This form must be completed by the appropriate DEM regional office and included as a part of the project
submittal information.
INSTRUCTIONS TO NC PROFESSIONAL ENGINEER:
The classification of the downslope surface waters (the surface waters that any overflow from the facility would flow toward)
in which these sewers will be constructed must be determined by the appropriate DEM regional office. Therefore, you are
required, prior to submittal of the application package, to submit this form. with items 1 through 7 completed, to
the appropriate Division of Environmental Management Regional Water Quality Supervisor (see page 8 of 8). At a

location of these sewers and the downslope surface waters in which they will be located. Identify the closest downslope
surface waters on the attached map copy. Once the regional office has completed the classification,
reincorporate this completed page and the topographic map into the complete application form and
submit the application package.

MANAGEMENT UNLESS ALL OF THE APPLICABLE ITEMS ARE
INCLUDED WITH THE SUBMITTAL
Requited Items
One original and one copy of the completed and appropriately executed application form. If modifications to the form are
required as a result of additional information requests by the Division, the additional information must be transmiaed under
the signature of the applicant and the actual modification must be initialed by the NC Professional Engineer.
b. Two (2) sets of detailed plans and specifications signed and sealed by a North Carolina Professional Engineer. The plans must
inclur_fe '- -"t 1' tin^ Map, a plan wed of the sewer extension, a profile of the sewer extension, and must show the
proximity of the sewer extension to other utilities and natural features. Specifications may be omitted for delegated
authorities. Each sheet of the plans and the•fmst page of the specifications must be signed and sealed. Two (2) copies of the
existing permit if a modification. The plans and specifications must not contain phrases (such as:. FOR REVIEW ONLY,
NOT FOR CONSTRUCTION, etc.) that indicate that they are anything other than final plans and specifications. However,
the plans and specifications may contain: FINAL DESIGN - NOT RELEASE) FOR CONSTRUCTION.
c. Two copies of all calculations, including pump selection, friction calculations, cycle time, pump curves (including system
curves applicable with one pump running, two pumps running, three pumps running, etc.), and evaluation of downstream
pump stations. These items must be submitted under the signature and seal of the NC Professional Engineer.
d. The appropriate permit processing fee, in accordance with 15A NCAC 2H .0205(c)(5). The fee for sewer extensions for
nondelegated municipalities is $400. The fee for sews extensions for delegated municipalities (applies only to those
governmental jurisdictions that have spec delegation review authority, as granted by the Environmental Management
Commission) is S200. Name changes without other modifications are S100.
e. If the owner/authority of the wastewater treatment facility (WWTF) that will be accepting the wastewater flow fro this
project is different from the applicant of the project, then a lener must be provided from the owner/authority of the WWIF
specifying the volume of flow that will be accepted The letter should be a recent letter and should refer to the project by the
same name as that identified on the application and the plans/specifications.
f. If the application is being submiaed in the name of a privately owned public utility, evidence must be submitted from the
Utilities Commission which demonstrates that the utility is authorized to bold the franchise for the area to be served. In the
case of contiguous service areas. evidence must be provided from the Utilities Commission acknowledging these areas are
covered under an existing franchise.
A properly executed Operational Agreement (original and two copies - form provided by DEM) must be submitted if the sewer
extension will be serving single family residences, condominiums, mobile homes, or town houses and if the subject sewer
extension is owned by the individual residents, a homeowners association, or a developer.
The downslope surface waters classification must be determined bythe
this t appropriate DEM regional office, using page 5 of 8 of
form, prior to the submittal of the application package to the Water Quality Permits and Engineering Unit. Once the
regional office has completed the classification, page 5 of 8 should be reincorporated into the application package and the
entire application package may then be submitted to Water Quality Permits and Engineering Unit.
i• A cover lener, which briefly describes the project, should be included with each application package. If necessary for clarity,
feel free to include attachments to the application form. Such attachments will be considered as part of the application and
should be numbered to correspond to the section to which they refer.
All materials and fees submitted in support of this request for a permit become the property of the Division of Environmental
Management; therefore, if the permit is not issued for any reason, the Division will return submitted materials only at its
option.

The Permits and Engineering Unit has completed a preliminary engineering review of the
subject application. The following items must be addressed before we can complete our review:
1. The submitted application references a power reliability letter from the power
company. This letter was not found in the application package. Please provide
a power reliability letter from the power company, so it can be used to
determine if the subject pump station has enough emergency storage.
2. The submitted application package did not contain any specifications. there are
some factory cut sheets for the pump station, but, no specifications for the
subject project. Please provide adequate specifications for the subject project.
3. The submitted plans are stamped "Preliminary..." on sheet 1 of 16. The
division can not stamp approve a set of plans that are not a final design. Please
provide a plan sheet that is not stamped "Preliminary...". The use of "FINAL
DESIGN - Not Released for Construction" or similar statement is acceptable.
4. The submitted plans appear to be missing plan and profile for the part of the
proposed 4 inch force main that runs along Courtland Drive. Please provide
revised plan sheets, or provide instruction as to where to find the plan and
profile of this part of the proposed force main.
Refer to the subject permit application number when providing the requested information.
Also, please note that failure to provide this additional information on or before March 10, 1997,
will subject your application to being returned as incomplete in accordance with 15 NCAC 2H

The Permits and Engineering Unit has completed a preliminary engineering review of the
subject application. Your additional information provided in response to the February 7, 1997,
request letter was very helpful. The following new items must be addressed before we can
complete our review:
1. The submitted additional information power reliability letter, from the power
company, indicates the storage in the proposed pump station wet well does not
have enough emergency storage designed into it, as indicated by the submitted
application. At the stated average flow of 29 gallons per minute into the pump
station (41,760 gpd -- 1440 min./day) there is only 33.6 minutes of emergency
storage between the alarm float level and the top of the pump station wet well.
The reliability letter provided indicates power outages as long as 2 hours have
happened in 1996. Please address this shortage of emergency space in the
proposed wet well, and provide revised pump station plans, if appropriate,
and/or a new alternate power plan.

Dear Permittee:
On March 19, 1997 the Division of Environmental Management issued you the subject permit. One of the
conditions of the subject permit read as follows:
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted
facility has been installed in accordance with the requirements of this permit and the
approved plans and specifications. Mail certification to the Permits and Engineering
Unit, P.O. Box 29535, Raleigh N.C., 27626-0535.
To date, our records show that no certification has been received for the subject permit. If the permitted
facility has been constructed and placed into operation please submit the required certification immediately.
Operation of the treatment system prior to submission of the required certification is in violation of your
permit and is subject to enforcement action. If you have any questions, please contact consulting engineer
for this project, GODWIN JORDAN TEW & ASSOC at (910)892-5159. If the facility has not been
constructed, please contact the Fayetteville Regional Office at (910)486-1541, so that they can update their
records.
