Please look at the scanned document (3 pages) we got in on 2/1/13. They are asking for a mod to the issued Version 4 &
this is all they supplied — they did include a $240 check. The cc's reflect that they did not send you a copy of this.
Would Karen be able to issued based on what they supplied or should I ask for PCN sets to clarify the mod?
