We are trying to coordinate our proposed BOS 200 injection work at the former BP24183 site in Concord for next week.
However, UIC doesn't appear to wantto permit the activities until they coordinate with the RO. I know you looked at the
remedial selection decision document I sent you last week, and you had some questions regarding our injection plan. If
you're available, I'd like to address your questions/concerns and solicit your help to get a firm approval from UIC so we
can proceed with the planned work for next week.

We have not received the hard copy yet but I went ahead and entered this NOi into our system and forwarded an
electronic copy to the regional office for their review. Even though these are deemed permitted sites, they need to be
submitted 2 weeks prior to injection. This give us time to review the package, enter into the system, and give an
opportunity to the regional office a chance review and inspect the site if they wish, which is optional. I asked the RO to
expedite the review process and let me know as soon as possible if they have any issues or wish to inspect or not. I'll let
you know if you can start injection prior to 2 weeks from yesterday. Thanks.

#24183 located at 1579 US Hwy 29N, Concord, Catawba County, NC 28027. The Central Office of the WQROS received
your complete NOi electronically on August 24, 2015.
Please note the following:
Please remember to submit the following regarding t his injection activity:
1) Well Construction Records (GW-1) and Abandonment Records (GW-30) when completed. Please provide copies of
the GW-ls and GW-30s if not already submitted (originals go the address printed on the form). NOTE: Direct push
or Geoprobe wells are considered wells and require construction (GW-1) and abandonment forms (GW-30). If well
construction/abandonment information is the same for the wells, only one form needs to be completed-just
indicate total number of injection points in the Comments/Remarks section of form. These forms can be found on
our website at http://portaLncdenr.org/web/wq/aps/gwpro/reporting-forms.
2) Injection Event Records (IER). All injections, including air and passive systems require an IER. The IER can be
modified for air sparge wells (e.g., air flow 'continuous' for date or rate of injection, etc.).
Also, Please let me know a few days in advance of the injection as I may want to come out and observe.
You can scan and send these forms directly to me at michael.ro gers @ ncdenr.gov, send by fax to my attention at 919-
807-6406, or via regular mail to address below. When submitting the above forms, you will need to enter the nine-digit
alpha-numeric number on the form (i.e., WI0XXXXXX) that has been assigned to the injection activity at this site. This
notification has been given the deemed permit number WI0300301. This number is also referenced in the subject line of
this email. You may if you wish, scan and send back as attachments in re p ly to this email, as it will already have the
assigned deemed permit number in the subject line.

Hey Mike,
Quickie review and MRO doesn't have concerns about this one. We don't plan to inspect the site. BIMS does need to be
updated to reflect that the county for this facility is actually Cabarrus and the latitude should start with 35 (and not 36
which would put this one in the WSRO).

NORTH CAROLINA DEPARTMENT OF ENVIRONMENT AND NATURAL RESOURCES
NOTIFICATION OF INTENT TO CONSTRUCT OR OPERATE INJECTION WELLS
The following are "permitted by rule" and do not require an individual permit when constructed in accordance
with the rules of 15A NCAC 02C .0200. This form shall be submitted at least 2 weeks prior to iniection.
AQUIFER TEST WELLS (1 5A NCAC 02c .02201
These wells are used to inject uncontaminated fluid into an aquifer to determine aquifer hydraulic characteristics.
IN SITU REMEDIATION ( 15A NCAC 02C .0225 > or TRACER WELLS C1 5A NCAC 02c .0229 ):
1) Passive Injection S vstems -In-well delivery systems to diffuse injectants into the subsurface. Examples include
ORC socks, iSOC systems, and other gas infusion methods.
2) Small-Scale Injection O perations -Injection wells located within a land surface area not to exceed 10,000
square feet for the purpose of soil or groundwater remediation or tracer tests. An individual permit shall be required
for test or treatment areas exceeding 10,000 square feet.
3) Pilot Tests -Preliminary studies conducted for the purpose of evaluating the technical feasibility of a
remediation strategy in order to develop a full scale remediation plan for future implementation, and where the
surface area of the injection zone wells are located within an area that does not exceed five percent of the land
surface above the known extent of groundwater contamination. An individual permit shall be required to conduct
more than one pilot test on any separate groundwater contaminant plume.
4) Air Injection Wells -Used to inject ambient air to enhance in-situ treatment of soil or groundwater.

The effects of long-term, low-level exposures to carbon have not been determined. Safe handling of this
material on a long-term basis should emphasize the avoidance of all effects from repetitive acute
exposures.
Persons subjected to excessive dust will be forced to leave area because of nuisance; i.e., coughing,
sneezing and nasal irritation.
CAUTION!!! This material, when wet, removes oxygen from air causing a severe hazard to workers
inside carbon vessels and enclosed or confined spaces. Before entering such an area, sampling and work

Dear Mr, Hvozdik,
AST Environmental, Inc. (AST) appreciates the opportunity to provide this proposal to install
BOS 200th' for the above referenced site. This letter proposal presents our preliminary remedial
approach and price to inject BOS 2005 to address petroleum hydrocarbon contamination in the
groundwater at this site. AST has prepared the injection design based on the data from your
recent email that included historical groundwater analytical summary tables and a site map. The
following is the NC GCL for the site:

SATURATED ZONE IMPACTS
The data used for the saturated zone impacts were the groundwater samples collected from
MW-8R from July 2014. Based on the mast recent groundwater data from MW-8R and MW-14,
the impacted groundwater appears to be concentrated in the shallow saturated zone adjacent to
the northern former dispenser island, Upon review of the boring logs, it is unclear whether the
impacts are confined to the shallow saprolite, or extent into the partially weathered rock (PWR).
Based on the limited saturated soil data, AST is proposing a two phase approach:
1) Phase 1 - Remedial Design Characterization
2) Phase 2 — BOS 200® Installation
The scope of work associated with each phase of the remediation and its estimated cost is
listed below.
REMEDIAL DESIGN CHARACTERIZATION (RDC)
Based on AST's review of the existing data, the impacted saturated zone appears to be located
within the PWR located approximately 15' bgs. Based on the lithology and the inability to
access it via direct push methods, it is proposed to perform supplemental saturated soil, PWR
and groundwater sampling. The data generated will optimize the injection approach to ensure
effective remediation and minimize the cost. This RDC will be used to:

• Determine the mass of contamination residing in the saturated soil and groundwater as
a function of horizontal and vertical location within the proposed treatment area, see
attached figure.
• Refine the conceptual site model.
• Determine the extent of the impacted PWR
• Determine the ability to access the media with various drilling methods.
The supplemental sampling will extend to approximately 35 feet. The scope of work associated
with the RDC effort is as follows:
Soil Borin s
• 1 continuous soil borings, to approximately 15' bgs. The soil borings will be analyzed
every two feet, starting at 4 feet bgs, and from discrete intervals identified to be visibly
stained or with elevated PIO readings. At the soil boring location, a two inch rock core
will be advanced from direct push refusal to the bottom of the PWR (approximately 35'-
38' bgs based on the well construction record of DW-9).
Groundwater Sam plin g
• At the rock core locations, a cluster of four groundwater implants (micro wells) will be
installed. The implant clusters will be screened from 15'-20', 20'-25', 25'-30', and 30-
35'. Implants consist of small diameter (-1/2") polyethylene tubing with holes drilled to
create discrete screened sections. The implants can be installed inside the open
borehole from the completed rock cores. The 4 groundwater implants and MW-BR will
be sampled and analyzed. A total of 13 groundwater samples will be collected and
analyzed.
The soil and groundwater samples will be analyzed by the RPI Group Quality Assurance
Laboratory in Golden, Colorado for volatile organic compounds (VOCs) using Method 8260B
and anions (nitrate, nitrite and sulfate -groundwater samples only). The laboratory analysis is
for remedial design use only and is not for regulatory compliance. The analytical services
performed by RPI Group are provided at no cost to the client.
AST estimates it will require 2 workdays to complete the soil boring, rock core and groundwater
sampling.
The results of this RDC will be used to finalize the BOS 200® loadings throughout the proposed
injection area. The final design loadings will vary with depth and location based on the
distribution of contaminants' mass.
TRAP AND TREAT® BOS 200®
BOS 200® provides a unique opportunity to utilize two proven technologies to effectively
remediate petroleum hydrocarbon sites. The two technologies are 1) the trapping of the
contaminants via carbon adsorption and 2) the subsequent treatment via biological degradation
within the BOS 200® matrix as the product incorporates both aerobic and anaerobic biological
processes.

These two proven and very powerful remediation mechanisms make what is called the "Trap
and Treat" process. The "Trap" provides the immediate mass reduction and plume control, while
the "Treat" provides the continued long-term remedial degradation.
The product comes as a fine-grained dry material which consists of: carbon, calcium sulfate,
nitrate, phosphate, and ammonia in a proprietary blend. BOS 200® is 77% by weight carbon
and up to 19% gypsum, the sulfate source. Gypsum is 79% by weight sulfate which translates to
approximately 15% by weight sulfate in BOS 200®. The BOS 200® is mixed with water and a
facultative blend of microbes (inoculation with aerobic and anaerobic microbes) to create a
solids suspension. This is now an ideal environment for the biological process, where
hydrocarbons are adsorbed on to BOS 200® particles made up of:
• Electron Acceptors: oxygen, nitrate, ammonia and sulfate (primary)
• Nutrients -phosphorus and nitrogen
• Aerobic and anaerobic blend of microbes (over 27 species of microbes)
One of the advantages of the product is that the design approach can vary from a treatment
objective where a complete immediate contaminant mass removal from the groundwater is
achieved, to a partial contaminant mass reduction working in conjunction with the biological
process driving the groundwater cleanup effort, or any number of somewhere in between. The
end effect is that plume can be controlled in a short period of time and treatment can be
extended over a longer period of time.
It should be noted that there is a lower limit of the amount of BOS 200® that can installed and
still be effective at a site. This amount is driven by a number of site features such as soil type,
groundwater flux, and contaminant concentrations.
The success in achieving cleanup goals is not just in the product installed, but the distribution of
the product in subsurface. Distribution is controlled by the injection techniques used: i.e., vertical
and horizontal spacing as a function of lithology, high pressure injection vs. low pressure
injection, and top down vs. bottom up.
It is expected that the injection pressures will vary from 200 to 600 psig (measured at the
discharge of the injection pump -the injection system pressure losses are vary from 190 to 500
psig depending on length and diameter of hoses and injection tip geometry needed). The
discussion of the vertical and horizontal injection spacing is provided below for each of the
injection areas.
BOS 2oo®TREATMENT AREA -SEE ATTACED FIGURE FOR TREATMENT AREA
Due to the inability to direct push in the impacted media, AST recommends the following
injection technique:
1. A rotosonic rig will advance the borehaole and casing for each injection point to the top
of the PWR.
2. A combination 4" and 6" sonic tooling will advance one foot into the PWR, the 4" tooling
will be removed and the outer 6" casing will be flushed with water to remove drill
cuttings.
3. The 6" tooling will be pulled up one foot and a 6" inflatable packer w injection tip will be
lowered to expose an in injection tip to the desired PWR interval (~one foot of exposed
PWR below the casing).

capable of 65 gpm at 1,200 psi. Once the injection is complete, the pressure
transducers in the packer will be monitored to ensure backpressure from the formaition
has reduced such that the likelihood of surfacing BOS 200® slurry is minimized.
5. Once the packer is removed, the 4" tooling will be returned to the hole and advanced
another 2' into the PWR to repeat the injection process.
Visual observations will be conducted to determine the horizontal distribution of the BOS
200® injection. The specific design for the saturated zone for treatment area is as
follows:
Pilot Injection Area (Defined by MW-BR groundwater benzene concentration -11,800 ug/L)
• Total Treatment Area: 400 ft2
• Number of Injection Points & Horizontal Spacing: 4 points on 1 O' centers
• Total Number of Injections and Vertical Spacing: 22 injections; 15'-25' vertical treatment
zone (alternate 15'-25' bgs and 16'-24' bgs)
• Contaminant Loading Used: 2.4 x10 4 lbs. benzene/ft3 (based on historical groundwater
data)
• BOS 200® Loadings & Amount Total Amount per Area: ~0.28 lbs. BOS 200®/ft3 or ~50
lbs. of BOS 200® per Injection (25 gallon shot volume)= 2,000 lbs. of BOS 200®
• Sulfate loadings & total amount per area: 0.08 lbs. sulfate/ft3 or ~13.6 lbs. of sulfate per
injection (25 gallon shot volume) = 300 lbs. of sulfate
• Amount of Bacteria Concentrate: 5 gallons of bacteria concentrate.
POST INJECTION SAMPLING
Following the injection effort AST will collect groundwater samples from MW-BR prior to
demobilizing. AST further recommends performing progress groundwater sampling events at
intervals of one month post-injection and quarterly for one year after the injection event for a
total of five (5) groundwater-monitoring event. AST understands that this will be performed by
others. The performance samples can be sent to Remediation Products, Inc. Laboratory in
Golden, Colorado for no cost of analyses. The cost of shipment will be the responsibility of
AECOM. The analyses will be consistent with that described previously (VOCs and anions).
The anions will track the biological processes.

ASSUMPTIONS
AST has made the following assumptions regarding this approach:
1. AST will support AECOM in preparing the UIC permit.
2. Pricing of the BOS 200® is based on the quantities outlined above. Should any of the
Proposed Injection Areas be removed from this design, pricing of the BOS 200® must be
adjusted to conform to the pricing structure shown at:

3. There will be adequate water onsite (75-gpm service) to prepare the BOS materials for
injection. The water service within 200' of all injection locations and fire hoses can be
placed and remain in-place for the duration of the injection. Shuttling of water is not
required. The water will be provided at no cost to AST. Hydrant permits and fees will be
provided by others.
4. If water is not available at the site, a water truck will be provided for $700/day.
5. The site stratigraphy allows for boreholes within the PWR to remain open after drilling.
6. Work is performed during standard workweek and hours (Monday-Saturday 7am-6pm)
7. Asphalt/concrete penetrations will be patched with like materials.
8. All utilities and underground appurtenances will be located prior to AST arriving on site.
AST has not included any provisions to perform subsurface utility clearance (such as
hand auger boreholes or contracted utility location services). Hand augering and/or other
invasive utility clearances such as air knife will be performed at a minimum of 72-hours
prior to mobilization of AST Injection Staff and this service is not included or priced in
this proposal. This can be provided by AST for an additional cost. AST will not be
responsible for repairs to mis-marked or unmarked buried utilities and other
appurtenances.
9. Boring logs will be prepared by others during the injection effort.
10. The total depth throughout the area identified in the injection design will be accessible
via direct push utilizing a Geoprobe® 7000 series rig.

Payment terms are full payment of product and shipping costs upon arrival. Payment of
injection services is not to exceed 30 calendar days frorn date of invoice. Interest will accrue at
a monthly rate of 1.5% for all outstanding balances including interest. The interest rate will be
