Dear Mr. Couch:
The final plans and specifications for the subject project have been reviewed
and found to be satisfactory. Authorization is hereby granted for the construction
of modifications to an existing 6,000 GPD wastewater treatment plant consisting of
a new 7,200 gallon septic tank, a new 720 square foot filter, a distribution box,
and a chlorine contact chamber to serve the Comm/Scope Company.
This is a Class I Wastewater Treatment Facility and the person in responsible
charge must hold a valid Grade I Certificate.
This Authorization to Construct shall be subject to revocation unless the waste-
water treatment facilities are constructed in accordance with the conditions and
limitations specified in Permit No. NC0034754.
One (1) set of approved plans and specifications is being forwarded to you.
