In accordance with the application for discharge permit received on February 5, 1998. the
Division is forwarding herewith the subject state - NPDES permit. This permit is issued pursuant,to
the requirements of North Carolina General Statute 145.215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated December 6, 1985.
In accordance with your comments on the draft NPDES permit received on May 6, 1999, the
following modifications have been made:
• The monitoring frequency for total suspended residue and fecal coliform has been
decreased from daily to weekly;
• The monitoring frequency for oil 11-Q grease has been changed to monthly: and
• In light of the change in treatment plant classification from IV to III, the monitoring
frequency for BOD, temperature and pl-1 has been changed from daily to 5/week.
You also requested that total residual chlorine monitoring be eliminated. Due to the toxic
nature of residual chlorine, monitoring will be maintained weekly when chlorination is employed (see
footnote 2 on the effluent limitations and monitoring page). Additionally, the weekly monitoring
requirement for nutrients is the result of the Division's current permitting strategy for facilities
discharging into the High Rock Lake watershed. This watershed shows signs of nutrient enrichment
and will therefore be subject to a nutrient management strategy at some point in the future. Until
such a strategy is in place, more stringent monitoring is being required of all discharges into the
watershed.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must he in the form of a written petition,
conforming to Chapter 150E of the North Carolina General Statutes, and filed with the office of
Administrative Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-7447, Unless
such a demand is made, this permit shall be final and binding.

mechanical bar screen, comminutor, equalization basin, chemical mixing tanks, primary
clarifier, nutrient addition, dual aeration basins, final clarifiers, gas chlorination with chlorine
contact chamber, dual activated carbon adsorption units with phi adjustment, a backwash
equalization basin and sludge holding located at PPG's Lexington manufacturing facility on
new Jersey Church Road in Lexington, Davidson County; and
2. Discharge from said treatment works at the location specified on the attached map into North
Potts Creek which is classified C waters in the Yadkin-Pee Dee River Basin.

A (2). CHRONIC TOXICITY PASS/FAIL PERMIT LIMIT (QUARTERLY)
The effluent discharge shall at no time exhibit observable inhibition of reproduction or significant mortality to
Ceriodaphnia duhia at an effluent concentration of 48%.
The permit holder shall perform at a minimum, quarterly monitoring using test procedures outlined in the
"North Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent
versions or "North Carolina Phase I1 Chronic Whole Effluent Toxicity Test Procedure" (Revised-February
1998) or subsequent versions. The tests will be performed during the months of February, May, August, and
November. Effluent sampling for this testing shall be performed at the NPDES permitted final effluent
discharge below all treatment processes.
If the test procedure performed as the first test of any single quarter results in a failure or ChV below the
permit limit, then multiple-concentration testing shall be performed at a minimum, in each of the two following
months as described in "North Carolina Phase Il Chronic Whole Effluent Toxicity Test Procedure" (Revised-
February 1998) or subsequent versions.
The chronic value for multiple concentration tests will be determined using the geometric mean of the highest
concentration having no detectable impairment of reproduction or survival and the lowest concentration that
does have a detectable impairment of reproduction or survival. The definition of "detectable impairment,"
collection methods, exposure regimes, and further statistical methods are specified in the "North Carolina
Phase It Chronic Whole Effluent Toxicity"hest Procedure" (Revised-February 1998) or subsequent versions.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1) for the months in which tests were performed, using the parameter code TGP311 for
the pass/fail results and THP3B for the Chronic Value. Additionally, DWQ Form AT-3 (original) is to be sent
to the following address:

Completed Aquatic Toxicity Test Forms shall he filed with the Environmental Sciences Branch no later than 30
days after the end of the reporting period for which the report is made.
Test data shall he complete, accurate, include all supporting chemical/physical measurements and all
concentration/response data, and be certified by laboratory supervisor and ORC or approved designate
signature. Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine is
employed for disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required,
the permittee will complete the information located at the top of the aquatic toxicity (XI) test form indicating
the facility name, permit number, pipe number, county, and the month/year of the report with the notation of
"No Flow" in the comment area of the form. The report shall be submitted to the Environmental Sciences
Branch at the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, monitoring will
be required during the following month.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of
Water Quality indicate potential impacts to the receiving stream, this permit may be re-opened and modified to
include alternate monitoring requirements or limits.

PPG Industries Fiber Glass Products, Inc. operates a fiberglass fiber manufacturing facility in
Lexington, NC. The Lexington plant currently concentrates the bulk of its fiberglass yarn
manufacturing on the textile industry as well as the printed circuit board industry.
Manufacturing begins with glass raw materials including limestone, silica, boron, clay and fluorspar.
These raw materials are blended in powder form and added to melt furnaces. The batch melt
furnaces, operating at 2800°F, transform the batch into molten glass. The molten glass is then
drawn from the furnaces through electrically heated platinum bushings into continuous fibers. These
fibers are gathered into strands and either spooled or chopped into specified lengths. XVater sprays
cool the glass and keep strand contact points clean during the winding process.
This permit was first issued in 1977 and most recently renewed on August 1, 1995 upon resolution
of issues related to a petition for- contested case filed by the per•mittee. Documentation associated
with the 1995 renewal is quite sparse. The reason for the adjudication is not evident, but it appears
to have been filed regarding extensive instream monitoring installed in association with High Rock
I-ake. The facility had conducted an extensive instream study in the late 70s and felt that additional
stream monitoring was neither justified no necessary. As part of the settlement agreement reached
between the facility and the Division, instream monitoring was maintained but at a greatly reduced
frequency. The facility was required to monitor at 10 instrearn stations once during the summer of
1995 and once in 1996, Monitored parameters were DO, temperature, conductivity, pl-1, total
phosphorus, nitrate, nitrite, ammonia and `KN. The monitoring was completed as required and is
not proposed with this renewal.

The current effluent limitations For BOD, and TSS appear to be based on water quality protection
and not on federal guidelines. Extensive research was conducted to determine if any federal
guidelines exist for such a manufacturing operation, lion Anderson and Hugh Wise were contacted
at EPA in D.C. regarding the applicability of textile and glass guidelines. After my conversations with
both, I determined that neither set of guidelines applies to PPG's process. A review of the wasteload
allocation file indicated that the limits installed in this permit in 1985 were based on BPJ.
As indicated previously, this Facility discharges into North Potts Creek. North Potts is tributary to
High Rock Lake and is not listed on the State's 505(d) list. High Rock Lake shows signs of heavy
eutrophication. A management plan will be developed in the near future to address nutrient loads to
the High Rock system. Currently, however, there is no strategy for controlling nutrients from this
discharge with the exception of nutrient monitoring.
PPG's Lexington facility discharges through a single permitted outfall, 001. This outfall is from the
wastewater treatment plant and receives the following kinds of wastewater-:
• Sanitary (American Children's Home)
• In-plant sanitary
• Boiler Feed
• Process WW
• Von-contact cooling water
• Sludge dewatering
The vast majority of the wastewater sent to the treatment plant is process wastewater with only a
small portion coming from sanitary sources.
COMPLIANCE SUMMARY:
A review of DNIR data has indicated that this facility is well operated and, with the exception of
toxicity, has historically performed well within permit limits. Effluent BOD, averages about 5 mg/L
while effluent TSS averages about 2 mg/L. Because of the low domestic loading, influent and
effluent ammonia as well as effluent NO, is consistently about 1 mg/L and 0.1 mg/L respectively.
Effluent total nitrogen (most of which is organic) averages about 8 mg/1- while effluent total
phosphorus averages about 1.2 mg/L. This facility appears to be achieving effluent quality as good as
could be expected for a manufacturing process of this kind.

This facility has had a history of toxicity failures, In December of 1996. PPG applied for and
received a Special Order by Consent (SOC) for toxicity failures. The facility failed monthly chronic
tests from at least January 1995 through November 1995, with sixteen failures from January 1995
until receipt of the SOC in December of 1996. As part of that SOC, the facility was required to
continue its Toxicity Reduction Evaluation (TRE) efforts so as to potentially identify the source of
toxicity. Early on in the TRE process, ferric chloride was replaced with ferric sulfate as a coagulant
aid. Toxicity persisted during the proceeding months. In accordance with their SOC, PPG received
an Authorization to Construct (ATC) for activated carbon adsorption facilities on April 4, 1997. 1t is
not clear as to whether or not bentonite clay is added. An Engineer's Certification was subsequently
received for this ATC on October 10, 1997. Although the documentation does not identify the
toxicity causing agent, the installation of carbon adsorption facilities seems to have solved the toxicity

failure problem. Should toxicity problems return, it should be made clear that PPG's first priority is
the determination of the toxicity causing agent(s).
INSTREAM MONITORING:
As discussed in the Summary, an extensive instream monitoring program was established with the
previous permit. The intent of that program was to determine PPG's impact on instream dissolved
oxygen and nutrient levels. The program required sampling once during the summers of 1995 and
1996, A review of the data did not provide conclusive evidence that PPG's discharge is impacting
instream water quality. As such. with the data collected, the instream monitoring program has been
removed from this permit.
PROPOSED CHANGES:
• Nlonitoringf Frequencies: The monitoring frequency for total phosphorus, NO., and TKNr will
be increased from monthly to weekly in accordance with the Division's strategy for nutrient
monitoring in permits discharging within the High Rock make watershed. The monitoring
frequency For all other parameters will be modified such that they are consistent with the 2B
.0500 rules for facilities with SIC 3100.3199. Monitoring for COD has been eliminated from
the permit as it is not required in the PPG Shelby plant. Additionally, a review of wasteload files
did not provide any justification for continued monitoring.
• instream YYlonitoring. The instream monitoring requirements will be deleted as the
sample period stipulated in the existing permit has passed.

THE work to me for comment.
I will try to make this concise.
PPG hired AEI in June 1993 as their toxicity consultant. AEI identified
ferric chloride as a potential toxicant and suggested the facility
switch use to ferric sulfate. This change was implemented in 9/9, the
facility had several "passes"; however, effluent toxicity returned. The
facility requested an SOC from DWQ in 8/95 which was signed by Preston
on 12/18/96. The SOC (EMC WQ No. 95-18) provided conditional compliance
for toxicity until 711/97. It also contained an upfront $3000.00
penalty for settlement of all chronic toxicity effluent violations from
3/95-7/95. File notes suggest that activated carbon was selected as the
treatment technology based on pilot testing results and information
obtained from limited characterization testing. This office expressed
concern that effluent toxicants may not have been sufficiently
characterized and that we had not reviewed data which would suggest that
activated carbon would reduce toxicity to acceptable levels. Since
7/1/97, the facility has met or achieved compliance with its 49% chronic
permit limitation. (NOTE: PPG operates another facility in Shelby
which was also having problems with attaining compliance with final
permit limits for toxicity and implemented a bentonite clay addition
system.)
