I was looking over the additional information you provided on March 3rd, 2021. Everything looks good except for a few
small issues:
Please correct the following:
- Please provide the original signed copy of the Supplement-EZ Form and O&M Agreement Form. If it is not
possible/practical to provide the original signed hard copies (due to the ongoing Covid pandemic) we will accept
the provided copies with electronic signatures.
- Supplement-EZ Form, Low -Density Page:
o Please show the slope of the swales as a percentage (%), not as a slope (ft/ft).
o Please clarify the fact that some of these swales flow into other swales and only the portion of the
drainage area that flows directly to the swale (not the upstream swales) is shown in the swale table. For
example, the total drainage area to swale 3 should be about 6 acres (Area A + B + D + F) whereas only
drainage area "B" is shown (You can add a note stating that "Swale 2 flows into Swale 4 and Swales 1 &
4 flow into Swale 3 (The drainage areas below show the portion of the drainage area that flows directly
to the swale and the flow takes the entire drainage area into account. Please refer to the calculations for
more information" or something along those lines) or adjust the drainage area information in the table
so that the total drainage area to the swale is shown).
- Swale 4: Per 15A NCAC 02H .1003(2)(c)(i), the maximum allowable side slope of a vegetated conveyance is 3:1. If
you would like to use a steeper side slope, you must provide calculations and an on -site soil investigation
showing that the soils and vegetation will remain stable.
Please provide the following:
- Original signed hard copies of the Supplement-EZ Form and O&M Agreement Form (If applicable).
- 1x hard & 1x electronic copy of the revised Low -Density Page of the Supplement-EZ Form.
- Either updated plan sheets & calculations showing Swale 4 with 3:1 side slopes or calculations & soil
determination showing that the 2:1 side slope will be stable (2x hard copies of plan sheets, 1x hard copy of any
other items, & 1x electronic copy of everything).
Hard copies should be mailed to me at the following address:

Since these are really minor things, once I get the hard copies, anything else that may have changed from addressing
these issues, and the documents are uploaded to Laserfiche (Provided everything is still in order), we can issue the
permit without doing another formal 30-45 day review.

The Application Form is set up in a way that assumes that the project is a high -density project and that the drainage area
columns shown in Section IV, 10 are the drainage areas to the proposed SCMs. However, this project is a low -density
project and there are no "drainage areas" (The entire project area is considered to be a single drainage area*). It doesn't
matter if your neighbor's property drains onto your property, for a low -density project the drainage area is just the
project area* (and the impervious area sum should only include BUA associated with this project). If you are proposing
to add impervious area that is not located within the project area (Such as a driveway entrance located within the public
Right-of-way), you can do this in one of two ways:
- Change the project area to the Limit of Disturbance (LOD) so that there is no new "off -site" impervious area
(since all disturbances should be located within the LOD).
- Show the proposed BUA that will be located outside of the project area on the "Off -site" row of Section IV, 10.
You may want to add a note on this page and/or the Supplement-EZ form explaining what this off -site area is.
Section IV, 11 is not needed since there are no "real" drainage areas for a low -density project.
TL;DR — The form is set up poorly and is not specifically designed for low -density projects.
I'd recommend submitting both the electronic & hard copies at the same time (unless you think there will be a lot of
issues with the submission). After I'm done with the review, if there are some small issues (such as an incorrect
number/value), I can correct the hard copy with a red pen. If a page or two need to be replaced, I can do that, but if
there are any issues with full-sized plan sheets, multiple (more than about 3-ish) pages, or signature pages, a new hard
copy submittal would be needed. You can do either the electronic only (with hard copies to follow) or both hard &
electronic submittal (I'm pretty flexible), it'll take about the same time to review either.
*The exception to this is if there is one or more natural drainage divides on a low -density project. In these cases, the
natural drainage divides divide the project up into "drainage areas" and the sum of these "drainage areas" equals the
total project area.

We wanted to clarify a couple items on the Request for Additional Information for Permit SW3201202 (Yadkin Regional
Water Supply Project — Water Treatment Plant). For Comment 3, which states that "since this project is low -density, the
off -site impervious area determination (Section IV, 11) is not needed" — does that comment apply only to question 11
(how the area was determined), or did you also want us to remove the Offsite Impervious Surface Area from Item IV.10
(the table) and the total impervious area calculation?
Also, would it be acceptable for us to upload the additional information/plans to the edocs portal and for you to look at
them before we send you the hard copies, just in case something looks like it is missing? Or do we need to go ahead and
send the hard copies to you now?
