The purpose of this document is to submit a modification request for the Section 401
Water Quality Certification, Neuse Buffer Authorization, The CAMA Major Permit, and
for USACE NWP 23. An additional modified permit drawing/buffer drawing is included
in this modification - request.
Permit Site Impacts
The modified permit drawing highlights the following changes and additional, impacts:
160 Sq. Ft. of temporary wetland impacts (Hand Clearing) to allow for the installation of
the end (wing) wall of the proposed culvert. This area is highlighted, in the attached,
drawing and the impacts have been added to the impact table as hand clearing.
Moratorium Extension
In order for the Bridge Demolition Plan to be accomplished the contractor has requested a
two week extension into the moratorium through February 28h, This request has been

approved by Travis Wilson (WRC) email attached. NCDOT respectfully requests this to
be included in the permit modification.
Compensatory Mitigation
No additional permanent wetland impacts are needed.
Section 404: The USACE has determined that the above - described activities will require
an email requesting modification and hard copies to follow in the mail.
Section 401: We are hereby requesting a modification to the 401 Water Quality
Certification from the NCDWQ issued July 3, 2012 (DWQ No. 20120712.). We are
providing an email requesting modification and hard copies to follow in the mail to
NCDWQ; for their approval.
CAMA: The NCDOT respectfully requests that the referenced CAMA Major
Development Permit (89 -12) be modified to reflect the changes in jurisdictional impacts as
outlined in this letter and presented in the attached revised drawings.

Hi Travis, Thank you for your response. After or onsite meeting yesterday (2/5) the contractor has indicated that the two
week extension should be enough time for him to accomplish his Bridge Demolition pending how long it takes for
agencies to turn around the permit modification request so that they can begin work.

Without knowing how long into the moratorium,the.contractor needs I cannot give a definitive answer. However WRC
would not object to allowing in'water work to complete the demolition of Bridge # 176 in Craven Co through February
28, 2013. Will this two week extension provide suitable time for the contractor to complete the necessary in water
work?

Palmetto believes they will have the entire structure removed by Feb. 15, but in the case something happens and they
