On September 27, 2012 Sue Homewood of this office conducted a site inspection of the above
referenced project On August 10, 2009 an approval of a 401 Water Quality Certification was
issued to you for impacts to streams and wetlands for the Kernersville Medical Parkway located
in Kernersville NC. Ms. Homewood's site inspection determined that the project had
commenced construction prior to September 27, 2Q12 however the Division of Water Quality
had not been contacted regarding a pre - construction meeting.
Accordingly, the following permit condition violations were noted during the Division of Water
Quality inspection and subsequent file review-
Condition Number 5 of the Approval letter for 401 Water Quality Certification #09 -0689
states that "The Permittee or their designee will conduct a Pre - Construction Meeting with
the environmental consultants, stream crossing designers /engineers, and the contractor to
discuss permit conditions, construction sequences and other site specific issues The
Division of Water Quality and the US Army Corps of Engineers shall be invited to the
pre - construction meeting "
At this time the Division does not require a response to this Notification, however please ensure
that your project complies with all conditions of 401 Water Quality Certification #09 -0689 as
construction continues.
