INSTRUCTIONS TO ENGINEER
In order to determine whether provisions for dual or standby power may be required for the
subject facility, the classification of the closest downslope surface waters (the surface waters that
any overflow from the facility would flow toward) must be determined. You are required to
submit this form, with items 1 through 10 completed, to the appropriate Division of Environmental•
Management Regional Water Quality Supervisor (see attached listing). At a minimum, you must
include an 8.5" by 11" copy of the portion of a 7.5 minute USGS Topographic Map which shows
the subject surface waters. You must identify the location of the facility and the closest downslope
surface waters (waters for which you are requesting the classification) on the submitted map copy.
The application may not be submitted until this form is completed and included with the submittal.

THE APPLICABLE ITEMS ARE INCLUDED WITH THE SUBMITTAL
Required Items
a. One original and three copies of the completed and appropriately executed application form.
b. Three sets of detailed plans and specifications signed and sealed by a North Carolina
Professional Engineer. The plans must include a general location map, a plan view of the
sewer extension, a profile of the sewer extension, details on the pump station, and must
show the proximity of the sewer extension to other utilities and natural features.
Specifications may be omitted for delegated authorities. Each sheet of the plans and the first
page of the specifications must be signed and sealed. Three copies of the existing permit if
a renewal or modification.
c. Three copies of all calculations, including pump selection, friction calculations, cycle
time, pump curves (including system curves applicable with one pump running, two
pumps running, three pumps running, etc.), and evaluation of downstream pump stations.
These items must be submitted under the signature and seal of the North Carolina •
Professional Engineer.
d. The appropriate permit processing fee, in accordance with 15A NCAC 2H .0205(c)(5).
e. •
If the owner/authority of the wastewater treatment facility; (WWTF) that will be accepting
the wastewater flow from this project is different from the applicant of the project, then a
letter must be provided from the owner/authority of the WWTF specifying the volume of
flow that will be accepted. The letter should be a recent letter and should refer to the project
by the same name as that identified on the application and the plans/specifications.
f. An Operational Agreement (original and two copies) must be submitted if the sewer
extension will be serving single family residences, condominiums, mobile homes, or town
houses and if the subject sewer extension is owned by the individual residents, a
homeowners association, or a developer.

c. Three copies of all calculations, including pump selection, friction calculations, cycle
time, pump curves (including system curves applicable with one pump running, two
pumps running, three pumps running, etc.), and evaluation of downstream pump stations.
These items must be submitted under the signature and seal of the North Carolina
Professional Engineer.
d. The.appropriate permit processing fee, in accordance with 15A NCAC 2I-I .0205(c)(5).
If the owner/authority of the wastewater treatment facility (WWTF) that will be accepting
the wastewater flow from this project is different from the applicant of the project, then a
letter must be provided from the owner/authority of the WWII? specifying the volume of
flow that will be accepted. The letter should be a recent letter and should refer to the project
by the same name as that identified on the application and the plans/specifications,
f. An Operational Agreement (original and two copies) must be submitted if the sewer
extension will be serving single family residences, condominiums, mobile homes, or town
houses and if' the subject sewer extension is owned by the individual residents, a

The Permits and Engineering Unit has completed a preliminary engineering review of the subject
application. Additional information is required before we can continue our review. Please provide the
following by December 15, 1995:
* Your application and plans indicate 445 linear feet of sewer pipe between manhole 8 and
manhole 18. Please submit written documentation to the Permits and Engineering Unit which
details the method of cleaning pipe lengths greater than 425 feet and the type of equipment to be
used. The letter should be signed by a responsible party of the Town of Angier. This letter will be
put on file and will be referred to whenever the Town of Angier submits applications for sewer
extensions with distances greater than 425 feet between manholes.
* In accordance with 15A NCAC 2H .0219 (h) (3) (B), a telemetry system must be used in
instances when portable generators are to be used for secondary power. Please revise the plans to
show the telemetry system and submit a section of specifications pertaining to the telemetry
system. Also, indicate that an autodialer is to be used in your application on sheet 5 of 10. The
revised plan sheets should be submitted in triplicate and the revised sheet of the application must be
individually signed and sealed and submitted in triplicate.
* A profile view of the force main is required to verify that the 3 feet minimum cover is
maintained and that air release valves are provided when necessary. Please submit the revised plan
sheets in triplicate.
* Please check the high elevation of the force main listed on sheet 5 of 10 of your application.
If any changes need to be made, revise sheet 5 of 10, sign and seal it, and submit it in triplicate.

The Permits and Engineering Unit has completed a preliminary engineering review of the subject
application. Additional information is required before we can continue our review. Please provide the
following by December 15, 1995:
* It appears as though this subdivision is to be constructed in different phases. Please be
advised that a separate permit is required for every separate construction contract. Please also be
advised that the operation of a collection system without an engineer's certification for that system
is a violation of the permit. The Division will not accept partial certifications, therefore, operation
of a portion of a collection system, covered under one permit, is a violation of that permit. If this
application needs to be split into different phases, application sheet 3 of 6 needs to be revised to
reflect only the sewer lines to be constructed and put into operation. Also, a revised, complete set
of plans for only this phase is required.
* Your application and plans indicate 445 linear feet of sewer pipe between manhole 8 and
manhole 18. Please submit written documentation to the Permits and Engineering Unit which
details the method of cleaning pipe lengths greater than 425 feet and the type of equipment to be
used. The letter should be signed by a responsible party of the Town of Angier. This letter will be
put on file and will be referred to whenever the Town of Angier submits applications for sewer
extensions with distances greater than 425 feet between manholes.
* In accordance with 15A NCAC 2H .0219 (h) (3) (B), a telemetry system must be used in
instances when portable generators are to be used for secondary power. Please revise the plans to
show the telemetry system and submit a section of specifications pertaining to the telemetry
system. Also, indicate that an autodialer is to be used in your application on sheet 5 of 10. The
revised plan sheets should be submitted in triplicate and the revised sheet of the application must be
individually signed and sealed and submitted in triplicate.
* A profile view of the force main is required to verify that the 3 feet minimum cover is
maintained and that air release valves are provided when necessary. Please submit the revised plan
sheets in triplicate.

Dear Permittee:
On December 27, 1995 the Division of Water Quality issued you the subject
permit. One of the conditions of the subject permit read as follows:
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer
certifying that the permitted facility has been installed in accordance
with the requirements of this permit and the approved plans and
specifications. Mail certification to the Permits and Engineering Unit,
P.O. Box 29535, Raleigh, N.C., 27626-0535.
To date, our records show that no certification has been received for the
subject permit. If the permitted facility has been constructed and placed into
operation please submit the required certification immediately. Operation of
the treatment system prior to submission of the required certification is in
violation of your permit and is subject to enforcement action. If you have any
questions, please contact your consulting engineer for this project, RAYBON
ENGINEERING at (919)637-7571. If the facility has not been constructed, please
contact the Fayetteville Regional Office at (919)486-1541, so that they can
update their records.
