the permanent discharge of clean fill material including a single culvert and rip rap into 148 linear feet of
stream channel and 0.23 acre of adjacent wetlands for a single subdivision crossing.

Under the conditions of the Nationwide Permit, mitigation is required for your project. The mitigation
proposal submitted with your application is insufficient.(see remarks.)
Your application did not include a statement explaining how avoidance and minimization for losses of waters
of the U.S. were achieved on the project site. (see remarks)
Your submitted project plans or maps were insufficient, too large, or not legible (see remarks).
Your application did not include a delineation of affected special aquatic sites, including wetlands, vegetated
shallows, and riffle and pool complexes as required.
You must submit a copy of your application to the NC Wildlife Resources Commission (WRC) since your
proposed work is in a designated trout water county (see remarks section below for the address of your WRC
representative)

REMARKS:
1.Nationwide Permit General Condition #23(Mitigation)(a), states, “The activity must be designedand
constructed to avoid and minimize adverse effects, both temporary and permanent, to waters of the
United States, to the maximum extent practicable at the project site”. Your plans include a rip rap
dissipation pad that is approximately55feet long. Similar crossings that have been previously
permitted typically do not require this length of rip rap. Please indicate if any other methods to
reduce velocitywere evaluated, (such as pre-formed scour holes)and if so, thereason these methods
cannot be used to reduce stream impacts.
2.The only overall site plan submitted with the application was labeled“Site Plan Previously Approved
by the City of Greensboro”. Please provide a current site plan for the site or indicate if the previously
approved plan is the same as the current plan.
The following are not requirements for the Pre-construction application to be considered
complete; however, it is provided for your information only:
1.Evans Engineering is listed as the applicanton the Pre-Construction Notification form. Please be
aware that if a Nationwide Permit verification is issued, it will be issued to the applicant. The

applicant is responsible for compliance with all the Nationwide permit general and special conditions
including the compensatory mitigation requirements. Please provide an updated application,
including a new signature page,should the applicant change.
