Dear Mr. Uskiewicz:
Enclosed please find a copy of the Compliance Evaluation Inspection Report from the inspection conducted on
March 18, 2005 by Mike Lawyer of the Fayetteville Regional Office. The inspection revealed the following items,
which must be addressed. The chlorine meter being used for the weekly chlorine monitoring must be calibrated
either annually with a five -point curve or before each use with a three-point curve. Documentation of this
calibration must be kept in a manner in which it can be presented upon request by an inspector. There was no ORC
visitation log. It was suggested during the inspection that Mr. Glover initial the daily operations and maintenance
book on those days that he is present at the plant. Also, the Dunn WTP is required to either attain a certification to
perform the upstream and downstream monitoring of turbidity or have another facility that is currently certified to
perform the tests. Information on attaining certification for specific parameters can be found under the Laboratory
Section of our website, located at the bottom of this page. Please respond to this office in writing as to your plans
to address the aforementioned items by April 30, 2005.

A review of the DMR's from July 2004 through January 2005 revealed no permit violations with regards to
effluent monitoring. During the inspection it was noted that the chlorine meter is not being calibrated and
that there was no ORC visitation log. The chlorine meter must be calibrated either annually with a five
point curve or before each test with a three point curve. Personnel at the Dunn WWTP can assist in this
calibration. It is suggested that the ORC initial the daily maintenance book with which the Back-up ORC's have
been signing on the days he is present at the plant. At the time of inspection, the grounds of the plant
appeared to be well -maintained and the discharge coming over the weir at the return pump basin was clear.

I am responding, to the corrections we need to make for our NPDES permit.
have reviewed all the recommended changes in your Evaluation Report.
1. The Chlorine Meter at the WTP is going to be Calibrated Annually with a 5 Point
Calibration Curve. We will keep this record on file for review. The WWTP is
currently performing this test for us in the 15 minute hold time allowed for Total
Chlorine Residuals.
2. The ORC Visitation Log issue will be handled by my signing the Operators Daily
Logbook each day. I am typically on the job 5 days a week. I will have the
Backup ORC sign in on the days I am not at the Plant for vacation or sickness
related absences.
3. The Upstream and Downstream Monitoring, for Turbidity is currently being
performed by Chemical and Environmental Tech. A Certified Lab in Cary, NC.
The City of Dunn is currently a member of the Middle Cape Fear River Basin
Association. I am hoping this contract removes us from the requirement of having
to sample for up and downstream monitoring.
Attached is a copy of our contract with the MCRBA.
Also attached is a copy of the Operators Daily Log Sheet with my signature as the
ORC signed in.

Thank you, for considering Meritech, Inc. for your testing needs. The following is an
outline of the parameters that you have requested along with the associated costs.
Our standard turn around time for the analyses is ten (10) working days. The complete
spreadsheet based report will be complete within thirty days (30) of the month the
samples are taken
Meritech, Inc. will use the current MOA as guidance to sampling and analysis, some of

The pricing reflects costs incurred the first year of sampling and analysis. Each
subsequent year would be subject to an increase of three percent (3%) per year, plus
inflation, to the total investment.
The above terms would be valid for three years from the date of acceptance.
Meritech, Inc. would require sixty days (60) to implement the sampling plan from the
date the contract is awarded.
