The following information is provided in response to your letter on April 09, 2021 that was received April 14, 2021,
related to the Town of Clayton's actions to correct and prevent future SSO occurrences:
Incident Number 202101064
This SSO was caused by several pieces of rip rap and clay pipe in the invert of MH 1945 which in turn collected other
debris causing a partial blockage in a typically high flow sewer line leading to the main outfall line along the little Creek.
The Town crews responded within 13 minutes to a call that was reported as a water leak, and quickly determined that it
was an SSO. We were unable to immediately clear the blockage by jetting the line from the downstream manhole, so we
used our Vactor Trucks in the upstream manhole to eliminate the surcharge which stopped the SSO, and access the line
from that direction until we could remove the blockage and return the line to full capacity. On March 22, 2021, town
crews utilized CCTV technology and found the gravity line upstream of the 550 had been damaged by a subcontractor
installing fiber optic conduit for Verizon sometime within the last 18 months, pictures attached. We were able to make
a temporary repair to keep any more debris from entering the gravity sewer line until a permanent repair can be made.
The Town has informed the responsible subcontractor and Verizon that their conduit needs to be moved or acceptable
arrangements made within 5 days or we will begin the necessary repairs.
In the last 3.5 months the maintenance crews have cleaned 9% of the 10% required annually by our collection system
permit. We have a CCTV program with an internal goal to visually inspect a minimum of 15% of the Town of Clayton's
existing gravity sewer lines per year.

Per G.S. 143-215.1 C(b), the responsible party of a c ischarge of 1,000 or more of untreated wastewater to surface waters
shall issue a press release within 24-hours of first knowledge to all print and electronic news media providing general
coverage in the county where the discharge occurred When 15,000 gallons or more of untreated wastewater enters surface
waters, a public notice shall be published within 10 days and proof of publication shall be provided to the Division within 30
days. Refer to the reference statute for further detail,
The Director. Division of Water Resources, may take enforcement action for SSOs that are required to be reported to
Division unless it is demonstrated that:
1) the discharge was cause by sever natural conditi ns and there were no feasible alternative to the discharge: or
2) the discharge was exceptional, unintentional, to porary and caused by factors beyond the reasonable control of the
Permlttee and/or owner, and the discharge could not have been prevented by the exercise of reasonable control.
Part II must be completed to provide a Justification claim for either of the above situations. This information will be the basis

How was the SSO remediated (i.e. Stopped and cleaned up)?
Sewer line was Jetted and MH was vacuumed oit and the debris was removed from the invert. Then all remnants of the
were cleaned up, lime was broadcasted over the affected area as well as flushing the storm drain and catching
