Dear Mayor Stafford:
In accordance with your request for modification of the instream sampling frequency for fecal coliform, the NPDES
staff has changed the instream sampling frequency from three times per week during the period from November I to March
• 31 to one time per week during the period from November 1 to March 31. In addition to the information provided in your
written request, the NPDES Unit reviewed instream and effluent fecal coliform data from 1999 and 2000. It would appear
that the effluent discharge from Old Fort is not contributing to the fecal coliform problem in Curtis Creek. However,
because of the class of the facility and water quality in Curtis Creek, instream fecal monitoring is still required. Accordingly,
we are forwarding herewith these modifications to the subject permit.
Please find enclosed the amended effluent pages for outfall 001,which should be inserted into your permit. The old
pages should be discarded. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1
and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you,
you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter.
This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes,
and filed with the office of Administrative Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-7447.
Unless such a demand is made,this permit shall be final and binding.
Please take notice that this permit is not transferable. Part II, EA. addresses the requirements to be followed in case
of change in ownership or control of this discharge.

1 Sample Locations: E-Effluent, I-Influent, U—Upstream of discharge location, D—Downstream at the mouth of Curtis Creek. Upstream and downstream samples shall be grab samples.
Stream samples shall be collected three times per week during June,July,August, and September and once per week during the remaining months of the year.
2 The monthly average BOD5 concentration shall not exceed 15%of the respective influent value(85%removal). The monthly average Total Suspended Residue concentration shall not exceed
30%of the respective influent value(70%removal).
3 The daily average dissolved oxygen effluent concentrations shall not be less than 5 mg/I.
a The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units.
5 Monitoring requirement applies only if chlorine is added for disinfection.
