Wetland Restoration As -Built Report

Dear Mr. Bailey & Ms. Homewood:
As you are aware, on February 27, 2020, Soil & Environmental Consultants, PA (S&EC) on behalf
of Mr. Pierre Goria of Goria Youngs Mill, LLC (the Owner of the referenced property), prepared
and submitted a Pre -construction Notification (PCN) application requesting the issuance of a
Nationwide Permit (NWP) 39 and the associated DWR 401 Water Quality General Certification
(WQGC) No. 4139. The permit requested approval for stream and wetland impacts related to
the proposed site development activities in addition to proposing a plan to resolve a previously
identified violation regarding unauthorized discharge (fill material) within existing site wetlands.
Included in the PCN application were various supporting Construction Drawings, impact maps,
etc. prepared by others. The project site is located approximately 4,000 feet west of the
intersection of McConnell Road and McConnell Center Drive in Greensboro, NC. See the
attached Figure 1- Vicinity Map.
The NWP 39 and WQGC No. 4139 were subsequently issued by your offices on April 21, 2020
and April 13, 2020, respectively. As a condition of both permits a total of 1.58 acres of
wetlands located within the bed of a previously existing pond on the site were required to be
restored as described on the Wetland Restoration Plan (Sheet EXH WL 3 of the Construction
Drawings) prepared by the Triad Design Group of Charlotte, NC. A copy of Sheet EXH WL 3 is
attached as Figure 2.

The purpose of this letter report is to summarize the completion of required wetland
restoration activities, document as -built conditions of this area (as required by both agencies as
conditions of the referenced permits) and provide additional detail regarding future monitoring
activities including the proposed monitoring schedule.
Construction Summary
Based on our discussions with Mr. Goria we understand that a pre -construction site meeting
was conducted on August 14, 2020 to review the Construction Drawings (prepared by the Triad
Design Group of Greensboro, NC), discuss the proposed restoration activities and the schedule
for the work. In attendance were the property Owner, the project design engineer (Mr. Allan
Hill, PE of Triad Design Group), the development manager (Mr. Jason Smith of Windsor
Commercial of Greensboro, NC) and the construction contractor (Mr. Taft Cables of Cable's
Enterprises, LLC of Brown Summit, NC). The Wetland Restoration Plan sheet provides a
Restoration Construction Sequence as well as Wetland Restoration Notes and Planting Plan
which were utilized by the construction contractor while performing the restoration activities.
Restoration grading was performed between August 18, 2020 and October 9, 2020 and
included the removal of approximately 42,000 cubic yards of fill material from within the old
pond bed and the grading of the adjacent slopes. After grading was completed the perimeter
slopes and upland areas were stabilized in accordance with the requirements of the approved
land disturbance permit. Stabilization of the restored wetland area was performed on March 8,
2021 by Sides Seeding & Erosion Control of Lexington, NC, and included hydroseeding the area
with a wetland seed mix and temporary seed. Where necessary hand application of the seed
mix and straw mulch were applied. The wetland seed mix utilized is provided in Attachment 1.
NOTE: Based on our discussions with Mr. Goria we also understand that the existing pond
dam was breached on December 11, 2020. At that time, the riser/barrel spillway was
removed, and the breach channel was lined with riprap and surrounding disturbed area
were stabilized with temporary/permanent seed (and straw mulch applied) and select other
temporary erosion control measures near and within the pond were also removed, all in
general accordance with the Pond Breach Plan. Sheet EXH WL 4 of the Construction
Drawings is attached as Figure 3.
Based on recent discussions with the project design engineer we understand that the wetland
restoration construction, the dam breach, and the associated stabilization of the disturbed
areas were performed in general accordance with the design intent and the approved permit
documents.
As -Built Survey & Photo Documentation
As described on the Wetland Monitoring Plan on Sheet EXH WL 3 the as -built survey for the
restoration work must include photo point documentation from a minimum of three (3) photo

points within the wetland restoration area and a plan view diagram of the restoration area (and
surrounding vicinity). Details regarding each of these requirements is described below.
Following the completion of construction, in coordination with S&EC, the property Owner
installed three (3) photo points within the wetland restoration area to allow for future site
monitoring. The location of each photo point is identified in the field with steel T-post (painted
orange for ease of location). Post -construction field survey data for the wetland restoration
area (including the location of the photo points) and the immediately surrounding vicinity was
collected by Borum, Wade & Associates, PA of Greensboro, NC. This survey data was utilized to
prepare the Topographic Survey Map (As -Built Survey Map) dated April 16, 2021. See Figure 4
attached. The photo points are numbered PNT#1, PNT#2 and PNT#3 and are shown on
Figure 4.
On March 12, 2021 S&EC visited the project site to observe post -restoration site conditions and
collect as -built site photographs at each of the three (3) photo points. While on site we took
multiple photos, in varying directions, at each of the three established photo points to
document as -built conditions. The locations and approximate directional orientation of each of
these photos are shown on the attached Photo Point Map (see Figure 5). See Photos 1 through
9 in Attachment 2. The same photo points (and directional views) will be used in future
monitoring reports to allow for the required year-to-year comparison of site conditions within
the wetland restoration area.
An additional photo was taken at a location upslope of each of the three photo points to
provide a general perspective reference for the location of each of the three photo points
within the restoration area and document overall site conditions at the time of our visit. See
photos 10 through 12. Photos 13 and 14 provide a view of the breach channel from upstream
and downstream, respectively. Based on recent discussions with Mr. Goria we understand that
the wetland restoration area and its surrounding vicinity have continued to stabilize with
vegetative cover.
Post -restoration Monitoring
As a condition of the USACE and DWR permits monitoring of the wetland restoration areas is
required for a period of three (3) years following the completion of the required restoration
activities or until such time that both agencies determine that the restoration area can be
discontinued. S&EC will perform monitoring visits twice per year (in March or April, and again
in August or September) as required by USACE and DWR permit conditions and as described in
approved Wetland Monitoring Plan on Figure 2 (Sheet EXH WL 3).
Site monitoring will be qualitative in nature and will include a general visual assessment of the
overall stability of wetland restoration area and the immediately surrounding slopes and upland
areas. A visual assessment of wetland planting success will be made within the vicinity of each
photo point. As required in the Wetland Monitoring Plan monitoring photos will be taken at
each of the photo points for comparison with those from the preceding monitoring years.

Monitoring reports will be submitted to USACE and DWR not later than December 31' of each
monitoring year and include the information described in the Monitoring Report Guidelines
shown on Sheet EXH WL 3. A copy of the guidelines is provided as Attachment 3.

Monitoring Report Guidelines
A. Monitoring Reports should be concise and effectively provide the information necessary to describe the
site conditions.
B. The Monitoring Report Narrative will contain the following:
1. Project Overview
a. Corps Permit Number.
b. Name of party responsible for conducting the monitoring and the datets) the inspection was
conducted.
c. A brief paragraph describing the purpose of the project, acreage and type restoration
performed.
d. Written description of the location, any identifiable landmarks of the project including
information to locate the project perimeter, and coordinates of the project site.

2. Summary Data
a. Provide photo documentation to support the findings and recommendations referenced in
the Monitoring Report. Submitted photos should be formatted to print on a standard 8.5" x
11" piece of paper, dated and dearly labeled with the direction from which the photo was
taken. The photo location points should also be identified on the appropriate maps.
3. Maps and Plans
a. Maps should be provided to show the location of the project site relative to other landscape
features, locations of photographic reference points, and/or other features pertinent to the
restoration plan.
b. The submitted maps and plans should delineate the site perimeter,
