On March 23, 2020, the Division of Water Resources (Division) received your application requesting
a 401 Water Quality Certification from the Division for the subject project. The Division has
determined that your application is incomplete and cannot be processed. The application is on hold
until the following items are addressed:
1. Please answer application questions #9 and #10 pertaining to the total amount of
disturbance associated with the project [15A NCAC 02H .0506(b)(1)]. Specifically:
a. Total amount of disturbance below the normal pool lake level/normal water level.
b. Total amount of disturbance above the normal pool lake level/normal water level
and 50 feet land-ward.
2. Please provide a diagram and cross-section details for proposed stabilization work per Form
SSGP 09-2020 [15A NCAC 02H .0506(b)] [15A NCAC 02H .0502(c)]. A link to the cross-section
template is provided within the online version of Form SSGP 09-2020 and is included within
the attached help form document on pages 4-6.
3. Please provide photos representing the current conditions of the shoreline [15A NCAC 02H
.0506(b)] [15A NCAC 02H .0502(c)]. The extent of erosion at the site is unable to be
determined due to the quality and text overlay on the photos provided with the application.
Please ensure that you are referencing the correct DWR project number (DWR# 20210437) when
submitting the requested information. The application for this project was submitted using the
incorrect DWR project number and there are duplicates on file. It is recommended that your office
develops a system for keeping track of project numbers, pre-filling meeting requests, and
submittals.
Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested
information for the proper consideration of the application. If all of the requested information is not
received, the Division will be unable to approve the application and it will be returned.
