Box 1111 Statesville, NC 28677 SUBJECT: Permit No. NC0031836 Authorization to Construct City of Statesville Fourth Creek Wastewater. Treatment Plant Expansion to 6.0 MGD Capacity Iredell County Dear Mr. Connet: A letter of request for an Authorization to Construct was received September 29, 1987, by the Division and final plans and specifications for the subject facilities expansion have been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of additions to the City of Statesville's existing 4.0 MGD Fourth Creek Wastewater Treatment Plant to increase capacity to 6.0 MGD, said additions consisting of a third 70 ft. diameter x 12 ft. deep clarifier, a second chlorine contact and effluent reaeration basin, a new aerated sludge digestion basin with three (3) 60 H.P. mechanical aerators, a new sludge pumping station with dual 25 H.P. rotary lobe sludge transfer pumps, a belt -type filter press and polymer conditioning system for sludge dewatering, and associated piping, valves, fittings and appurtenances. This Authorization to Construct is issued in accordance with Part III paragraph C of the NPDES Permit No. NC0031836 issued November 19, 1987, and shall be subject to revocationunless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0031836. The sludge wastes generated from these treatment facilities must be disposed of in accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. The Permittee must employ a certified wastewater operator in accordance with Part III paragraph D of the referenced permit. Pollution Prevention Pays P.O. Such notification to the Regional Supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. In event the facilities fail to perform satisfactorily in meeting its NPDES permit effluent limits, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of Additional wastewater treatment and disposal facilities. Four monitoring wells, one (1) upgradient and three (3) downgradient, shall be installed to monitor groundwater quality. The location and construction details for these wells must be by approval of the Division's Mooresville Regional Office, from which a well construction permit must be obtained. The monitoring wells shall be sampled initially after construction and thereafter every March, .luly, and November for the following parameters: NO3 Total Ammonia Total Dissolved Solids Water Levels Total Coliforms pH Total Organic Carbon Chloride Total Organic Halides (in November only) Total Suspended Solids (Initial sampling only) The measurements of water level must be made prior to sampling for the remaining parameters. The analytical methods used for Total Organic Halides must be capable of detecting the total of all halogenated organic compounds present at a concentration of 5.0 parts per billion (ppb) or greater. Any individual halogenated organic compound present at or above the method detection limit (MDL) must be identified, quantified and reported with the sampling results. The results of the sampling and analysis shall be sent to the Division of Environmental Management on Form GW-59 (Compliance Monitoring Report Form) every April, August and December. Any additional groundwater quality monitoring deemed necessary by the Division shall be provided. The aerobic digestor basin shall be constructed with a compacted Bentonite/clay liner at least one foot thick at all locations and with a maximum permeability not to exceed 1 x 10 —6 cm/sec. Following installation and inspection of the basin liner, and prior to use, vertification of the liner's compliance with permeability and thickness requirements shall be provided to the Division by the project Engineer.
