DIVISION OF ENVIRONMENTAL MANAGEMENT July 13, 1994 MEMORANDUM o moll TO: Dave Goodrich ti q'u//j ( i THROUGH: Matt Matthews Y' /30 f y @X 6I FROM: Kristie Robeson 4 / SUBJECT: Permit Modification NCDOT Surry Co. I-77 Rest Area NPDES Permit No. NCO029190 Surry County By cover letter dated June 30, 1994, the subject facility was issued NPDES Permit No. NCO029190 with an effective date of August 1, 1994, and expiration date of June 30, 1999. Upon review of the permit a discrepancy was noted in the M3 page and the conditions page of the permit. The M3 page of the permit indicates that toxicity testing information is found in Part III, Condition D. However, the conditions page indicates toxicity testing information is found in Part III, Condition E. It is the recommendation of this office that the M3 page be modified to read "Chronic Toxicity (Ceriodaphnia) P/F at 13%: February, May, August, and November; See Part III, Condition E. I appreciate your assistance to undertake the necessary steps to modify the existing permit and incorporate the recommendation cited above.
