This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency dated December 6. 1953. The conditions in this final permit include two minor changes from the draft permit the Division sent to you in December 1999. We have altered your Facility Location Map to correct the location of Outfall 001, per your correspondence to us dated December 23, 1999. In addition, we have resorted the effluent table by Sample Location to facilitate use. Please note that the monitoring requirements and parameter limits remain unchanged. If any parts, measurement frequencies, or sampling requirements contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the office of Administrative Hearings. The facility treats 100% locally derived domestic wastewater (permitted flow 0.03 MGD). The treatment system consists of a bar screen, an Imhoff tank, a sludge-holding tank, dual surface-sand filter beds, and a chlorine-tablet disinfecting process. The NPDES permit became effective on October 1, 1994 and expired on October 3t, 1999. The Division has received the appropriate documentation for permit renewal. The Mooresville Regional Office inspected the facility on March 3, and again on November 3, 1999 (Wesley Bell). They reported the facility to be well maintained and in good operating condition. System effluent was described as clear and free of foam. Anticipating permit renewal, the Regional Office recommends discontinuing influent monitoring for BODS and TSR for this limited size facility. The last regional Water Compliance Inspection Report (November 4, _1999) rated the Self- monitoring Program as "marginal" due to minor violations. These included failure to record the time for more than several sampling events, and failure to report conductivity data. Although the Region reported no permit limit violations, they did issue a Notice of Deficiency requesting the permittee to respond in writing to the minor violations. Fact sh®et NPDES Renewal Page I PERMITTING APPROACH Outfall Location Discrepancy Resolved -- Permit review disclosed a discrepancy regarding outfall location 001. Several past Waste Load Allocation (WLA) sheets indicated a discharge point on an unnamed tributary (UT) —0.4 miles (2012 feet) upstream of a small pond and dam on Duck Creek. However, Regional Office site visitors estimated the outfall point only —500 feet upstream from the pond. Confusion was exacerbated by a Nov. 1989 WLA (marked "NOT PLOTTED") proposing to discharge to another UT entering Duck Creek downstream of the pond. This "downstream" location is also marked on the USGS Quadrangle on file with the Division. To resolve this issue, the Division called the permittee and verified that waste is indeed discharged to a UT — 2000 feet upstream of the pond on Duck Creek, and that the proposed outfall downstream was never constructed (Don Kelly, Director of Business Operations, personal communication to Joe Corporon, 08Dec99). Mr. Kelly promised to fax Division a map indicating the location of outfall 001. The permit reviewer corrected the site map and the notes on the in-house USGS Quad. Verification of Existing Conditions -- The Division has verified that Duck Creek is a Class C stream; summer 7Q10= 0 cfs, winter 7Q10= 0 cfs, and 30Q2 = 0 cfs; and that Duck Creek is not listed as a 303 (d) stream (Joe Corporon, December 3, 1999). This reviewer also noted that the last two staff reports incorrectly locate this site in sub-basin 030708 (a sub-basin not in Iredell County). He has corrected these reports to read 030706. Review of DMR/Instream Data -- The Department considered the DO, temperature, and fecal coliform data from the existing stream by reviewing DMRs. Fecal coliform counts upstream are generally similar to the effluent but occasionally higher by a factor of IOX suggesting no significant impact by the facility. Dissolved oxygen levels appear believable (average -- l 1 mg/L winter; — 9 mg/L summer for `98-`99). Temperatures were also believable for the given seasons: summer, 18' to 20°C; and winter, 7" to 11 'C. PROPOSED CHANGES Permit Deletions: 1. DWQ recommends that this facility discontinue influent sampling for BODS and TSR. Considering its limited size and confined influent, this facility appears to have been assigned influent sampling in error. 2. In keeping with the Department's policy (12May99) toward dischargers with 100%-domestic effluents, this reviewer has deleted fecal coliform and conductivity testing from this facility's instrearn monitoring requirements. Permit Additions: 1. Monitoring -- In keeping with proposed nutrient monitoring for the Yadkin Basin (memorandum from Steven Pellei, December 7, 1998), the Department recommends quarterly monitoring (4/year) of treatment system effluent for total nitrogen (N), for total phosphorous (P). Similarly, pH sampling is also required 2/month. Therefore, the reviewer has added these parameters to this permit. NPDES Renewal Past., 2. Continuous Flow Monitoring Considered —This facility currently estimates flow with a dosing counter. The Division reviewed actual discharge flow volumes in gallons per day (gpd) for the last two years. The average daily discharges for a 20-month period, based on DMRs for 1998 and 1999, indicate 10,600 gpd (Nov. thru Mar.) and 7,100 gpd (Apr. thru Oct.). Of these monthly averages, 13 of 20 equaled or exceeded the 10,000-gpd limit recommended to trigger continuous monitoring. Furthermore, the facility has made initial inquiries proposing to construct an onsite recreation facility associated with the YMCA. The estimated future impact to the treatment system is +6,000 gpd (Don Kelly, Director of Business Operations, personal communication to Joe Corporon, IODec99). Based on current and projected flows, it may be appropriate to recommend continuous flow monitoring. PART IV - EVALUATION AND RECOMMENDATIONS Barium Springs Home for Children has requested renewal of the subject Permit. The number of users (students/staff) has risen from 100 in 1994 to 160 at the time of permit renewal. With the exception of the dosing counter, there have been no changes to the W WTP and/or permit since the last permit renewal.
