Attached is a Notification of Intent to Construct a CLOSED-LOOP, WATER-ONLY
Geothermal System. This form is to be used only if the system circulates potable water in
continuous piping that completely isolates the fluid from the environment. If your system has had
any sort of coolant added to the fluid or if it does not have continuous piping, you must apply for
a Injection Well Permit. You can contact me at 715-6165 for a permit application.
I understand that you will be shutting down for a few weeks, and hope that you can address this
