Our records indicate that you have not paid your annual permit fee of $2,865.00 for the year 2007. Failure to pay fees
violates your NPDES permit [see permit Part II. B.14].
Attached please find a copy of invoice (2007PR002509) previously sent by the Division's Budget Office. Failure to
pay your annual fees is grounds for the State of North Carolina to revoke your permit, (see, Parts II. B. 13 and II. B.
14).
This letter serves as final notice - you must resolve this matter within 30 days of receiving this letter. If you fail to
pay your fees before Monday, April 6, 2009, the Department will refer this matter to the North Carolina Attorney
General's Office for collection through the courts, and further actions may be initiated against your permit.
Please make your check payable to NCDENR in the amount of $2,865.00 and include the subject permit number to
ensure proper credit. Send your check to the attention of Fran McPherson, DWQ Budget Office, 1617 Mail Service
Center, Raleigh, NC 27699-1617. If you have written proof that this fee has already been paid, please FAX it to me,

This annual, feeds required by the North Carolina Administrative Code. It covers the administrative costs associated with your
permit. It is required of any person holding a permit for any time during the annual fee period, regardless of the facility's operating
status. Failure to pay the fee by the due date will subject the permit to revocation. Operating without a valid permit is a violation
and is subject to a $10,000 per day fine. If the permit is revoked and,you later decide a permit is needed, you must reapply, with the
understanding the permit request may be denied due to changes in environmental, regulatory, or modeling conditions.
