A. (2) ACUTE TOXICITY PASSNAIL PERMIT LIMIT (QRTRLY)
The permittee shall conduct acute toxicity tests on a quarterly basis using protocols defined in the North
--Carolina-Procedure--Document entitled.-`-`Pass/Fail-Methodology -For.-Determining Acute--Toxicity.-ln.-A- .....
Single Effluent Concentration" (Revised-July, 1992 or subsequent versions). The monitoring shall be
performed as a Fathead Minnow (Pimephales promelas) 24 hour static test. The effluent concentration at
which there may be at no time significant acute mortality is 90% (defined as treatment two in the
procedure document). Effluent samples for self-monitoring purposes must be obtained during
representative effluent discharge below all waste treatment. The tests will be performed during the months
of February, May, August and November.
All toxicity testing results required as part of this permit condition will be entered on the Effluent
Discharge Monitoring Form (MR-1) for the month in which it was performed, using the parameter code
TGE6C.Additionally, DWQ Form AT-2 (original) is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Branch no later
than 30 days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements
performed in association with the toxicity tests, as well as all dose/response data. Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for disinfection of
the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is
required, the permittee will complete the information located at the top of the aquatic toxicity (AT) test
form indicating the facility.name, permit number, pipe number, county, and the month/year of the report
with the notation of "No Flow" in the comment area of the form. The report shall be submitted to the
Environmental Sciences Branch at the address cited above.
Should any single quarterly monitoring indicate. a failure to meet specified limits, then monthly
monitoring will begin immediately until such time that a single test is passed. Upon passing, this monthly
test requirement will revert to quarterly in the months specified above.
Should the-permittee-fail to monitor ddi�ing a month in 'which toxicity monitoring is required, then-`'
monthly monitoring will begin immediately until such time that a single test is passed. Upon passing, this
monthly test requirement will revert to quarterly in the months specified above.
Should any test data from either these monitoring requirements or tests performed by the North Carolina
Division.of Water Quality indicate potential impacts to the receiving stream, this permit may be re-opened
and modified to include alternate monitoring requirements or limits.

If the Permittee monitors any pollutant more frequently then required by this permit, the results of such
-monitoring shall--be included-in the--calculation-&-iepoitiiig af,the dafa submitted on the DMR & AT

NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control
organism survival and appropriate environmental controls, shall constitute an invalid test and will
require immediate follow-up testing to be completed no later than the last day of the month following the
month of the initial monitoring.
A. (3) REPORTING OF OPERATION
The Permittee and/or Permittee representative must contact the Mooresville Regional Office/Surface
Water Protection Section, phone [phone], during normal business hours within 48 hours of
resuming operation and discharge. Contact information should include (1)contact person name and phone
number, (2) NPDES permit number, (3) date and time of resumption of operation, and (4) expected
operation for next 90 days. Additional information may be requested by Regional staff. Upon start up, any
previous monitoring/reporting waivers are no longer applicable.

BACKGROUND
The permit was originally issued to CR Industries for the treatment of domestic and industrial '
waste. The permit has been modified extensively since it was issued in 1996. In 1998 they added
a groundwater treatment system and began discharging the treated groundwater through the same '
outfall as the industrial/domestic treatment plant. In 1999 they closed the facility and ceased the
domestic/industrial discharge. They eventually sold the facility and the new owner connected
the domestictindustrial portion of the effluent to the City of Gastonia WWTP. SKF USA ,
retained the groundwater treatment system and the responsibility for its operation and
maintenance. The groundwater remediation system was installed to remediate/extract
trichloroethene,tetrachloroethene, and cis-1,2dichloroethene from the groundwater. '
Initially, a Chronic toxicity test at 0.33% concentration was required for the GWR discharge. But
in the 2002 permit renewal the permitted flow was decreased resulting in the toxicity test
requirement being changed to Acute to comply with Division policy. The policy stated that when '
the toxicity test concentration (defined by the IWC) is less than 0.25% then a Chronic test is not
applicable and an Acute toxicity test at 901/6 must be applied.
The last reported discharge from this GWR system was in March 2008. In February 2010, the '
permittee requested and was granted a wavier to discontinue monitoring and submittal of DMR
while no discharge occurred. The treatment system is currently deemed inactive, and the region '
has confirmed that subsequent piping had been removed to disengage the treatment system from
the outfall. All fees and other permit conditions still apply.

The groundwater treatment system consists of 2 collection wells which discharge to a 500 gallon
surge tank. A transfer pump conveys the liquid from the surge tank through a bag filter and then
to a 5 tray air stripper. As the flow leaves the air stripper collection sump, a flow meter/recorder
measures the flow. The effluent flow continues through a buried gravity line and discharges
through the permitted outfall into Crowders Creek. Crowders Creek flows into South Carolina
and is a tributary to Lake Wylie.
Hart & Hickman, PC, representativeloperator for this GWR system has submitted a permit
renewal as they believe they will be required to restart this system.
TOXICITY ISSUE, 303(d)LIST, AND COMPLIANCE
There was a historical problem with the toxicity testing, both Chronic and Acute test have been
performed. The facility was consistently .passing.the-Chronic test at 0.33% concentration but
when they switch to the Acute test at 90% concentration they failed 2 of the 4 Acute test,
including the last test performed before the discharge was ceased in 2008. There were 5
additional Chronic toxicity test performed in 2006 and 2007 that the facility passed.
This segment of Crowders Creek continues to be listed on the 303(d) list as impaired for
biological integrity to the fish community.
This permit will be submitted to the South Carolina Department of Environmental Health for
comment.
There were no compliance actions or NOV issued during this permit cycle. The Region staff
report recommended renewal pending approval of the final permit..
PERMITTING STRATEGY
DMRs were reviewed for the period''of February 2006 to April 2008 (last discharge event). The
average MGD was 0.0009. The five organic chlorinated solvents monitored during this permit
cycle were reported as less than values:•A.RPA report was•prepared for the five solvents that had
been monitored.
To assist with the renewal evaluation the permittee, upon request, provided the last wells
analyses. The well data did not indicate any additional pollutants of concern that have not been
previously addressed or are not currently addressed.
To validate no pH issues, pH monitoring on a quarterly bases will be added. The Permittee may
petition to have pH monitoring removed as part of the next renewal, and a note will added to the
file. The Acute toxicity narrative will be amended to match the current version.
Since the existing system has been inactive for 2 years but is subject to start up in the future, a
special condition will be.added that will require the permittee to alert the Mooresville DWQ
regional office once the treatment system and discharge resumes.

4.0 Ground Water Recovery and Treatment System •
The ground water remediation system at the site began full-time operation in August 1999. This
pump-and-treat system consists of ground water recovery wells RWA (formerly MW-15) and
RW-2 (near MW-I), treatment via a stacked-tray air stripper, then discharge of the treated
effluent to a permitted surface water outfall under NPDES permit number NC0004260. On
March 18, 2008, the remediation system was shutdown due to effluent toxicity testing failures.
The remediation system has remained inactive in order to evaluate site ground water conditions
for passive remediation by natural attenuation processes. Figure 2 depicts the approximate
locations of the recovery wells, the remediation system building that houses the air stripper, and
the underground piping connecting the recovery wells with the treatment system.

PART IV - EVALUATION AND RECOMMENDATIONS
SKF USA Inc. is applying for renewal of its NPDES permit. The facility is in inactive
status and ceased discharge since March 2008. According to Mr. Chad Grubbs, with Hart &
Hickman, PC, the Inactive Hazardous Site Branch (IHSB) required them to do more assessments
in order to provide appropriate remedial action. According to Mr. Grubbs, Hart& Hickman is in
the process of submitting the findings to the IHSB in the very near future and they are planning to
discharge again from the facility. Also, Mr. Grubbs indicates that the only challenge with the
permit requirements was the change from the Chronic to Acute toxicity. He indicated that they
are having problem with passing the Acute Toxicity test (Fathead minnows) while they did not
have any problem passing the Chronic Toxicity (Ceriodaphnia) test.
Pending review and approval by the Western NPDES Program Unit, it is recommended
that the permit be renewed as requested.
