On December 14, 2012 the Division of Water Quality (DWQ) received information for the above
referenced project. The DWQ has determined that the application was incomplete and/or provided
inaccurate information as discussed below.
Additional Information Requested:
® The correct permitting fee to remit for the above project is $570.00 based on the cumulative
temporary/permanent stream impacts over 150 linear feet. Check # 1698 was in the amount of
$240 and was applied to the project. Please remit the balance in the amount of $330 to begin
the review and approval process.
Please submit this information within 30 calendar days of the date of this letter. This letter only addresses
the application review and does not authorize any impacts to wetlands, waters or protected buffers.
Please be aware that any impacts requested within your application are not authorized (at this time) by the
