Dear Mr. Harrell:
On March 1, 1999, the Division issued the subject permit. You have since requested (via the Washington
Regional Office) modification of several sampling parameters due to the reclassification of the treatment system
covered under the subject permit. This permit modification changes the following monitoring frequencies:
> Influent& effluent SOD5 monitoring is changed to Weekly
> Influent & effluent Total Suspended Solids monitoring is changed to Weekly
> Effluent NH3 (as N) monitoring is changed to Weekly
> Effluent Dissolved Oxygen monitoring is changed to Weekly
> Effluent Fecal Coliform monitoring is changed to Weekly
> Effluent Total Residual Chlorine monitoring is changed to 2/Week
> Effluent pH monitoring is changed to Weekly
Please find enclosed the revised permit page. This page should be inserted into your permit. The old page
may then be discarded. All other terms and conditions contained in the original permit remain unchanged and in
full effect. This permit modification is issued under the requirements of North Carolina General Statutes 143-
215.1 and the Memorandum of Agreement between North Carolina and the U. S. Environmental Protection
Agency.
If any parts, measurement frequencies or sampling requirements contained in this permit modification are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days
following receipt of this letter. This request must be a written petition conforming to Chapter 150E of the North
Carolina General Statutes, filed with the Office of Administrative Hearings, Post Office Drawer 27447. Raleigh,
North Carolina 276 1 1-7447. Unless such demand is made, this decision shall be final and binding.

Notes:
1. The monthly average effluent BOD5 and Total. Suspended solids concentrations shall not exceed 15
percent of the respective influent value (85 percent removal).
2. The daily average dissolved oxygen effluent concentration shall not be less than 6.0 mg/I.
3. The pH shall not be less than 6.8 standard units nor greater than 8.5 standard units and shall be
monitored as per the requirements outlined in Special Condition A. (3) on the Supplement to
Effluent Limitations and Monitoring Requirements Page.
4. Special monitoring requirements for Dissolved Oxygen, Temperature, Conductivity, Salinity, pH.
TKN, NH3,-NO2-'+ NO3, and Chlorophyll-a are included in Special Condition A. (3) on the
Supplement to Effluent Limitations and Monitoring Requirements Page.
There shall be no discharge of floating solids or visible foam in other than trace amounts.
Special permit conditions applicable to the subject facility are listed on the Supplement to
Effluent Limitations and Monitoring Requirements Page.

Town of Columbia
Tyrrell County
Mr. Carlisle Harrell, Manager for the Town of Columbia has
submitted an NPDES permit modification request in behalf of the
Town. The Town has undergone a reclassification of its wastewater
treatment plant from a Class III to a Grade II Biological Water
Control Treatment System. Mr. Harrell asks that the monitoring
requirements be modified to reflect their new classification. This
is the only requested change.
Please find attached a cover letter, a new effluent
limitations and monitoring page, and the letter from Mr. Harrell
requesting the change. The new effluent limitations and monitoring-
page was prepared in accordance with 15A NCAC 2B . 0508 (2) (d)
Domestic Wastewater and Other Facilities Discharging Primarily
Domestic-Water Quality Limited. The cover letter was prepare with a
format supplied by Mr. Tom Belnick of the NPDES Unit .

Dear Mr. Hodge:
This letter is to follow-up our telephone conversation today on monitoring requirements
for our wastewater treatment plant which has been reclassified from Grade III to Grade 11.
Enclosed is a letter dated June 27, 1999, from DEHNR reclassifying our wastewater
treatment plant from Grade III to Grade 11. The Town is requesting a change in monitoring
requirements to reflect those of a Grade II plant.

• � 4 In accordance with North Carolina General Statute § 90A-37, the Water Pollution -
Control System Operators Certification Commission is required to classify all water
- ' pollution control systems. The rating procedure, found at 15A NCAC 8G .0300, was
adopted by the Commission to classify water pollution control systems.
The Water Pollution Control System Operators Certification Commission hereby
y reclassifies the subject facility as a Grade 2 Biological Water Control Treatment
' System.

As required by 15A NCAC 8G .0202 and your permit, you must designate an
' Operator in Responsible Charge (ORC) and Back-Up Operator of the appropriate

= type and grade for the subject facility. Your facility requires a Grade 2 or higher

Wastewater Treatment Plant Operator in Responsible Charge and Grade 1 or higher
""'�--------=� Back-Up Operator. Failure to designate a properly certified operator and back-up
operator constitutes a violation of the permit issued for this facility. This
reclassification does not affect monitoring requirements. Changes in monitoring
• requirements would require a permit modification.
