Dear Mayor Lewis:
In accordance with your application received August 17, 1994, we are forwarding herewith Permit
No. WQ0009882 dated November 10, 1994, to the Town of Lillington for the construction and operation
of the subject wastewater collection extension. Please be advised that any increase in the flow over the
amount allowed by this permit will require that a permit amendment be requested and obtained prior to the
discharge into the sewers permitted herein.
This permit shall be effective from the date of issuance until rescinded and shall be subject to the
conditions and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the
right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of
this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings, P.O. Drawer
27447, Raleigh, NC 27611-7447. Unless such demands are made this permit shall be final and binding.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO
Town of Lillington
Harnett County
FOR THE
construction and operation of approximately 550 linear feet of 8-inch gravity sewer to serve Edwards
Brothers and the discharge of 7,298 GPD of collected domestic and industrial wastewater into the Town of
Lillington's existing sewerage system, pursuant to the application received August 17, 1994, and in
conformity with the project plan, specifications, and other supporting data subsequently filed and
approved by the Department of Environment, Health and Natural Resources and considered a part of this
permit.
This permit shall be effective from the date of issuance until rescinded and shall be subject to the
following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in accordance with the
conditions of this permit, the approved plans and specifications, and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
3. The facilities must be properly maintained and operated at all times.
4. The sewage and wastewater collected by this system shall be treated in the Town of Lillington
Wastewater Treatment Facility prior to being discharged into the receiving stream.
5. This permit is not transferable. In the event there is a desire for the facilities to change ownership,
or there is a name change of the Permittee, a formal permit request must be submitted to the
Division of Environmental Management accompanied by an application fee, documentation from
the parties involved, and other supporting materials as may be appropriate. The approval of this
request will be considered on its merits and may or may not be approved.
6. Construction of the sewers, pump station(s) and force main shall be scheduled so as not to
interrupt service by the existing utilities nor result in an overflow or bypass discharge of
wastewater to the surface waters of the State. --

7. Upon Completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been
installed in accordance with this permit, the approved plans and specifications, and other
supporting materials. Mail the Certification to the Water Quality Permits and Engineering Unit,
P.O. Box 29535, Raleigh, NC 27626-0535.
8. A copy of the approved plans and specifications shall be maintained on file by the Permittee for a
minimum of five years from the date of the completion of construction.
9. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee
to an enforcement action by the Division of Environmental Management, in accordance with North
Carolina General Statute 143-215.6(a) to 143-215.6(c).
10. The issuance of this permit does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances which may be imposed by other government agencies
(local, state, and federal) which have jurisdiction.
11. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required
by this Division, such as the construction of additional or replacement wastewater collection
facilities.
12. Noncompliance Notification:
The Permittee shall report by telephone to the Fayetteville Regional Office, telephone number 910-
486-4874 as soon as possible, but in no case more than 24 hours or on the next working day,
following the occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility incapable
of adequate wastewater transport, such as mechanical or electrical failures of pumps, line
blockage or breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving
waters without treatment of all or any portion of the influent to such station or facility.
Persons reporting such occurrences by telephone shall also file a written report in letter form within
15 days following first knowledge of the occurrence. This report must outline the actions taken or
proposed to be taken to ensure that the problem does not recur.

Dear Mr. Rawls:
In response to your letter dated September 9, 1994, the Town of
Lillington has further reviewed the issue of accepting wastewater
discharge from the Edwards Brothers, Inc., facility expansion.
Based upon this review, the following information has been
ascertained:
Wastewater from the proposed expansion
- Volume of wastewater is 6,400 gallons per day
- Edwards Brothers, Inc., proposed to discharge 800
pounds of hydroquine per year. This equates to 3.07
pounds per day utilizing a 260-day operating
schedule.
- The concentration of hydroquine leaving the new
facility expansion will be 57.5 parts per million.
Combined wastewater from existing facility and new
expansion
The volume of wastewater generated from both
facilities will be 12, 400 gallons per day.
3,000 pounds of hydroquine is currently discharged
from the existing plant each year. This amount,
coupled with the addition of 800 pounds per year
from the new facility, will provide a combined mass
of 14.6 pounds per day.
- The combined concentration of hydroquine will be 137
parts per million.
Concentration of hydroquine at the WWTP using the average
day for the past 12 months
- The average day for the past 12 months is .398 MG.
- The concentration of hydroquine contributed by
Edwards Brothers at the WWTP will be approximately
4.4 parts per million.
The concentration of hydroquine at the WWTP using the
minimum daily flow for the past 12 months
- The minimum daily flow at the WWTP for the past 12
months is .212 MG.
- The concentration of hydroquine, based upon the
minimum daily flow for the past 12 months is 8.3
parts per million.
For concentrations of hydroquine between 10 and 100 parts per
million, the EPA database indicates that wastewater plants can
achieve a 99.9% removal utilizing activated sludge technology.
Given this information, coupled with the ongoing toxicity study
being conducted by McKim & Creed Engineers, PA, I believe that
the addition of this service from Edwards Brothers, Inc., will
not adversely affect the treatment efficiency or further degrade
existing toxicity levels at the wastewater treatment plant.

prisms. It melts at 165°C.

Potential Exposure: Hydroquinone is a reducing agent and is used as a photo-
graphic developer and as an antioxidant or stabilizer for certain materials which
polymerize in the presence of oxidizing agents. Many of its derivatives are used
as bacteriostatic agents, and others, particularly 2,5-bis(ethyleneimino) hydro-
quinone, have been reported to be good antimitotic and tumor -inhibiting agents.
Incompatibilities: Strong oxidizers.
Permissible Exposure Limits in Air: The Federal standard is 2 mg/m3. The Y.h
tentative STEL value is 4 mg/m3. The IDLH level is 200 mg/m3.
Determination in Air: Collection on a filter, workup with acid, analysis by
high pressure liquid chromatography. See NIOSH Methods, Set 2. See also
reference (A-10). A colorimetric method may also be used (A-23).
Permissible Concentration in Water: No criteria set.
Route of Entry: Inhalation of dust, ingestion, eye and skin contact. y .'
Harmful Effects and Symptoms: Local — The dust is a mild primary irritant. }Y
Skin sensitization to the dry solid is very rare but does occur on occasion from
contact with its alkaline solutions. The skin may be depigmented by repeated
applications of ointments of hydroquinone, but this virtually never occurs from
contact with dust or dilute water solutions. Following prolonged exposure to
elevated dust levels, brownish conjunctiva stains may appear. These may be .7
followed by corneal opacities and structural changes in the cornea which may
lead to Toss of visual acuity. The early pigmentary stains are reversible, while the
corneal changes tend to be progressive.
Systemic — Oral ingestion of large quantities of hydroquinone may produce
blurred speech, tinnitus, tremors, sense of suffocation, vomiting, muscular
twitching, headache, convulsions, dyspnea and cyanosis from methemoglob-
inemia, and coma and collapse from respiratory failure. The urine is usually.
green or brownish green. No systemic symptoms have been found following
inhalation of hydroquinone dust.
Points of Attack: Eyes, respiratory system, skin, central nervous system.

Based upon additional information provided by Edwards Brothers, Inc., along with
historical flow data from the Town of Lillington, the following revisions have
been made to the gravity sewer extension application:
The new facility will generate about 6,400 per day of wastewater, 25% of
which will be domestic in nature. The remaining 75% will be industrial.
The new expansion will dispose of 800 pounds of hydroquine per year.
This equates to 3.07 pounds per day (260 operating days). Therefore, the
concentration leaving the new addition will be 57.5 parts per million.

This office has received the subject application and the
following information should be provided to complete the review
process. The application indicates that the flow is 100%
industrial. The flow requested appears to be based on the
calculation 170 employees x 25 gpd resulting in requested 4,250
gpd. This type calculation is normally associated with 100%
domestic waste. Please clarify if the requested flow will be
industrial or domestic waste originating at the industrial
facility. The application also indicates that the waste stream will
have a projected 92 ppm hydroquine concentration. Is the 92 ppm
concentration a historical effluent value or is the concentration
reflective of a increase of the hydroquine loading to the
wastewater treatment facility?
The Town is asked to provide to this office sufficient
information to demonstrate that the additional waste will not
adversely affect the treatment efficiency of the treatment system
or independently or in conjunction with other wastes contribute to
continued effluent limit violations including toxicity.

sewer extension to other utilities and natural features. Specifications may be omitted for
delegated authorities. Each sheet of the plans and the first page of the specifications must be
signed and sealed. Three copies of the existing permit if a renewal or modification.
c. The appropriate permit processing fee, in accordance with 15A NCAC 2H .0205(c)(5).
d. If the owner/authority of the wastewater treatment facility (WWTF) that will be accepting
the wastewater flow from this project is different from the applicant of the project, then a
letter must be provided from the owner/authority of the WWTF specifying the volume of
flow that will be accepted. The letter should be a recent letter and should refer to the project
by the same name as that identified on the application and the plans/specifications.
e. An Operational Agreement (original and two copies) must be submitted if the sewer

The Permits and Engineering Unit has completed a preliminary engineering review of the subject
application. The following items must be addressed before we can complete our review:
1) Please verify that the proposed 92 ppm of hydroquine is acceptable under your current
pretreatment program.
2) Please clarify the source of the hydroquine. Specifically, what quantity of flow will generate
the hydroquine. This flow must be added to the amount of domestic flow listed in question 6
(page 2) of the application. Please revise page 2 to reflect any additional flow to be generated
and forward 3 sealed and signed copies to my attention.
Refer to the subject permit application number when providing the requested information. Please
submit three (3) copies of all information to my attention at the address below. Also,
please note that failure to provide this additional information on or before October 8, 1994 will subject
your application to being returned as incomplete, in accordance with 15A NCAC 2H .0208.
