In accordance with the application for discharge permit received on October 19. 1998, the
Division is forwarding herewith the subject state - NPDES permit. This permit is issued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated December 6. 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(50) days following receipt of this letter. This request must be in the form of a written petition, -
conforming to Chapter 150E of the North Carolina General Statutes, and filed with the office of
Administrative Hearings, Post Office Drawer 27447, Raleigh, North Carolina 2 76 1 1-744 7. Unless
such a demand is made. this permit shall be final and binding.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land Resources,
Coastal Area Management Act, or any other Federal or Local governmental permits which may be
required.

SUPPLEMENT TO EFFLUENT LIMITATIONS
AND MONITORING REQUIREMENTS
SPECIAL CONDITIONS
A (2). 'CHRONIC TOXICITY PASS/FAIL PERMIT LIMIT (QUARTERLY)
The effluent discharge shall at no time exhibit chronic toxicity using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure." Revised November 1995, or subsequent
versions.
The .effluent concentration at which there may be no observable inhibition of reproduction or significant
mortality is 1.5% (defined as treatment two in the procedure document). The permit holder shall perform
quarterly monitoring using this procedure to establish compliance with the permit condition. The tests will be
performed during the months of January. April, July and October. Effluent sampling for this testing shall be
performed at the NPDES permitted final effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1) for the month in which it was performed. using the parameter code TGP3B.

Test data shall be complete and accurate and include all supporting chemical/physical measurements
performed in association with the toxicity tests, as well as all dose/response data. Total residual chlorine of the
effluent toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste
stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required,
the permittee will complete the information located at the top of the aquatic toxicity (AT) test form indicating
the facility name, permit number, pipe number, county, and the month/year of the report with the notation of
"No Flow" in the comment area of the form. The report shall be submitted to the Environmental Sciences
Branch at the address cited above.
Should any single quarterly monitoring indicate a failure to meet specified limits, then monthly monitoring will
begin immediately until such time that a single test is passed. Upon passing, this monthly test requirement will
revert to quarterly in the months specified above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then monthly
monitoring will begin immediately until such time that a single test is passed. Upon passing, this monthly test
requirement will revert to quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of
Water Quality indicate potential impacts to the receiving stream, this permit may be re-opened and modified to
include alternate monitoring requirements or limits.

Proposed Chanties

• Inclusion of instream monitoring exemption language resulting from
facility's participation in the Yadkin-Pee Dee River Basin Association;
monitoring requirement will activate automatically should the facility
choose to terminate its participation in the Association.
Background
This facility was previously permitted at flows of 1.0 MGD and 2.0 MGD. An ATC
for the expansion from 1 .0 to 2.0 MGD was issued on December 22, 1995 by
Construction Grants. That ATC authorized the modification of the chlorine contact
chamber to accommodate two channels as well as the installation of sodium bisulfite
dechlorination facilities. The basin plan attributes high levels of fecal coliform
bacteria in this area of the Yadkin River to this facility as well as the Wilkesboro
U/WTP. Since November of 1996, this facility has not had a monthly average fecal
coliform value greater than 17 colonies per 100 ml. Furthermore, it is evident from
discharge monitoring reports that dechlorination is online and effectively removing
residual chlorine.
Wasteload Allocation Summary
This discharge was modeled at 2.0 MGD most recently in May of 1995. Although
fecal coliform levels were high at one point, the problem appears to have been
rectified. No limit changes are proposed for conventional or oxygen consuming
parameters.
The only metal monitored for in the permit is silver. Silver has been reported at
less than detection (5 µg/L) for at least the past two years. To that end, no
reasonable potential analysis is necessary. Silver monitoring will be eliminated from
the permit but will be maintained in the facilities Long-Term-Monitoring-Plan
(LIMP).
This facility has recently become a member of the Yadkin-Pee Dee River Basin
Association for instream sampling. As such, instream monitoring has been waived
in lieu of monitoring by the association in accordance with the MOA between the
Division and the Association.
Compliance Review
This facility has been in compliance with its permit since at least December of
1996. Oxygen consuming wastes are being removed effectively as is TSS and fecal
coliform. The dechlorinating system seems to be performing to standards as well.

The Town of North Wilkesboro has a Land Application Permit No. W00002040. Southern Soil
Builders are contracted by the Town to apply biosolids to Town's permitted property.
The Town of North Wilkesboro would like to request that silver monitoring be taken out of this
permit. The reason for this is because results are always lower than the detection limit of<0.005 MG/L.
Note: The Town of North Wilkesboro's W WTP would fall into classification change#.0302(3c)
of proposed revisions to the Water Pollution Control Systems Operator Certification Commission Rules.
Under this new ruling the Town of North Wilkesboro W WTP would be classified as a grade III Facility.
