Failure to submit all required items will necessitate additional processing and review time. For more information, visit the System -wide Collection System Permitting website: General— When submitting an application to the Pretreatment, Emergency Response, & Collection Systems (PERCS) Unit, please use the following instructions as a checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided boxes will help produce a quicker review time and reduce the amount of requested additional information. The Applicant shall submit one original and one copy of the application and supporting documentation. A. Cover Letter ® Submit a cover letter listing all items and attachments included in the permit application package B. No Application Fee Required ➢ No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit ➢ The appropriate annual fee for systemwide wastewater collection system permits may be found at: ➢ Annual Non -Discharge Fees C. System -Wide Wastewater Collection System (FORM: CSA 04-16) Application: ® Submit the completed and appropriately executed System -wide Wastewater Collection System (FORM: CSA 04- 16) application. Any unauthorized content changes to this form shall result in the application package being returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long as the attachments are numbered to correspond to the section and item to which they refer. ❑ If the Applicant Type in Section L3 is a Privately -Owned Public Utility, provide the Certificate of Public Convenience and Necessity (CPCN) from the North Carolina Utilities Commission demonstrating the Applicant is authorized to hold the utility franchise for the area to be served by the wastewater collection system, or ❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an application for a franchise has been received and that the service area is contiguous to an existing franchised area or that franchise approval is expected. ❑ If the Applicant Type in Section L3 is a corporation or company, provide documentation if it is registered for business with the North Carolina Secretary of State. D. General Information: ➢ The Authorized signing official listed in Section L4 should match with that of the Applicant certification page in accordance with 15A NCAC 02T .0106(b). Per 15A NCAC 02T .0106(c), an alternate person may be designated as the signing official if a delegation letter is provided from a person who meets the criteria in 15A NCAC 02T .0106(b). ➢ NOTE - Public Works Director's are not authorized to sign this permit application according to the rule unless they are delegated. The goals with respect to responding to SSOs are: • Respond quickly to minimize the volume of the SSO; • Eliminate the cause of the SSO; • Contain the spilled wastewater to the extent feasible; • Minimize public contact with the spilled wastewater; • Mitigate the impact of the SSO; and • Meet the regulatory reporting requirements. These procedures will be reviewed annually, or whenever revised guidance from county, state or federal authorities dictate a revision is necessary. Sewer System Overflow (SSO) Reporting Procedures 1. Upon discovery or report of a possible S.S.O. during normal operating hours the Public Works Department should be notified immediately. 2. Any S.S.0 discovered after normal operating hours should be reported to the Banner Elk Police Department and they will notify the necessary personnel. 3. If the employee on site requires assistance from other departments within the town or the assistance from surrounding town maintenance personnel, the Town Manager will be contacted and informed of the situation and the type of assistance needed. 4. The ORC will be the primary interface between the Town of Banner Elk and the State of North Carolina Division of Water Quality and the Avery County Health Department on all 5.5.0 matters. 5. The ORC will report overflows from any sewer line or pump station to the North Carolina Department of Environmental and Natural Resources, Division of Water Quality. This requirement applies in the following cases: • Any spill of 1,000 gallons or more; or • Any spill regardless of the volume, if any waste reaches the surface waters of the State. Please note that conveyances such as drainage ditches and storm sewers are considered waters of the State. 6. The ORC is required to provide a verbal report regarding reportable spills to the Asheville Regional Office as soon as possible, but in no case later than 24 hours following the spill event. Voice mail messages or faxed information shall not be considered as the initial verbal report. • Additionally, if a spill occurs after normal business, the event must be reported during the next working day. Should a spill occur after hours which reaches the surface waters and/or may be considered as either an environmental or health hazard, the report must be made immediately to the Division of Emergency Management At 1-800-858-0368 or 1- 919-733-3300. Their office will contact appropriate Division of Water Quality staff; • A written report must be submitted to the Asheville Regional Office within five (5) days of the time that the Town of Banner Elk becomes aware of the spill occurrence. All parts of the form must be completed in detail and submitted either at the time of the verbal report or within the required five (5) days. It is highly recommended that the form be faxed to the regional office, then followed up by a telephone call to confirm the fax has been received and to provide any additional information that may be needed; • In the event of a discharge of 1,000 gallons or more of untreated waste to the surface waters of the State, issue a press release to all print and electronic news media that provide general coverage in the county where the discharge occurred setting out the details of the discharge. The owner or operator shall issue the press release within 24 hours after the owner or operator has determined that the discharge has reached the surface waters of the State. The owner or operator shall retain a copy of the press release and a list of the news media to which it was distributed for at least one year after the discharge and shall provide a copy of the press release and the list of the news media to which it was distributed to any person upon request; • In the event of a discharge of 15,000 gallons or more of untreated waste to the surface waters of the State, publish a notice of the discharge in a newspaper having general circulation in the county in which the discharge occurs and in each county downstream from the point of discharge that is significantly affected by the discharge. The Secretary shall determine, at the Secretary's sole discretion, which counties are significantly affected by the discharge and shall approve the form and content of the notice and the newspapers in which the notice is to be published. The notice shall be captioned "NOTICE OF DISCHARGE OF UNTREATED WASTE". The owner or operator shall publish the notice within 10 days after the Secretary has determined the counties that are significantly affected by the discharge and approved the form and content of the notice and the newspapers in which the notice is to be published. The owner or operator shall file a copy of the notice and proof of publication with the Department within 30 days after the notice is published. Publication of a notice of discharge under this subdivision is in addition to the requirement to issue a press release under subdivision. Containment Procedures 1. As soon as the employee arrives on site of the S.S.O. their priority will be to contain the spill as to not allow it to reach surface waters. If there Is no danger of the S.S.O. reaching surface waters, contain the S.S.O. to as small of an area as possible. 2. Then priority will pivot to securing the S.S.O. area with yellow caution tape to prevent any unauthorized persons from entering the contaminated area. This includes the banks of the surface waters the S.S.O has reached. 3. Containment shall be accomplished in any number of ways including but not limited to the construction of; ditches, ponds, or dams. The sewage contained in the containment areas shall be pumped back into the sewer system or transported to the WWTP by a pumper truck. 4. Trash pumps capable of handling the flow of the sewer line are to be placed at the overflowing manhole and the sewage pumped in to the next downstream manhole until the blockage is corrected. An employee must remain onsite until the pumps are no longer in operation. Blockage Correction Procedures 1. As Soon as The S.S.O. has been contained and a bypass pumping system has been set up attempt to eliminate the blockage by first checking for problems in the invert of the overflowing manhole. The blockage will be eliminated by utilizing the Town's Equipment and outside contractors (if needed). Containment and bypass pumping are to remain operational until the blockage is corrected. Clean-up Procedures At all S.S.O. sites regardless of their surroundings any paper, plastic or any other non -decomposing material should be removed from the site, these materials should be bagged in a container clearly marked biohazard and transported to the WWTP. All maintenance personnel are responsible for following safety procedures at all times. Special safety precautions must be observed when performing sewer work. Special consideration should be given to following all local traffic, confined space, and safety procedures. On Asphalt or Concrete 1. Set up containment at or near the storm drainage system; do not let the spill enter the storm drain if possible. 2. Put up warning tape around the S.S.O area. 3. Spread lime on spillage and sand to a depth of 1 inch. Let this stand for 1 hour and sweep up. If moisture or grease is still present, repeat procedure. 4. After removing the contaminated material. Wash down the area with high pressure water and Clorox. Pump this back into the sewer system. Transport wet sand to drying beds at WWTP. 5. If this spill goes into the storm drainage system, refer to Creek and Stream procedures. In Creek or Stream 1. If possible a dam needs to be constructed near a manhole or at a location which is accessible to a vacuum or pumper truck. Build the dam using dirt that is capable of handling creek and hydrant water without dam failure. Station two pumps at the darn that can handle the flow of the creek. Use one pump for back-up. Have a back hoe on site with extra material at all times so dam can be rebuilt if breach occurs. There will be two workers at the pump site at all times during pumping operation. Purnp effluent from the dam into nearest manhole. 2. Put up warning tape from the S.S.O. site to the pump site on both sides of the creek in populated areas. In unpopulated areas restrict access where there is any chance of humans encountering the S.S.O. 3. Flush the Stream with water from a sufficient number of hydrants to remove waste product from the creek. Have Wastewater Operator pull samples. Continue to pump and flush stream until all fecal coliform tests come back 500 colonies/ml or less. Pull samples every 24 hours until this reading is obtained. Pull samples 100 feet above S.S.O., at the site, and at 400 feet downstream of the pump site. 4. In Rip Rap, wash down with high water pressure until there is no waste residue remaining on, or with the rim rap structure. 5. If sludge is found in creek, broom creek bottom and sides until there is no waste residue remaining. 6. If creek flows into a body of water used for recreation, and or drinking, notify the affected county and local officials as soon as the problem is discovered. If this happens, refer to pond, lake procedure. In Pond or Lake 1. Put up Warning tape, black on red at the shoreline of the contaminated area. 2. In a pond, have WWTP operator collect fecal coliform samples and dissolved oxygen readings around the entire circumference of the body of contaminated water. Set up pumping operations below the dam. 3. In a large lake, Have WWTP operator collect fecal coliform samples 2 feet off the bottom. Dissolved oxygen readings are to be done 1 foot under the surface. Take equal sampling amounts around the circumference of the contaminated body of water. 4. When the first sampling results come in, contact the North Carolina Department of Water Quality for appropriate action if the fecal coliform counts are greater than 8,000 colonies/ml. 5. Continue pumping until 500 colonies/ml or less are obtained in the body of water. 6. Keep monitoring fecal coliform counts until the North Carolina Department of Water Quality or the Avery County Public Health Department says that monitoring is no longer required. Outfall 1. Put up warning tape, "Danger Do Not Enter," signs around the S.S.O site. 2. In open area, lime heavily and till. Repeat procedure. Seed and then put down straw. 3. In an area where there is undergrowth or small trees, wash area with large amounts of water. Push towards the containment area and pump site. Do this until no waste remains. Wait for 12 to 24 hours then lime the are heavily. Do not lime when the weather forecast is predicting rainfall. In Citizens Yard 1. Till the area whenever possible. Apply lime, seed, and straw. 2. If not able to till, apply lime and two inches of topsoil and mix thoroughly. Lime again lightly, then seed, and cover with straw. J Q .r e 828-733-2776 0 • 0 • — J Z f--_- E 0 Seven Devils -r n Beech Mouniji❑ 828 387-1703 WWTP AND COLLECTION SYSTEM EMERGENCY RESPONSE PLAN Normal operating procedures during power failure • Check generators at WWTP and Elk River Pump Station to make sure they have started and operating properly. • Do a walk around at the WWTP to ensure all pumps, blowers and other mechanical equipment is operating properly. • Check fuel level in tanks at generator sites every 12 hours and log fuel usage and use the figures to estimate how much time is left before fuel runs out. • If fuel tank at Elk River Pump Station falls below 40% contact propane supplier and set up an emergency delivery. • If fuel tank at WWTP falls below % tank contact Gouge oil Company and set up an emergency deliver, if needed fuel can be transported from Public Service Building to WWTP. • Check all pump stations and generator sites every six hours. • If a RAS, or EQ pump station fails to operate contact a sewer pumping service and have the station pumped ASAP and problem can be resolved. • If Elk River Pump Station fails and pumping service cannot be reached or is delayed the water main that feeds the eastern part of town can shut off to help reduce the influent into the pump station. • Contain any overflows. (See S.S.O Clean-up and Pump Procedures) Operating procedures if a pump fails at Elk River Lift Station • Turn pump that has failed into the off position in the control panel. (The remaining pump should continue to cycle and keep the lift station at a satisfactory level.) • Remove failed pump, replace with new spare pump that is stored in building at the WWTP. • Wash down failed pump and send to electric motor repair shop. Operating procedures if both pumps fail at Elk River Lift Station • Call sewer pumping service, have them pump the basin ASAP. • Domestic water main can be shut off for the East side of town at intersection on Hwy 194 and Beech Mtn. Parkway to reduce influent at Elk River pump station. • Turn both pumps off in control panel. • Remove both failed pumps, replace with new spare pumps stored at WWTP. • Wash down failed pumps and send to electric motor repair shop. Operating Procedures in event of sewer main blockage. • In the event of a main line blockage the tow behind sewer line jetter will be used to remove the blockage and restore the sewer to it normal flow. • If the blockage is causing an S.S.O a bypass pump can be set up by public service staff to divert the flow until the blockage is removed. • If the up stream and downstream manholes are located to far apart for a bypass pump set up a sewer pumping service will be called to transport from overflowing manhole to next manhole not affected by the blockage. • After the blockage is removed and sewer flow is back to normal any S.S.O that has occurred will be assessed, reported, documented, and cleaned up per the S.S.O response plan and clean-up procedures. **PLEASE NOTE BANNER ELK HAS PUBLIC SERVICE EMPLOYEES ON A ROTATING** STAND-BY SCHEDULE YEARROUND. THIS EMPLOYEE SHOULD BE ABLE TO RESPOND TO ANY INCIDENT WITHIN 30 MINUTES OF BEING NOTIFIED. EMERGENCY FUND AVAILIBILITY • If an event occurs that would render necessary more funds than were budgeted for the operation of the Elk River Lift Station or Collection System the Public Service Supervisor would request the Town Manager present the Town Council with a budget amendment that would cover the expense of the event, also the Town has established a reserve fund that could be used in an emergency event.
