In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations, permission is hereby granted to the
Cannon School Inc.
Cabarrus County
for the construction and operation of approximately 1,421 linear feet of 8 inch gravity sewer; and the
discharge of 19,375 gallons per day of collected domestic wastewater into the Rocky River Wastewater
Treatment Facility existing sewerage system, pursuant to the application received July 22, 2013 and in
conformity with 15A NCAC 2T; the Division's Gravity Sewer Minimum Design Criteria adopted February
12, 1996 as applicable; the Division's Minimum Design Criteria for the Fast -Track Permitting of Pump
Stations and Force Mains adopted June 1, 2000 as applicable; and other supporting data subsequently
filed and approved by the Department of Environment and Natural Resources and considered a part of
this permit.
This permit shall be effective from the date of issuance until rescinded and shall be subject to the
specified conditions and limitations contained therein.

The sewage and wastewater collected by this system shaH be treated in the Rocky River Wastewater
Treatment Facility (VVSA[C) prior to being discharged into the receiving stream.
Permitting of this project does not constitute on acceptance of any part of the project that does not
meet 164NCA[Zl,; the Division's Gravity Sewer Minimum Design Criteria adopted Februaryl2' 1996 as
applicable; and the Division's Minimum Design Criteria for 'the Fast -Track Permitting V[Pump Stations
and Force Mains adopted June 1' ZQ00asapplicable, unless specifically mentioned herein. Division
approval is based on acceptance of the certification provided by a North Carolina -licensed Professional
Engineer in the application. it shall be the Pemo|ttee's responsibility to ensure 'that the as -constructed
project meets the appropriate design criteria and rules,
Construction and Operation is contingent upon compliance with the Standard Conditions.
I. STANDARD CONDITIONS
l. This permit shall not betransferable. Inthe event there isadesire for the wastewater collection facilities tn
change ownership, or there is a name change of the Permiuee. a formal permit request shall be submitted to
the Division accompanied by documentation from the parties involved, and other supporting materials as may
be appropriate, The approval of this request shall be considered on its merits and may or may not be
approved,
2, This permit -;hall become, voidable unless the wastewater ooliec iom facilities are constructed in accordance
with the conditions of this uermi� ISA N[A[ ZT; the Division's Gravity Sewer Minimum Design Criteria
adopted February lZ' 1596asapp|icab|e,- the Division's Minimum Design Criteria for the Fast -Track Permitting

specifically mentioned herein.
J. This permit shoH be effective only with respect to the nature and volume of wastes described in the
application and other supporting duca.
4. The wastewater collection facilities shall be properly maintained and operated at all times. The Pe,mittee
shall maintain compNancc*/ich an individual system -wide collection system permit for the operation and
maintenance of these facilities as required by I54 NCA[ 7T.0103. If an individual permit is not required' the
following performance criteria shall bomet aaprovided inl54N[ACZT,O4O1
a. The sewer system shall be effectively maintained and operated at all times to prevent discharge to land or
surface waters, and to prevent any contravention of groundwater standards or surface water standards.
b, A map of the sewer system shafl be developed and shall be actively maintained.
c An operation and maintenance plan including pump station inspection frequency, preventative
maintenance schedule, spare parts inventory and overflow response has been developed and
implemented.

�� Pumpctatioos that are not connected totelemetryeme�system shall beinspec�d every day (ie,3G6 days

peryear). Pump stations that are connected to a telemetry system shall be inspected at least once per
week.
e� High -priority sewer lines shall he inspected at least once per every six -months and inspections are
documented.
A general observation of the entire sewer system shall be conducted at least once per year.
g. Overflows and bypasses shall be reported to the appropriate WWsbm regional office in accordance with
lSA N[A[2B .0506(a), and public notice shall be provided as required by North Carolina General Statute
§143'215.l[.
hA Grease Control Program is in 1p�ace as follows:
1. For public owned ud|ecbnn systems, the Grease Control P,oK'am shall include at least biannual
distribution of educational materials for both commercial and residential users and the legal means to
require grease interceptors at existing etabWshments. The plan shall also include legal means for
inspections of the grease interceptors, enforcement for violators and the legal means to control
grease entering the system from other public and private satellite sewersystems,
Z For privately owned collection systems, the Grease Control Program shall include at (eau biannual
distribution of grease education materials in users of the collection system by the perm|ttoe or its
representative,
I Grease education materials shall he distributed more often than required in Pans (l) and (2) of this

S. Noncompliance Notification:
The Permittee shall report by tOephone to a water resources staff member at the Mooresville Regional Office,
telephone number 704663'I699, as soon as possible, but in no case more than 24 hours or on the next
working day, following the occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility -incapable vf
adequate wastewater transport, such as mechanical o,electrical failures of pumps, line blockage or
breakage, etc.;or
b. Any 5SOand/or spill over I'0O0gallons; ur
c. Any5JO and/or spill, regardless ofvolume, that reaches surface water
Voice mail messages or faxed information is permissible, but this shall not be considered as the initial verbal
report. Overflows and spills occurring outside normal business hours may also be reported to the Division of
Emergency Management at telephone number (800) 858{)368 nr(919) 733'3300, Persons reporting any of
the above occurrences shall file a spill report by completing and submitting Part I of Form CS-550 (or the most
current Division approved form) within fivedays following first knowledge ofthe occurrence, This report must
outline the actions taken or proposed to be taken to ensure that the problem does not recur. Part || of Form
[3-SCO(or the most current D|Wdnm approved form) can also be completed to show that the SSQwas beyond

fully executed and the supporting documents are provided asapplicable. Any wastewater flow made tributary
to the wastewater collection system extension prior to completion of this Engineer's Certification shall be
considered aviolation nfthe permit and shall subject the Pe/mbteeroappropriate enforcement actions.
If the permit is issued to a private entity with an Operational Agreement, then a copy of the Articles of
Incorporation, Declarations/Covenants/Restrictions, and Bylaws that have been appropriately filed with the
applicable County's Register of Deeds office shall be submitted with the certification.
Acomplete certification is one where the form is fully executed and the supporting documents are provided as

c Changes to the project that do not result in non-compliance with this permit, regulations, or the Minimum
Design {neriu should be clearly identified on the record drawings, on the certification in the space
provided, nr\nwritten summary form.
Prior to Certification (Final or Partial): Permit modifications are required for any changes resulting in non-
compliance with this permit (including pipe length increases of 10Y. or greater, increased flow, pump station
design capacity deign increases of 5% or greater, and increases in the oumbeVtype of connections),
regulations, orthe Minimum Design Criteria. Requested modifications orvariances tothe Minimum Design
Criteria will be reviewed on a case -by -case basis and each on its own merit. Please note that variances to the
Minimum Design Criteria should be requested and approved during the permitting process prior to
construction- After -construction requests are discouraged by the Division and may not be approved, thus
requiring replacement nrrepair prior tocertification &activation.
8. 4 copy of the construction record drawings shall be maintained on file by the permitteo for the life of the
wastewater collection facilities.
9. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2T; the Division's Gravity
Sewer Design Criteria adopted February 12' 1996 as applicable; the Division' s Minimum Design Criteria for the
Fast -Track Permitting of Pump Station and Force Mains adopted June l' 7000 as applicable; and other
supporting materials may subject the Permittee to an enforcement action by the D|viuon, in accordance with
North Carolina General Statutes §143-715.6A through Y14}'ZlS.G[,construction ufadditional o,replacement
wastewater collection facilities, and/or referral of the North Carolina -licensed Professional Engineer to the

10. In the everit that the wastewater colleetim facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required
by this Division, such as the construction of additional or replacement facilities.
11, The issuance of this permit shall not exempt the Permittee from complying with any and all statutes, rules,
reguiations, or ordinances that may be imposed by other government agencies (local, state and federal) which
have jurisdiction, including but not limited to applicable river buffer rules in 15A NCAC 2B .0200 erosion and
sedimentation control requirements in 1,5A NCAC Ch. 4 and under the Division's General Permit NCG010000,
and any requirements pertaining to wetlands under 15A NCAC 2B .0200 and 15A NCAC 2H .0500,

Re: Cannon School Existing Private Sewer Narrative
This Cannon School Existing Private Sewer submittal is to request a modification to the
previously approved and certified private gravity sewers. The Cannon School wishes to
relocate the existing sewer in order to allow construction of new onsite infrastructure. There will
be no increase in sewer flow due to the relocation.
This existing sewer consists of approximately 1,421 LF- 8" private gravity sewer and related
appurtenances, and connects to an onsite existing 30" WSACC public gravity sanitary sewer
interceptor. After relocation, the total length of private sewer will be approximately 1,565 linear
feet.

Performing Arts Center 8-Inch Private Sewer engineering plans and specifications, a digital copy of
the engineering plans, and private wastewater collection system application, as prepared arid scaled
by Steven S. Wilson, NCPE, with Land.works Design Group, PA, for the private wastewater
collection system extension consisting of approximately 1,421 linear feet of 8-inch sanitary
sewer with '7 manholes and other appurtenances, to provide service to the cannon school student
population of 1000 students and 175 employees, located off Poplar Tent Rd, and Education
Way Drive, with zero increase to the wastewater discharge. The commercial wastewater
discharge of 19,375 gpd has been allocated in the downstream public sewer main extension under
the City's delegated permitting program. ATC permit 4.13-0531507, issued fitly .9, 2013,
The City of Concord's public wastewater collection system has the capacity to
accommodate the allocated commercial wastewater discharge of 1.9,375 GPD, as specified in the
City's delegated permitting program permit, ATC permit #13-0531507 and based on the
information indicated on the NCDENR Fast Track application for the project. The City of
Concord, however, will not guarantee the acceptance of any additional requests, revisions,
deficiencies, or expansions that were not included in the consulting engineer's submitted

This letter shall not be construed as an Authorization to Construct .by the City of Concord.
All applicable permits associated with this project must be obtained and copies must be
presented on site prior to commencement of the work.
IMPORTANT: Cannon School, inc., must submit a copy of the NCDENR-Division
of Water Quality, Non -Discharge approval letter and authorization to construct permit and
a copy of the engineer's final certification to the City of Concord prior to interconnection
with the City of Concord's public wastewater collection system. The City of Concord must
be present to observe the connection of this private system, in order to meet the Standard
Specifications for Wastewater Collection and Water Distribution for Cabarrus County.

prior to the initiating system connection.
Construction of -the private wastewater collection system project shall be :in accordance
with 15A NCAC 0217.0300; The Division of Water Quality's Gra-vity Sewer Minimum Design
Criteria adopted February 12, 1996, as applicable. This does not constitute an acceptance of any
part of the project that does not meet North Carolina Administrative Code Title 15A 02T.0300,
as applicable. Failure to comply may result in penalties in accordance with North Carolina
General Statute §I43-215.6A through §1.43-215.6C, construction of additional or replacement
facilities, and/or referral of the North Carolina -licensed Professional Engineer to the licensing
board.
Please be aware that the City will not assume any responsibility for the operation and
maintenance of the subject private wastewater collection system that is located on the property
beyond the public utility easementlrights-of-way.
The Cannon School, Inc„, will be -responsible for complying with the regulatory
requirements in accordance- with North Carolina Administrative Code Title 15A 02T.0300, the
maintenance and operation of the private wastewater collection system, and obtaining the
required permit approvals associated with the private wastewater collection system. Please
review this section of the Administrative Code prior to submitting your application to the State.
If the permit applicant is unable to perform all of the operation and maintenance duties required
by the State's Administrative Code, a private wastewater collection system extension should not
be requested.
The City reserves the right to disconnect the subject wastewater collection system and
potable water service from the City of Concord's public system if improper operation and

maintenance of the private system violations of any Federal, State or local regulations,
ordinances, or policies.
After the private wastewater collection system has been constructed and prior to water
service activation, Cannon School, Inc., must complete the NCDENR DWQ regulatory
requirements in accordance with North Carolina Administrative Code 15A NCAC 2T for final
approval and Cannon School, Inc.'s engineer must certify that the private wastewater collection
system has been installed in accordance with North Carolina Administrative Code 1.5A NCAC
2T. A copy of the engineer's certifications, private system final approval/acceptance letter, and
correspondences, as applicable, must be submitted to the City of Concord.

This is an approval of flow acceptance that \vas received. by the Water and Sewer Authority of
Cabarrus County (WSACC) for the City of Concord on June 26. .2013, WSA.CC has or will have
capacity at the Rocky River Regional Wastewater Treatment Plant and appropriate sewer
interceptors to transport and treat the domestic \vastewater from the proposed project entitled
Cannon School Performing Arts Center 8-Inch Public Sewer located along Poplar Tent Road just
south of the intersection with George W. L,iles Parkway. The referenced project includes an
existing school complex with 1000 student and 175 employees, and has a projected wastewater
flow of 1.9,375 gpd.
The wastewater generated by this project will be conveyed through the City of Concord's
proposed 8-inch gravity system, and then enter WS.ACC's system at the existing 30-inch Middle
Coddle Creek Interceptor line located on the east side of the existing school complex. WSACC
agrees to accept wastewater from the existing school complex based on information provided by
Landworks Design Group. P.A. (Civil Engineer for the project) using . North Carolina
Department. of Enviromnent and Natural Resources (NCDENR) wastewater flow guidelines.
The projected flow is 19,375 gpd: however WSACC expects the actual wastewater 'flow to more
closely approximate 1.5,500 gpd based upon Master Plan engineering data.
The WSACC Capital hnprovem.ent Program (CIP) has been on pay as you go since Fiscal Year
2005 — 2006 and has funded Capital Projects entirely from Capital Reserves, and expects to
continue this policy into the .future. Projects on the CIP are subject to approval by WSACC's
Board of Directors, support from local governing bodies, regulatory approvaland funding
availability. At this time there are no scheduled upgrades to WSACC facilities in the current six
year CIP that are located d.ownstreamof this proposed project. It should be noted that the list
and schedule of CIP projects are tentative and are revised periodically to reflect changing
conditions and needs.
Since this sewer connection w ill be made directly to the existing WSACC 30-inch Middle
Coddle Creek Interceptor, WSACC must be notified 48 hours prior to the connection being
made. Notification is to be made to Mark Lomax at [phone], extension 231.

If construction of this project is not started within two (2) years of the date of this approval, the
approval is rescinded and a second request and approval will be required. Please notify
WSAC(, in writing, when construction of this project is initiated.
