Dear Mr. Caulder:
On May 23, 2001, the Division of Water Quality (Division) received your request
to modify permit NC0020338. After reviewing your letter and the letter received from
Tony Arnold of the Technical Assistance_ and Certification Unit, it has been
determined that your permit should be adjusted to reflect your facility's
reclassification as a Grade 3 Biological Water Pollution Control Treatment System.
Accordingly, the monitoring frequency for BODs, Total Suspended Residue, NH3-N,
dissolved oxygen, pH, total residual chlorine, fecal coliform, oil and grease, and
conductivity has been reduced from daily to three times a week.
Please find enclosed the revised permit pages. The revised pages should be
inserted into your permit. The old pages may then be discarded. All other terms and
conditions contained in the original permit remain unchanged and in full effect. This
permit modification is issued under the requirements of North Carolina General
Statutes 143-215.1 and the Memorandum of Agreement between North Carolina and
the U. S. Environmental Protection Agency.
If any parts, measurement frequencies or sampling requirements contained in
this permit modification are unacceptable to you, you have the right to an
adjudicatory hearing upon written request within thirty (30) days following receipt of
this letter. This request must be a written petition conforming to Chapter 15013 of the
North Carolina General Statutes, filed with the Office of Administrative Hearings
(6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such
demand is made, this decision shall be final and binding.

Dear Mr. Williams:
In accordance with North Carolina General Statute § 90A-37, the Water Pollution Control
System Operators Certification Commission is required to classify all water pollution control
systems. The rating scale for classification of Biological Water Pollution Control Treatment
Systems, found at 15A NCAC 8G .0302, was adopted by the Commission to classify water
pollution control systems. The classification of a facility is'determined by the permitted flow
and the requirement to achieve biological nutrient reduction.
The Water Pollution Control System Operators Certification Commission hereby reclassifies the
subject facility as a Grade 3 Biological Water Pollution Control Treatment System.
As required by 15A NCAC 8G .0202 and your permit, you must designate an Operator in
Responsible Charge (ORC) and Back-up Operator of the appropriate type and grade for the
subject facility. Your facility requires a Grade 3 or higher Wastewater Treatment Plant
Operator in Responsible Charge and Grade 2 or higher Back-up Operator. Please complete and
return the enclosed ORC designation form by Au ust 10 2001. Failure to designate a properly
certified ORC and Back-up Operator by the above date constitutes a violation of the permit
issued for this facility. This reclassification does not affect current monitoring requirements for
this facility. A chance in monitoring re uirements would require a permit modification.

Dear Mr. Goodrich,
The Town of Yadkinville requests a change of classification of its Wastewater Treatment
Plant to a Grade 3. The WWTP's currently holds a Grade 4, with a permitted flow at 2.5
MGD.
The request of reclassification is flow reduction, due to industrial flow decrease.
The Town's largest industry, with an average flow of 0.250 MGD,will permanently shutdown
sometime in December 2001.
The following shows the WWTP's flows for the previous year:

The Town also requests a reduction in sampling frequency to 3 times a week. The WWTP
currently samples 5 times a week for BOD, TSS, NH3-N, O/G, Fecal Coliform and
Conductivity parameters.
Effective June 01, 2001, the WWTP will be without a Grade 4 ORC. The job has been in
newspaper circulation for 3 weeks. The Town has had no Grade 4 applicants to date.
