The following is an abbreviated application form for those applying for a 401 Water Quality
Certification to impact FERC (Federal Energy Regulatory Commission) regulated lake shore or
bottom under the U.S. Army Corps of Engineers Permit GP30. This Application cannot be used
for wetland or stream impacts. Please provide five (5) copies of this application and supporting
information as well as a non - refundable application fee to the I)ivision of Water Quality of $240.00
for impacts to lake bottom of less than 1 acre and $570.00 for impacts to lake bottom of greater or
equal to 1 acre.

The description of the planned excavation is as follows:
Purpose is to allow us to get a pontoon boat up to our existing dock, as we are regularly
dry docked due to depth issues.
Dredge 1,555 cubic yards. 240'x 70'. 5' foot depth at the deepest.
Machinery to be used: track hoe and dump truck
Haul away off site
We have 3 sides that are rip rap retaining wall.
We have Terry Leonard scheduled to do the work upon approval.
Schedule for work is as soon as approved. Duration of job is
approximately 5 complete working days, dependent on weather.
