Thanks for the additional information. Based upon our phone conversation and looking at the plans further, this does
meet the criteria of a UIC Class V stormwater well system as it collects stormwater runoff, including from the roof top,
and diverts to infiltration gallery. Below is screen shot of the UIC stormwater rule readopted effective 9/1/19. There is
also a link to EPA's UIC website. And attached is a revised updated Notification form.
Also, please reply via response with by this email within 30 days whenever the system is constructed as it will change the
status of the UIC stormwater system from proposed to active in our database.

15ANCAC 02C .0227 STORMWATERDRAINAGE WELLS SYSTEMS
(a) Stormwater Drainage Wells Systems means well systems that receive the flow of water that occurs during rainfall or a snowmelt
event.
1
(b) The following Stormwater Drainage Wells Systems shall be permitted by rule pursuant to Rule .0217 of this Section:
(1) systems designed in accordance with stormwater controls required by federal laws and regulations, State statutes and
rules, or local controls; and
(2) infiltration systems, which receive storrnwater from roof tops.
(c) Nothing in this Rule shall be construed as to allow untreated stormwater to be injected directly into any aquifer or to otherwise result
in the violation of any groundwater quality standard as specified in 15A NCAC 02L.
(d) Reporting. Within 30 days of a change of status of the well drainage system, the owner or operator shall submit the following
information to the Division:
(1) the facility name, address, and location indicated by either:
(A) latitude and longitude with reference datum, position accuracy, and method of collection; or
(B) a facility site map indicating property boundaries;
(2) the name, telephone number, and mailing address of owner or operator;
(3) the ownership of facility as a private individual or organization, or a federal, State, county, or other public entity;
(4) the number of injection welts drainage and collection systems; and
(5) the well injection system status as proposed, active, inactive, temporarily abandoned, or permanently abandoned.

(land surface) where the stormwater infiltration trench is placed is 2281 +/-. The elevation at the bottom of the
proposed infiltration trench sand layer is 2274.66 (6.34 ft depth)
Attached are the Plans and the WO. trench detail section to the left in a PDF format. Borings were done to 20 ft.
depth on the site near the proposed WO infiltration trench and ground water was not encountered. I have also
attached detail sheet c4 as requested.
