A letter of request for. an Authorization to Construct was received May 8,
1989, by the Division and final plans and speci`fcat:ions for the subject. project
have been reviewed and found to be satisfactory. Authorization is hereby granted
for the construction of an. addition to the City of Winston-Salem's Lower Muddy Creek
Wastewater Treatment Plan.t•consisting of a pH monitoring device, a 10,000 gallon
caustic storage tank and caustic feed lines.
This Authorization to Construct is issued in accordance with Pa.r.t. III paragraph
B of the NPDES Permit No. NC0050342 issued May 31, 1989, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with. the conditions and limitations specified in. Permit No. NC0050342.
Upon 50% completion of the subject facilities the Permittee must employ a
certified wastewater operator in accordance with. Part III paragraph C of the
referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approva.ble by the North. Ca.rolin.a.
Division of Environmental Management.
The Winston-Salem.Regi_onal Office, telephone number 919/761-2351 shall be
notified at least twenty-four (24) hours in advance of operation of these
facilities. Such notification. to the Regional Supervisor shall be made during the
normal office hour. from 8:00 a.m. until 5:00 p.m. on Monday through Friday,
excluding State Holidays.

In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional
wastewater treatment and disposal facilities.
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer certifying
that the permitted facility has been installed in accordance with the approved plans
and specifications.
A set of approved plans and specifications for the subject project must be
retained by the applicant for the life of the project.
