On March 25, 2002, I spoke with Keith Haynes of the Asheville Regional Office and he informed me that instream monitoring locations listed in the permit were incorrect. These have been changed to reflect the instream monitoring points listed in the previous permit. Please find enclosed the revised permit pages. The revised pages should be inserted into your permit. The old pages,may then be discarded. All other terms and conditions contained in the original permit remain unchanged and in full effect. This permit modification is issued under the requirements of North Carolina General Statutes 143- 215.1 and the Memorandum of Agreement between North Carolina and the U. S. Environmental Protection Agency. If any parts,measurement frequencies or sampling requirements contained in this permit modification are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must be a written petition conforming to Chapter 150B of the North Carolina General Statutes, filed with the Office of Administrative Hearings (6714 Mail Service Center,Raleigh,North Carolina 27699-6714). EFFLUENT LIMITATIONS AND MONITORING REQUIREMENTS - FINAL During the period beginning on the effective date of the permit and lasting until expiration, the permittee is authorized to discharge from outfall(s) serial number 001. Instream monitoring shall be grab samples taken 3/wk (Jun-Sep) and 1/wk (Oct-May). 2 The monthly average effluent BOD5 and Total Suspended Solids concentrations shall not exceed 20%of the respective influent value (80%removal). 3 The daily average dissolved oxygen effluent concentration shall not be less than 6.0 mg/l. a Chronic Toxicity (Ceriodanhnia). P/F, 9%; February, May,August, and November; See Part A. (2.). s The quantitation level for cyanide is 10.0 ug/l. If the measured levels of cyanide are below the detection limit, then the measurement is considered to be zero for purposes of compliance evaluation only. A North-Carolina-certifled laboratory should continue to report all cyanide values detected, even if these values are below the Division's specified quantitation level of 10 ug/L. 6 The detection limit for mercury is 0.2 ug/l. If the measured levels of mercury are below the detection limit, then the measurement is considered to be zero for purposes of compliance evaluation only. A North Carolina certified laboratory should continue to report all levels detected, even if these levels are below 0.2 ug/L. 7 The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units. There shall be no discharge of floating solids or visible foam in other than trace amounts. i
