It is also near the septic systems from the adjacent single family residences that may be influencing the well. MW-6 is located within the review boundary and is located immediately adjacent to the infiltration site 1, which has the potential to influence the MW-6. Pursuant to 15A NCAC 02L .0108, please contact Geoff Kegley, our Wilmington Regional Office staff at geoffkegleykncdenr.g_ov, to determine the appropriate locations for MW-5 and MW-6 so that both wells are on the review boundary and are representative of the groundwater influenced by the WWTF operations. According to the recent inspection, abandonment of MW-1 has not been completed. Pursuant to 15A NCAC 02C .0113, a schedule will be added to the permit and require that MW-1 be permanently abandoned by a North Carolina Certified Well Contactor. B. Site May: 1. The site map submitted (see attached) did not properly identify the location of the existing digester tank. The infiltration sites should be identified and label as well. Pursuant to 15A NCAC 02T .0105(d), please provide an updated site map. The site map should include the following information when applicable: a. Legend, north arrow, and scale b. Topographic contours on all disposal sites c. Soil mapping units on all disposal sites d. All facility -related structures and fences within the treatment, storage, and disposal areas e. All habitable residences and places of assembly within 500 feet of all treatment, storage, and irrigations sites f. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds, ditches, and other surface drainage features within 500 feet of all waste treatment, storage, and disposal site(s) g. Latitude and longitude coordinates of all monitoring wells (decimal degrees to 6 digits) h. Location and identification of major components of the waste disposal system i. The wetted perimeter of all irrigation fields with field names (named according to the approved permit) j. Location and ownership of property boundaries within 500 feet of the disposal area (including road/rail right-of-ways) k. The delineation of compliance and review boundaries 1.
