- structure work area. Ensure future concrete washout operations are contained in a designated washout pit as required by the NCG01 Permit. Progress: - Contractor is also nearing completion of construction at -Y6A- and -DWY- - Contractor was working at the time of review to pursue temporary stabilization of the roadbed from -Y13- to - Y26-. - Structure work continues at -Y16- and -Y26- -Earthwork has continued since previous review North of -Y16- -Work has progressed since previous review at -Y20- -Construction has progressed at -Y10- and -Y11- since previous review. The following concerns were noted at the time of review: - Permit Site #8 inlet: Recommend installing per plan dissipator pad vicinity -Y13- Sta 11+50 Lt at the outlet of the 15 inch pipe. Also, recommend adjusting the JS tie in vicinity -Y13- Sta 11+50 Lt and stabilizing with rip rap and Native Seed and coir fiber mat as discussed with Della Moore on site. - Recommend installing per plan EC measures vicinity -Y13- Sta 13+/- to 18+/- Lt. - Recommend retrieving project material from the rip rap ditch vicinity -L- Sta 108+50 Lt and replacing rip rap as needed. - Recommend pursuing stabilization of the exposed median vicinity -L- Sta 105+/- to 108+/- CL to meet NCG01 stabilization timeframes. - Recommend pursuing stage seeding of the slopes above the outlet of permit site 11 to meet ESA requirements. This has been noted on multiple previous reviews. -Noted earthen material stockpile potentially beyond the R/W vicinity -Y26REV- Rt. Recommend staking the R/W to confirm stockpile is located within the project limits and relocate as needed. - Ensure remaining disturbed area is stabilized within the required NCG01 timeframes vicinity -Y26REV-. - Recommend clean out of basin 27.2 as well as re-establishing per plan runoff management to this basin. - Ensure devices utilize PAM per plans and per spec vicinity-Y16RPA- Sta 12+/- to 20+/- Lt. It is also allowable to construct final phase basin 29.1. *This was also noted on previous review. - Recommend pursuing stabilization of the earthen stockpile from directional bore operations at the end of - Y20- within the required NCG01 stabilization timeframes. - Class I rip rap should be installed per plans and per spec at the outlet end of the 60 inch pipe at Permit Site 20. - Recommend pursuing urgent SF maintenance at the outlet of the 60 inch pipe -Ensure per plan number of outlets are installed in the SF vicinity -Y11- Sta 11+/- to 16+/- Rt. -Recommend pursuing SF maintenance vicinity -Y11- Sta 16+/- to 18+/- Rt. -Ensure per plan number of outlets are installed in the SF vicinity -Y10- Sta 21+/- to 29+/- Rt. -Recommend stabilizing disturbed area that remains vicinity -Y11- Sta 11+/- Lt. - Recommend pursuing maintenance of the SF outlets vicinity -Y10- Sta 19+50 to 23+/- Rt. Recommendations Moving Forward: - It is allowable to remove basin 20.2 to allow for permanent ditch construction. Ensure remaining disturbed area is stabilized per plans in a timely manner following basin removal and perimeter EC maintained to good working order. - It is allowable to remove the SF and outlets vicinity -L- Sta 62+/- to 72+/- Rt and -Y7A- Sta 11+/- to 20+50 Rt. -It is allowable to remove the SF vicinity -Y9- Sta 11+50 to 13+/- Rt as well as the wattles vicinity -Y7A- Sta 22+/- to 24+/- Lt. - Pursue stabilization of idle disturbed area within the 7 day stabilization timeframes per NCG01 requirements for HQW areas. - Recommend installing/re-installing diversions and TSD's daily and in advance of forecasted rains as grading operations progress. - Recommend pursuing stage seeding efforts as grading continues. - Recommend utilizing and maintaining construction entrances at all access points to prevent tracking material onto the roadways. - In areas noted as ESAs: - Ensure work progresses in a continuous manner to a state of stabilization per Project Special Provisions. - Ensure stage seeding is utilized as fill/cut operations progress per Project Special Provisions. - Ensure Grubbing is not performed until immediately prior to grading operations in any areas noted as an ESA per Project Special Provisions. - NPDES Records were not reviewed.
