inaccurate information as discussed below.
Additional Information Requested:

and contacts. The package submitted was dated April 1999 and signed by Leonard Rindner
who is not involved with this current submission.
Check # 1698 for $240 is being returned with this package and resubmission will require an
application fee of $570 because cumulative temporary/permanent stream impacts are over 150
linear feet.
Please resubmit the entire application package (five complete sets) with current and updated
information and signatures.
