Dear Mr. Roberts:
In response to your request to modify the subject NPDES permit, and following your review and
comments, the Division of Water Quality (the Division or DWQ)hereby transmits this permit
modification. We issue this modification pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S.
Environmental Protection Agency dated October 15, 2007, or as subsequently amended.
DWQ appreciates your efforts to replace existing Ion-Exchange (IE) and green-sand filters with more
environmentally friendly waste-treatment technologies. We judge the proposed scope of work a minor
permit modification to wasteflow treatment that therefore does not warrant Public Notice under the
NPDES Program.
Changes to Your Previous Permit. Based on your modification request(acknowledged November 29,
2011), we have revised your permit, after considering the Division's current permitting procedures for
potable-water treatment plants (WTPs),according to technology. This is a"phased"permit with two
effluent pages providing permit limits and monitoring condition for two different effluents, one before
your proposed system renovations [see §A. (1.)] and one after [see §A. (2)]. We have also clarified
your facility description, updated your facility map, and added parameter codes to the effluent
monitoring pages for your convenience [see Supplement to Cover Sheet and permit § A. (l.) and A.
(2)]•
Changes to the Permit Modification Draft. During draft review, we inadvertently left out Total
Dissolved Solids (TDS) as a monitoring requirement for ion-exchange technologies under §A. (1.). We
have therefore added this parameter.

Please note that this permit is not transferable except after notifying the Division of Water Quality. The
Division may require permit modification, or revocation and re-issuance. Please note that this permit
does not affect your legal obligation to obtain other permits required by the Division of Water Quality,
the Division of Land Resources, the Coastal Area Management Act, or other federal or local
governments.

In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful standards
and regulations promulgated and adopted by the North Carolina Environmental Management
Commission, and the Federal Water Pollution Control Act, as amended,
Aqua North Carolina, Inc.
is hereby authorized to discharge wastewater from a facility located at the
Snow Hill Subdivision - Well #1 Water Treatment Plant (WTP)
Snow Hill Drive, Dobson
Surry County
to receiving waters designated an unnamed tributary of Cody Creek within the Yadkin—Pee Dee River
Basin in accordance with effluent limitations, monitoring requirements, and other conditions set forth in
Parts I, II, III, and IN hereof.
The permit modification shall become effective on March 14, 2012.
This permit and the authorization to discharge shall expire at midnight on February 28, 2014.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this tacility, whether for operation or discharge are hereby
revoked, and as of this issuance, any previously issued permit bearing this number is no longer effective.
Therefore, the exclusive authority to operate and discharge from this facility arises under the permit
conditions, requirements, terms, and provisions described herein.
Aqua North Carolina, Inc.
is hereby authorized to:
1. continue to treat filter-backwash wastewater generated during the production of potable water
from a well, utilizing a combination of green-sand filter and Ion-Exchange (IE) water-
treatment technologies, from facilities including
• two (2) each ion-exchange (IE)water softeners (with added caustic)
• four(4) each potassium permanganate feed tanks
• four(4) each green-sand filters [to be replaced]
• drain line to outfall, and
after converting from green-sand to chemical-free sand filtration technology and removing ion-
exchange facilities, utilize
• three (3)each iron-removing, chemical-free oxidizing sand filters
• appurtenant valves and piping
• drain line to outfall,
all facilities located at the Snow Hill Subdivision - Well #1 WTP, Snow Hill Drive, Dobson,
Surry County, and
2. discharge from said treatment works, at the location specified on the attached map,to an
unnamed tributary of Cody Creek [stream segment 12-63-14],a waterbody currently classified
C within Subbasin 03-07-02 of the Yadkin Pee Dee River Basin.

A. (3.) CHRONIC TOXICITY PERNUT - MONITORING ONLY (QUARTERLY)
The Permittee shall conduct quarterly chronic toxicity tests using test procedures outlined in the"North Carolina
Ceriodaphnia Chronic Effluent Bioassay Procedure,"Revised February 1998, or subsequent versions.
The effluent concentration defined as treatment two in the procedure document is 90%. The permit holder shall
perform quarterly monitoring using this procedure to establish compliance with the permit condition. The tests
will be performed during the months of March,June,September, and December. Effluent sampling for this
testing shall be performed at the NPDES permitted final effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1) for the month in which it was performed, using the parameter code TGP3B.

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no later than 30
days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements performed
in association with the toxicity tests, as well as all dose/response data.Total residual chlorine of the effluent
toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required,
the Permittee will complete the information located at the top of the aquatic toxicity(AT)test form indicating the
facility name, permit number, pipe number,county, and the month/year of the report with the notation of"No
Flow" in the comment area of the form. The report shall be submitted to the Environmental Sciences Section at
the address cited above.
Should the Permittee fail to monitor during a month in which toxicity monitoring is required, then monthly
monitoring will begin immediately. Upon submission of a valid test,this monthly test requirement will revert to
quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of
Water Quality indicate potential impacts to the receiving stream,this permit may be re-opened and modified to
include alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control organism
survival, minimum control organism reproduction, and appropriate environmental controls, shall constitute an
invalid test and will require immediate follow-up testing to be completed no later than the last day of the month
following the month of the initial monitoring.

These filters are only for removing iron and manganese. Generally speaking,this technology will not replace ion
exchange filters for uses such asa softening the water, removal of arsenic or radionuclides etc.

Scott,
Joe and I have discussed this and the proposal looks interesting.We are pleased that the ion exchange units will be
eliminated. Joe has a full workload and planned to address the modification for Snow Hill in early Feb. Please contact
Joe at 919-807-6394 if you have any additional questions.
When this sytem is up and running and after several sampling analyses have been performed I would be very interested
in visiting the facility. Ion Exchange units,while very popular, discharge chlorides and some metals that consistently fail
aquatic toxicity tests. If this alternative is successful it could prove to be satisfactory for other water treatment plants in
the state as well.

Aqua North Carolina, Inc. (Aqua)wishes to do a minor modification to their NPDES permit
(NC0088617).which is located at the Snow Hill water system (PWSID#NC 02-86-116). Currently,tite
system is permitted for the discharge of four greensand filters, but has a discharge limit page more similar
to those issued for lon Exchange WTP's.
We wish to modify our permit to reflect the use of another oxidation filtration technique similar
to greensand in that it oxidizes the contaminants into larger particles and then captures them on the media.
The reason Aqua is requesting this change is based on water chemistry and so that the unperinitted ion
exchange filters can be removed. Aqua is familiar with the permitting strategies for WTP's and,
therefore, would like to eliminate this ion exchange discharge. This new WTP will consist of three
oxidation filters. Aqua understands the discharge Flow requirements will remain the same, but does ask
that the limits be changed back to those typically associated with oxidation filtration plants such as
greensand filters.
