January 9,2O14, we are forwarding herewith Permit No. VVQ003692I, dated February 14, 2O14'tothe
Town of Harrisburg for the construction and operation of the subject wastewater collection system
extension. This permit shall be effective from the date mfissuance until rescinded and shall be subject
to the conditions and limitations as specified therein. This cover letter and supplement shall be
considered a part of this permit and are therefore incorporated therein by reference.
Please pay particular attention to the following conditions contained within this permit:

amended, and other applicable Laws, Rules, and Regulations, permission is hereby granted to the
Town of Harrisburg

for the construction and operation of approximately 1,440 linear feet of 8-inch gravity sewer to serve 23
lots within the Courtyards at Harrisburg project and the discharge of 5,520 gallons per day of collected
domestic wastewater into the Town of Harrisburg existing sewerage system, pursuant to the application
received January 8, 20.14, and additional information received on January 9, 2014, and in conformity
with 15A NCAC 2T; the Division's Gravity Sewer Minimum Design Criteria adopted February 12, 1996 as
applicable; the Division's Minimum Design Criteria for the Fast -Track Permitting of Pump Stations and
Force Mains adopted June 1, 2000 as applicable; and other supporting data subsequently filed and
approved by the Department of Environment and Natural Resources and considered a part of this.

The Town of Harrisburg is hereby authorized to:

Permitting of this project does not constitute an acceptance of any part of the project that does not
meet 15A WCA[2T; the Division's Gravity Sewer Minimum Design Criteria adopted February 12' 1995 as
applicable; and the Division's Minimum Design Criteria for the Fast -Track Permitting ufPump Stations
and Force Mains adopted June l, 2800 as applicable, unless specifically mentioned herein. Division
approval is based on acceptance of the certification provided by a North Carolina -licensed Professional
Engineer inthe application. It shall be the, Pe,mittee'srespunsihi|ity to ensure that the as -constructed
project meets the appropriate design criteria and ruIes,
Construction and operation is contingent upon compliance with the Standard Conditions and any Special
Conditions identified below,
1. SPECIAL CONDITIONS
L This permit shall become voidable unless the agreement between the Town o[Harrisburg and the Water and
Sewer Authority of[abarrus County for the collection and final treatment of wastewater is in b/U force and
eff ect,

system until an application for permit modification for an increase in flow has been submitted /o and
approved by the Division of Water Resources.
3, Gravity sewers installed greater than ten percent below the minimum required slope per the Division's Gravity
Sewer Minimum Design Criteria shall not be acceptable and shall not be certified until corrected, If there is an
unforeseen obstacle in the field where all viable solutions have been examined, a slope variance can be
requested from the Division with firm supporting documentation, This shall be done thmugh a permit
modification with tee. Such variance requests will be evaluated on aca»ebycase basis. Resolution ofsuch
request shall be evident prior to completing and submitting the construction certification,
III. STANDARD CONDITIONS
L This permit shall net be transferable, In the event there is a desire for the wastewater collection facilities to
change ownership, o, there is a name change of the Pe,nnittee, a formal permit request shall besubmitted tu
the Division accompanied by documentation from the parties involved, and other supporting materials as may
be appropriate, The approval of this request shall be considered on its nnehrs and may or may not be

4. The wastewater, collection facilities shall be properly maintained and operated at all times. The Permkor

8. Overflows and bypasasshaU be reported to the appropriate Division regional office in accordance with
15AN[A[2B0506(a), and public notice shaP be provided as required by North Carolina General Statute
§143-715�1[.
h. A Grease Control Program is in place as follows:
L For public owned collection systems the Grease Cemtnz| Program shall include at least biannual
distribution of educational materials for both commercial and residential users and the legal means to
require grease interceptors at existing establishments. The plan shall also include legal means for
inspections of the grease interceptors, enforcement for violators and the legal means to control
grease entering the system from other public and private satellite sewer systems.
2. For privately owned collection systems, the Grease Control Program shall include at least bi-annual
distribution of grease education materials to users of the collection system by the permiuee or its
representative.
3. Grease education materials shall be distributed more often than required in Parts (l)amd (2) of this
Subparagraph if necessary to prevent grease -related sanitary sewer overflows.

5. Noncompliance Notification:

working day, following the Occurrence or first knowledge of the occurrence of either of the following -
a. Any process, unit failure, due to known or unknown reasons, that renders ,he facility incapable of
adequate wastewater transport, such as mechanical or electrical failures of pumps, line blockage or
breakage, etc.,m/

c� Any SSQ and/or spill, regardless ofvolume, that reaches surface water

to the wastewater collection system extension prior to completion of this Engineer's Certification shall be
considered a violation of the permit and shall subject the Permittee to appropriate enforcement actions,
If the permit is bsued to a private entity with an Openukmd Agreement, then a copy of the Articles of
Incorporation, Dedaration s/Covena nts/R estrictio ns, and Bylaws that have been appropriately filed with the
applicable County's Register of Deeds office shall be submitted with the certification,
A complete certification is one where the form |sfully executed and the supporting documents are provided as
applicable. Supporting documentation shall include the following:

of the wastewater collection system extension, Final record drawings should be clear on the p|ans or on
digital media ([0 or DVD disk) and are defined as the design drawings that are marked up or annotated
with after construction information and show required buffers, separation distances, material changes,

Prior to Certification (Final or Partial): Permit modifications are required for any changes resulting if) non-
compliance with this permit (indudin8 pip* length increases of 18% or greater, increased flow, pump station

design capacity design increases of 5% or greater, and increases in the number/type of connections),
regulations, or the Minimum Design Criteria, Requested modifications or variances to the Minimum Design
Criteria will be reviewed on a case -by -case basis and each on its own merit. Please note that variances to the
Minimum Design Criteria should be requested and approved during the permitting process prior to
construction. After -construction requests are discouraged by the Division and may not be approved, thus
requiring replacement or repair prior to certification & activation.
A copy of the construction record drawings shall be maintained on file by the Permittee for the life of the
wastewater collection facilities,
9. Failure to abide by the conditions and limitations contained in this permit. 1SA NCAC 2T, the Division's Gravity
Sewer Design Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for the
Fast -Track Permitting of Pump Station and Force Mains adopted June 1, 2000 as applicable; and other
supporting materials may subject the Permittee to an enforcement action by the Division, in accordance with
North Carolina General Statutes §143-215.6A through §143-215.6C, construction of additional or replacement
wastewater collection facilities, and/or referral of the North Carolina -licensed Professional Engineer to the
licensing board.
10. In the event that the wastewater collection facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required
by this Division, such as the construction of additional or replacement facilities.
11. The issuance of this permit shall not exempt the Permittee from complying with any and all statutes, rules,
regulations, or ordinances that may be imposed by other government agencies (local, state and federal) which
have jurisdiction, including but not limited to applicable river buffer rules in 15A NCAC 2B .0200, erosion and
sedimentation control requirements in 15A NCAC Ch. 4 and under the Division's General Permit NCG010000,.
and any requirements pertaining to wetlands under 1.5A NCAC 2B .0200 and 15A NCAC 2H .0500,

be sent in a manner that provides proof of receipt by the Division.

Phis is an approval of flow acceptance that was recewed by the Water and Sewer Authori,
(WSACC) for the 'town of Harrisburg on December 23, 2013. WSACC has or will have capacity tt t:1tc Rocky
River Regional 'Wastewater Treatment Plant and appropriate sewer interceptors to transport and treat the
domestic wastewater from the proposed project entitled Courtyards at Harrisburg located along Robinson Church
Road just north of the intersection with Railroad Avenue. The referenced project will serve 2.3 single family
residential homes (2-bedroom) and has a projected wastewater .flow of 5,52..0 gpd.
The wastewater generated by this will be conveyed through the Town of I-larritihurg's existing.lproposed
8-inch lines, an existu1 12-inch line, and then enter WSACC's system at the existing I2-inch Back Creek
Interceptor located east rrfthe crossing; t'ith 1lickory Ridge Road. WSACC agrees to accept wastewater frog
proposed residential development baseinformation provided by EMIT 'T, Inc. (Civil Engi
project) using North Carolinas. Department of Environment and. Natural Resources (NCDENR) wastewater Clow
guidelines. The projected Clow is 5,520 gpd however \\V`SACC expects the actual wastewater flow to snore
closely approximate 4,420 gpd based upon Master Plan engines rirta data.
The WSACC Capital Inrpro'CAP) has been on pay as you go since I isc rl Year 2005 — 2006 and
has funded Capital Projects entirely ti•orn Capital Reserves, and expects to continue. this policy ins+ t1 e f�

collection system extension. Final record drawings should be clear on the plans or on digital media (CD or DVD
disk) in pdf format. Record drawings should indicate the design and the marked up changes during construction_
• Supporting design calculations (selected pumps, system curve, operating point, available storage if portable
generator(s) or storage greater than longest past three year outage reliability option selected) for any pump stations
permitted as part of this project
• Changes to the project should be clearly identified on the record drawings or in written summary form Permit
modifications are required for any changes resulting in non-compliance with this permit, regulations or
minimum design criteria. Modifications should be submitted prior to certification.
This project snail not be considered complete nor allowed to operate until the Division has received this Engineer's
Certification and all required supporting documentation. Therefore, it is highly recommended that this certification
be sent in a manner that provides proof of receipt by the DivisiorL
