On Friday, April 30th, 2021, Alex Lowe of the WSRO conducted a routine CEI at Jacob's Creek Nursin
and Rehabilitation Center WWTP (NC0044750). ORC Clifford Cain was present for the inspection.
A copy of the permit was available for review on site. The permit was instituted on 10/01/2017, and is
set to expire on 05/31/2022. The facility is as described in the permit, though the sand filter is currently
not in use according to Mr. Cain.
Records were well -organized and maintained. A random check of three reporting months in 2020
showed no discrepancies between data collected by Pace Analytical Labs and data reported on the
eDMR, and all instances of permit exceedances were reported. Laboratory and eDMR data for past
years are stored at Pace Labs, according to Mr. Cain. ORC visitation logbooks were available for
review, with records dating back to 2018.
Treatment processes appeared to be well -maintained and operating as designed. No shortfalls in
maintenance, cleaning, or operation were observed during the inspection. When asked about the
owner's willingness to fund maintenance and repairs for the plant, ORC stated that he believes that the
resources would be provided if requested.
EQ well is without an alarm, but a bypass to the digester is installed to handle any spikes in flow. After
EQ well, waste is split between two treatment trains.
According to Mr. Cain, digester solids are wasted two to three times per week into aerated solids
holding tank, and solids are removed and trucked away twice yearly.
Clarifiers appeared relatively clean, with very little scum or algae growth around the weirs.
A flow meter after the clarifiers was observed to be operational, and a certification of calibration was
present, dated 04/06/2021.
Mr. Cain stated that UV bulbs are cleaned daily, and replacement bulbs are kept onsite for
maintenance. Furthermore, the original chlorine disinfection system is maintained as a redundancy in
case of UV failure. Proper tubes and wastewater grade tablets are kept onsite.
The small post -treatment aeration basin used to increase DO levels before discharge was clean and
free of solids.
All observations made in this inspection indicate proper operation of the plant and compliance with the
permit.

Comment: DMR and laboratory results provided for all of 2020. Random checks completed on six days
throughout August -December 2020. No discrepancies were found and all parameter
exceedances were reported.
