In accordance with your application received on June 6, 2005, and additional information
received on June 20, 2005, we are forwarding herewith Permit No. WQ0020654, dated June 22, 2005,
to the Town of Harrisburg for the construction and operation of the subject wastewater collection
system extension, This permit shall be effective from the date of issuance until rescinded, shall void
Permit Nov WQ0020654 issued May 24, 2004, and shall be subject to the conditions and limitations as
specified therein. This cover letter shall be considered a part of this permit and is therefore incorporated
therein by reference. The modification includes the upgrade of an existing 250 gallon per minute pump
station (Abbington) to 400 gpm.
Please pay particular attention to Permit Condition 3 which requires that the wastewater
collection facilities be properly operated and maintained in accordance with 15A NCAC 2H ,0227 or any
individual system -wide collection system permit issued to the Permittee.
Permitting of this project does not constitute an acceptance of any part of the project that does
not meet 1) 15A NCAC 2H .0200; 2) the Division of Water Quality's (Division) Gravity Sewer Minimum
Design Criteria adopted February 12, 1996, as applicable; 3) and the Division's Minimum Design
Criteria for the Fast -Track Permitting of Pump Stations and Force Mains adopted June 1, 2000, as
applicable, unless specifically mentioned herein, Division approval is based on acceptance of the
certification provided by a North Carolina -licensed Professional Engineer in the application. It shall be
the Permittee's responsibility to ensure that the as -constructed project meets the appropriate design
criteria and rules. Failure to comply may result in penalties in accordance with North Carolina General
Statute §143-215.6A through §143-215.6C, construction of additional or replacement wastewater
collection facilities, and/or referral of the North Carolina -licensed Professional Engineer to the licensing
board.

This permit shall be effective from the date of issuance until rescinded and shall be subject to the
following specified conditions and limitations:
1. This permit shall become voidable unless the wastewater collection facilities are constructed in
accordance with the conditions of this permit; 15A NCAC 2H .0200; the Division of Water Quality's
(Division) Gravity Sewer Minimum Design Criteria adopted February 12, 1996, as applicable; the
Division`s Minimum Design Criteria for the Fast -Track Permitting of Pump Stations and Force
Mains adopted June 1, 2000, as applicable; and other supporting materials unless specifically
mentioned herein.
2. This permit shall be effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
3. The wastewater collection facilities shall be properly maintained and operated at all times. The
Permittee shall maintain compliance with an individual system -wide collection system permit for the
operation and maintenance of these facilities as required by 15A NCAC 2H .0227. If an individual
permit is not required, the following performance criteria shall be met as provided in 15A NCAC 2H
.0227:
a. The sewer system shall be effectively maintained and operated at all times to prevent
discharge to land or surface waters, and any contravention of the groundwater standards in
15A NCAC 2L .0200 or the surface water standards in 15A NCAC 2B .0200.
b. A map of the sewer system shall be developed and shall be actively maintained.
c. An operation and maintenance plan shall be developed and implemented.
d. Pump stations that are not connected to a telemetry system shall be inspected every day (i.e.
365 days per year). Pump stations that are connected to a telemetry system shall be
inspected at least once per week,
e. High -priority sewer lines shall be inspected at least once per every six-month period of time.
f. A general observation of the entire sewer system shall be conducted at least once per year.
g. Inspection and maintenance records shall be maintained for a period of at least three years.
h. Overflows and bypasses shall be reported to the appropriate Division regional office in
accordance with 15A NCAC 2B .0506(a), and public notice shall be provided as required by
North Carolina General Statute §143-215.1C.

4, This permit shall not be transferable. In the event there is a desire for the wastewater collection
facilities to change ownership„ or there is a name change of the Permittee, a formal permit request
shall be submitted to the Division accompanied by documentation from the parties involved, and
other supporting materials as may be appropriate. The approval of this request shall be
considered on its merits and may or may not be approved.
5. Construction of the gravity sewers, pump stations, and force mains shall be scheduled so as not to
interrupt service by the existing utilities nor result in an overflow or bypass discharge of wastewater
to the surface waters of the State,
6. Per 15A NCAC 2H .0220, upon completion of construction and prior to operation of these
permitted facilities, the completed Engineering Certification form attached to this permit shall be
submitted with the required supporting documents to the address provided on the form. A complete
certification is one where the form is fully executed and the supporting documents are provided as
applicable.
7. A copy of the construction record drawings shall be maintained on file by the Permittee for the life
of the wastewater collection facilities.
Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2H .0200; the
Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable; the Division's
Minimum Design Criteria for the Fast -Track Permitting of Pump Station and Force Mains adopted
June 1, 2000 as applicable; and other supporting materials may subject the Permittee to an
enforcement action by the Division, in accordance with North Carolina General Statutes §143-
215.6A through §143-215.6C,
9. In the event that the wastewater collection facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Perrnittee shall take immediate corrective action, including
those as may be required by this Division, such as the construction of additional or replacement
facilities.
10. The issuance of this permit shall not exempt the Pemiittee from complying with any and all
statutes, rules, regulations, or ordinances that may be imposed by other government agencies
(local, state and federal) which have jurisdiction, including but not limited to applicable river buffer
rules in 15A NCAC 26 .0200, erosion and sedimentation control requirements in 15A NCAC Ch. 4
and under the Division's General Permit NCG010000, and any requirements pertaining to wetlands
under 15A NCAC 2B .0200 and 15A NCAC 2H .0500.
11. Noncompliance Notification:
The Permittee shall verbally report to a Division of Water Quality employee at the Mooresville
Regional Office, telephone number [phone], as soon as possible, but in no case more than
24 hours or on the next working day, following the occurrence or first knowledge of the occurrence
of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility incapable
of adequate wastewater transport, such as mechanical or electrical failures of pumps, line
blockage or breakage, etc,; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving waters
without treatment of all or any portion of the influent to such station or facility.

Voice mail messages or faxed information is permissible, but shall not be considered as the
Initial verbal report. Overflows and spills occurring outside normal business hours may also be
reported to the Division of Emergency Management at telephone number [phone] or
[phone]. Persons reporting any of the above occurrences shall file a spill report by
completing Part I of Form - (or the most current Division approved form), within five days
following first knowledge of the occurrence. This report shall outline the actions taken or
proposed to ensure that the problem does not recur. Part 11 of Form CS-SSO(or the most.
current Division approved form) can also be completed to show that the SSO was beyond
control,

I am reviewing the subject sewer extension for the Town of Harrisburg in
Cabarrus County. This is a modification for an existing permit WQ.0020654
to add. the pump station only; however upon review it is not clear if the
owner is upgrading an existing pump station r250 gpm Abbington) or
constructing a new pump station.
Please clarify.
Also the certification page 6 of the application (Section D-1) has Carl
Parmer certifying the LaSalle Subdivision project (this should be
Kensington Forest/Abbington Subdivision).

in accordance with your application received on May 10, 2004, we are forwarding herewith
Permit No. WQ0020654, dated May 24, 2004, to the Town of Harrisburg for the construction and
operation of the subject wastewater collection system extension. This permit shall be effective from the
date of issuance until rescinded, shall void Permit No. WQ0020654 issued December 14, 2001, and
shall be subject to the conditions and limitations as specified therein. This cover letter shall be
considered a part of this permit and is therefore incorporated therein by reference. The modificati
on
includes the upgrade of the existing Abbington pump station from 150 gallons per minute (gem) to 250
gpm, and the addition of a 200-gpm pump station (Kensington), approximately 12,104 linear feet of 6-
inch gravity sewer, approximately 5,452 linear feet of 6-inch force main and 70,200 gallons per day of
flow.
Please pay particular attention to Permit Condition 3 which requires that the wastewater
collection facilities be properly operated and maintained in accordance with 15A NCAC 2H ,0227 or any
individual system -wide collection system permit issued to the Permittee.
Permitting of this project does not constitute an acceptance of any part of the project that does
not meet 1)15A NCAC 2H .0200; 2) the Division of Water Quality's (Division) Gravity Sewer Minimum
Design Criteria'adopted February 12, 1996, as applicable„ 3) and the Division's Minimum Design
Criteria for the Fast -Track Permitting of Purnp Stations and Force Mains adopted June 1, 2000, as
applicable, unless specifically mentioned herein. Division approval is based an acceptance of the
certification provided by a North Carolina -licensed Professional Engineer in the application. It shall be
the Pem'nittee's responsibility to ensure that the as -constructed project meets the appropriate design
criteria and rules. Failure to comply may result in penalties in accordance with North Carolina General
Statute §143-215.6A through §143-215.6C, construction of additional or replacement wastewater
collection facilities, and/or referral of the North Carolina -licensed Professional Engineer to the licensing

adopted February 12, 1996, as applicable; the Division's Minimum Design Criteria for the Fast -Track

right to request an adjudicatory hearing upon written request within 30 days foliovwing receipt of this
permit. This request must be in the form of a written petition, conforming to Chapter 150B of North
Carolina. General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service

This permit shall be effective from the date of Issuance until rescinded and shall be subject to the
following specified conditions and limitations:
1. This permit shall become voidable unless the wastewater collection facilities are constructed in
accordance with the conditions of this permit; 15A NCAC 2H .0200; the 'Division of Water Quality's
(Division) Gravity Sewer Minimum Design Criteria adopted February 12, 1996, as applicable; the
Division's Minimum Design Criteria for the Fast -Track Permitting of Pump Stations and Force
Mains adopted June 1, 2000, as applicable; and other supporting materials unless specifically
mentioned herein.
2, This permit shall be effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
3. The wastewater collection facilities shall be properly maintained and operated at a times. The
Permittee shall maintain compliance with an individual system -wide collection system permit for the
operation and maintenance of these facilities as required by 15A NCAC 2H .0227, If an individual
permit is not required, the following performance criteria shall be met as provided in 15A NCAC 2H
0227:
a. The sewer system shall be effectively maintained and operated at ail times to prevent
discharge to land or surface waters, arid any contravention of the groundwater standards in
15A NCAC 21...0200 or the surface water standards in 15A NCAC 28 ,0200.
b. A map of the sewer system shall be developed and shall be actively maintained.
c. An operation and maintenance plan shall be developed and implemented.
d. Pump stations that are not connected to a telemetry system shall be inspected every day (i.e.
365 days per year). Pump stations that are connected to a telemetry system shah be
inspected at least once -r week.
e. High -priority sewer lines shall be inspected at least once per every six-month period of time.
f. A general observation of the entire sewer system shall be conducted at least once per year.
g. Inspection and maintenance records shall be maintained for a period of at least three years.
h, Overflows and bypasses shall be reported to the appropriate Division regional office in
accordance with 15A NCAC 2B .0506(a), and public notice shah be provided as required by
North Carolina General Statute §143-215.1 C.

shah be submitted to the division accompanied by documentation from the parties involved, and
other supporting materials as may be appropriate. The approval of this request shall be
considered on its merits and may or may not be approved.
Construction of the gravity sewers, pump stations, and force mains shall be scheduled so as not to
interrupt service by the existing utilities nor result in an overflow or bypass discharge of wastewater
to the surface waters of the State.
Upon completion of construction and or tea crib of these permitted facilities, a certification, a
copy of the construction record drawings, as well as supporting design calculations for any pump
stations permitted as part of this project shall be received from a North Carolina -licensed.
Professional Engineer certifying that the facilities have been installed in accordance with this
permit; 15A NCAC 2H .02Q0; the Division's Gravity Sewer Design Criteria adopted February 12,
1996 as applicable; the Division's Minimum Design Criteria for the Fast -Track Permitting of Pump
Station and Force Main adopted June 1, 2000 as applicable; and other supporting materials. if this
project is to be completed in phases and partially certified, you shall retain the responsibility to
track further construction approved under the same permit, and shall provide a final certificate of
completion once the entire project has been completed,, A copy of the construction record
drawings, indicating the facilities constructed in the phase being certified, shall be submitted with
each partial certification. Mall the Engineer's Certification, one copy of the °Construction Record
Drawings," and one copy of the supporting design calculations to NCDENR, Water Quality, 919
North Main Street, Mooresville, NC 28115.
7. A copy of the construction record drawings shall be maintained on file by the Pern ittee for the life
of the wastewater collection facilities.
d. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2H .0200; the
Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable; the Division`s
Minimum Design Criteria for the Fast -Track Permitting of Pump Station and Force Mains adopted
June 1, 2000 as applicable; and other supporting materials may subject the Permittee to an
enforcement action by the Division, in accordance with North Carolina General Statutes §143-
215.6A through §143-215,6C.
9, In the event that the wastewater collection facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take immediate corrective action, including
those as may be required by this Division, such as the construction of additional or replacement
facilities.
10. The issuano$ of this permit shall not exempt the Permittee from complying with any and ail
statutes, rules, regulations, or ordinances that may be imposed by other government agencies
(local, state and federal) which have jurisdiction, including but not limited to applicable river buffer
rules in 15A NCAC 2B .0200, erosion and sedimentation control requirements in 15A NCAC Ch. 4
and under the. Division's General Permit NCG01 Q00Q, and any requirements pertaining to wetlands
under 15A NCAC 2B .0200 and 15A NCAC 2H .0500.

11. Noncompllartce Notification:
The Permittee shall verbally report to a Division of Water Quail s+' employeeat the Mooresville
Regional Office, telephone number [phone], as soon as possible, but in no case more thane.
24 hours or on the next working day, following the occurrence or first knowledge of the occurrence
of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility incapable
of adequate wastewater transport, such as mechanical or electrical failures of pumps, line
blockage or breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting In a by-pass directly to receiving waters
without treatment of all or any portion of ttte influent to such station or facility.
Voice mail messages or faxed information is permissible, but shall not be considered as the initial'
verbal report. Overflows and spills occurring outside normal business hours trnay y at or 91eso be �73ed
to the Division of Emergency Management at telephone number (600) 5
3300. Persons reporting any of the above occurrences shall file a spill report by completing Part I
of Form CS-SSO (or the most current Division approved form), within five days following first
knowledge of the occurrence. This report shall outline the actions taken or proposed to ensure that
the problem does not recur. Part ll of Form CS-SSO (or the most current Division approved form)
can also be completed to show that the SSO was beyond control.

stations permitted as part of this project shall be received from a North Carolina -licensed
Professional Engineer certifying that the facilities have been installed in accordance with this
permit; 15A NCAC 2H .0200; the Division's Gravity Sewer Design Criteria adopted February 12,
1996 as applicable; the Division's Minimum Design Criteria for the Fast -Track Permitting of Pump
Station and Force Main adopted June 1, 2000 as applicable; and other supporting materials. If this
project is to be completed in phases and partially certified, you shall retain the responsibility to
track further construction approved under the same permit, and shall provide a final certificate of
completion once the entire project has been completed. A copy of the construction record
drawings, indicating the facilities constructed in the phase being certified, shall be submitted with
each partial certification. Mail the Engineer's Certification, one copy of the "Construction Record
Drawings," and one copy of the supporting design calculations to NCDENR, Water Quality, 919
North Main Street, Mooresville, NC 28115.
7. A copy of the construction record drawings shall be maintained on file by the Permittee for the life
of the wastewater collection facilities.
8. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2H .0200; the
Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable; the Division's
Minimum Design Criteria for the Fast -Track Permitting of Pump Station and Force Mains adopted
June 1, 2000 as applicable; and other supporting materials may subject the Perrnittee to an
enforcement action by the Division, in accordance with North Carolina General Statutes §143-
215.6A through §143-215.6C.
9. In the event that the wastewater collection facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take immediate corrective action, including
those as may be required by this Division, such as the construction of additional or replacement
facilities.
10. The issuance of this permit shall not exempt the Permittee from complying with any and all
statutes, rules, regulations, or ordinances that may be imposed by other government agencies
(local, state and federal) which have jurisdiction, including but not limited to applicable river buffer
rules in 15A NCAC 2B .0200, erosion and sedimentation control requirements in 15A NCAC Ch. 4
and under the Division's General Permit NCG010000, and any requirements pertaining to wetlands
under 15A NCAC 2B .0200 and 15A NCAC 2H .0500.

11, Noncompliance Notification:
The Permittee shall verbally report to a Division of Water Quality employee at the Mooresville
Regional Office, telephone number [phone], as soon as possible, but in no case more than
24 hours or on the next working day, following the occurrence or first knowledge of the occurrence
of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility incapable
of adequate wastewater transport, such as mechanical or electrical failures of pumps, line
blockage or breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving waters
without treatment of all or any portion of the Influent to such station or facility.
Voice mail messages or faxed information is permissible, but shall not be considered as the initial
verbal report. Overflows and spills occurring outside normal business hours may also be reported
to the Division of Emergency Management at telephone number [phone] or (919) 733-
3300. Persons reporting any of the above occurrences shall file a spill report by completing Part I
of Form CS-SSO (or the most current Division approved form), within five days following first
knowledge of the occurrence. This report shall outline the actions taken or proposed to ensure that
the problem does not recur. Part II of Form CS-SSO (or the most current Division approved form)
can also be completed to show that the SSO was beyond control.

for the following reason(s):
✓ Per the attached letter, the central office no longer handles Fast -Track permitting of sewer
extensions. The revised application instructs as to where to submit the application. In this
instance, the application would be sent to Sonja Williams at our Mooresville Regional Office,
✓ A flow acceptance letter was not provided by the owner of the downstream sewers and/or the
owner of the receiving wastewater treatment facility as is required by Instruction V. of the
application. Flow acceptance letters must contain the following minimum information: applicant and
project name, amount of flow accepted and name and permit number of the receiving wastewater
treatment facility. The flow acceptance must not expire prior to permit issuance and must be dated
less than a year prior to the application date. Intergovernmental agreements or other contracts will
not be accepted in lieu of a project -specific flow acceptance letter. While the cover letter indicates a
flow acceptance letter was included, none was located. The Division needs the flow acceptance
letter to proceed.
Item A(2)(a) of the application indicates the project name as Kensington Forest with (Abbington
Subdivision) next to it. The Division has one permit issued for Abbington Subdivision for Phase 1 &
7 under WQ0020654. Item B(1) indicates this is permit modification. However, the permit number
shown is not a Division of Water Quality issued number. Please provide the correct permit number
if this is a permit modification. The permit should start with "WQ00."

This is in .response to the Town's request for flow acceptance received 4/30/04. The Water
and Sewer Authority of Cabarrus County (WSACC) has or currently expects to be able to make
available capacity at the Rocky River Wastewater Treatment Plant and appropriate sewer
interceptors to transport and treat the domestic wastewater from the proposed Kensington Forest
Subdivision consisting of 188 residential lots; WSACC agrees to accept wastewater from the
proposed sewer line based on information provided by Latham -Walters Engineering, Inc. (Civil
Engineer for the project) using North Carolina 'Department of Environment and Natural Resources
(NCDENR) wastewater flow guidelines. The projected flow is 70,200 gpd; however WSACC
expectsthe actual wastewater flow to more closely approximate 48,750 gpd based upon Master Plan
engineering data. The Town of Harrisburg understands that separate Flow Acceptance Requests
will be required for the future flows. that will connect into this sewer line.
By requesting flow acceptance, the Town of Harrisburg acknowledges that WSACC may
need to upgrade, expand, or construct new facilities to accommodate the increased flow volume
granted by this and previous flow acceptance. Appropriate design and construction will be initiated
when actual flows dictate but may be subject to situations that WSACC does not expect or control.
Based upon flow of 70,200 gpd and cost of 58.126/gpd to provide treatment and transportation
capacity, this flow acceptance represents an estimated capital expenditure in excess of $570,445..
For your information and based upon NCDENR wastewater flow guidelines. WSACC has
approved 1,325,497 gallons of flow acceptance for the Town of Harrisburg from 1992 to present
and 205,200 gallons of flow acceptance year-to-date including this request. Based upon the
approved flow acceptances WSACC has committed capacity valued in excess of $10„770,988 for
the Town of Harrisburg since 1992 including $1,667,455 for this calendar year.
Downstream facilities from the proposed project are scheduled in WSACC's current Capital
"Improvement Program (CIP) to increase available capacity. Projects on the CIP are subject to
approval by WSACC's Board of Directors, support from local governing bodies, regulatory

approval, and funding availability. The following schedule of CIP projects is tentative and may be
delayed by unforeseen cireumstaames:
• Back Creek Interceptor will be paralleled. Scheduled completion date: 2005,

Please note that if construction of this project is not started W ithin one year of the date of this

WQ0020654, dated December 14, 2001, to the Town of Harrisburg for the construction and operation of the
subject wastewater collection system extension. This permit shall be effective from the date of issuance until
rescinded and shall be subject to the conditions and limitations as specified therein. This cover letter shall be
considered a part of this permit and is therefore incorporated therein by reference.
Please pay particular attention to Permit Condition 3 which requires that the wastewater collection
facilities be properly operated and maintained in accordance with 15A NCAC 2H .0227 or any individual system-
wide collection system permit issued to the Penrnittee.
Pei miffing of this project does not constitute an acceptance of any part of the project that does not meet
15A NCAC 2H .0200; the Division of Water Quality's (Division) Gravity Sewer Minimum Design Criteria
adopted February 12, 1996 as applicable; and the Division's Minimum Design Criteria for the Fast -Track
Permitting of Pump Stations and Force Mains adopted June 1, 2000 as applicable, unless specifically mentioned
herein. Division approval is based on acceptance of the certification provided by a North Carolina -licensed
Professional Engineer in the application. It shall be the Permittee's responsibility to ensure that the as -constructed
project meets the appropriate design criteria and rules. Failure to comply may result in penalties in accordance
with North Carolina General Statute §143-215.6A through §143-215.6C, construction of additional or
replacement wastewater collection facilities, andJor referral of the North Carolina -licensed Professional Engineer
to the licensing board.
In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations, permission is hereby granted to the Town of
Harrisburg for the construction and operation of approximately 9,450 linear feet of 8-inch gravity sewer (Phase
1), and 2,811 linear feet of 8-inch gravity sewer (Phase 7); a 150—gallon per minute pump station with duplex
pumps, on -site audible and visual high water alarms, and portable generator quick -connect receptacle; as well as
approximately 1,486 linear feet of 4-inch force main to serve 65 three bedroom residences (Phase I) and 58
three -bedroom residences (Phase 7) as part of the Abbington Subdivision -Phase 1 & 7 project, and the discharge
of 44,280 gallons per day of collected domestic wastewater into the Water & Sewer Authority of Cabarrus
County's existing sewerage system, pursuant to the application received November 1, 2001 and in conformity
with 15A NCAC 2H .0200; the Division's Gravity Sewer Minimum Design Criteria adopted February 12, 1996 as

applicable; the Division's Minimum Design Criteria for the Fast -Track Permitting of Pump Stations and Force
Mains adopted June 1, 2000 as applicable; and other supporting data subsequently filed and approved by the
Department of Environment and Natural Resources and considered a part of this permit.
The sewage and wastewater collected by this system shall be treated in the Water & Sewer Authority of
Cabarrus County Wastewater Treatment Facility (Permit No. NC0036269) prior to being discharged into the
receiving stream.
This permit shall become voidable unless the agreement between. Town of Harrisburg and the Water &
Sewer Authority of Cabarrus County for the collection and Final treatment of wastewater is in full force and
effect.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to
request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request
must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and
filed with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, NC 27699-671.4. Unless
such demands are made, this permit shall be final and binding.

WASTEWATER COLLECTION SYSTEM EXTENSION PERMIT
This permit shall be effective from the date of issuance until rescinded and shall be subject to the following
specified conditions and limitations:
1. This permit shall become voidable unless the wastewater collection facilities are constructed in accordance
with the conditions of this permit; 15A NCAC 2H .0200; the Division of Water Quality's (Division) Gravity
Sewer Minimum Design Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design
Criteria for the Fast -Track Permitting of Pump Stations and Force Mains adopted June 1, 2000 as applicable;
and other supporting materials unless specifically mentioned herein.
2. This permit shall be effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
3. The wastewater collection facilities shall be properly maintained and operated at all times. The Permittee
shall maintain compliance with an individual system -wide collection system permit for the operation and
maintenance of these facilities as required by 15A NCAC 2H .0227. If an individual permit is not required,
the following performance criteria shall be met as provided in 15A NCAC 211 .0227:
a. The sewer system shall be effectively maintained and operated at all times to prevent discharge to land
or surface waters, and any contravention of the groundwater standards in 15A NCAC 2L .0200 or the
surface water standards in 15A NCAC 213 .0200.
b. A map of the sewer system shall be developed prior to January 1, 2004 and shall be actively
maintained.
c. An operation and maintenance plan shall be developed and implemented.
d. Pump stations that are not connected to a telemetry system shall be inspected at least three times per
week until July 1, 2000; thereafter, pump stations shall be inspected at least daily, as defined in 15A
NCAC 213 .0503(5), until July 1, 2001; and thereafter, pump stations shall be inspected every day.
Pump stations that are connected to a telemetry system shall be inspected at least once per week.
e. High -priority sewer lines shall be inspected at least once per every six-month period of time.
f. A general observation of the entire sewer system shall be conducted at least once per year.
g. Inspection and maintenance records shall be maintained for a period of at least three years.
h. Overflows and bypasses shall be reported to the appropriate Division regional office in accordance with
15A NCAC 2B .0506(a), and public notice shall be provided as required by North Carolina General
Statute §143-215.1C.
4, This permit shall not be transferable. In the event there is a desire for the wastewater collection facilities
to change ownership, or there is a name change of the Permittee, a formal permit request shall be submitted
to the Division accompanied by documentation from the parties involved, and other supporting materials as
may be appropriate. The approval of this request shall be considered on its merits and may or may not be
approved.

5. Construction of the gravity sewers, pump stations, and force mains shall be scheduled so as not to interrupt
service by the existing utilities nor result in an overflow or bypass discharge of wastewater to the surface
waters of the State.
6. Upon completion of construction and prior to operation of these permitted facilities, a certification, a copy of
the construction record drawings, as well as supporting design calculations for any pump stations permitted
as part of this project shall be received from a North Carolina -licensed Professional Engineer certifying that
the facilities have been installed in accordance with this pei mit; 15A NCAC 2H .0200; the Division's
Gravity Sewer Design Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design
Criteria for the Fast -Track Permitting of Pump Station and Force Main adopted June 1, 2000 as applicable;
and other supporting materials. If this project is to be completed in phases and partially certified, you shall
retain the responsibility to track further construction approved under the same permit, and shall provide a
final certificate of completion once the entire project has been completed. A copy of the construction record
drawings, indicating the facilities constructed in the phase being certified, shall be submitted with each
partial certification. Mail the Engineer's Certification, one copy of the "Construction Record Drawings",
and one copy of the supporting design calculations to the Non -Discharge Permitting Unit, 1617 Mail Service
Center, Raleigh, NC 27699-1617.
7. A copy of the construction record drawings shall be maintained on file by the Permittee for the life of the
wastewater collection facilities.
Failure to abide by the conditions and limitations contained in this permit; I 5A NCAC 2H .0200; the
Division's Gravity Sewer Design Criteria adopted February 12,. 1996 as applicable; the Division's Minimum
Design Criteria for the Fast -Track Permitting of Pump Station and Force Mains adopted June 1.., 2000 as
applicable; and other supporting materials may subject the Permittee to an enforcement action by the
Division, in accordance with North Carolina General Statutes §143-215.6A through §143-21.5.6C.
9, In the event that the wastewater collection facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Perm.ittee shall take immediate corrective action, including those as may be required
by this Division, such as the construction of additional or replacement facilities.
10. The issuance of this permit shall not exempt the Perrnittee from complying with any and all statutes, rules,
regulations, or ordinances that may be imposed by other government agencies (local, state and federal) which
have jurisdiction, including but not limited to applicable river buffer rules in 15A NCAC 2B .0200, erosion
and sedimentation control requirements in 15A NCAC Ch. 4 and under the Division's General Permit
NCG0100000, and any requirements pertaining to wetlands under 15A NCAC 2B .0200 and 15A NCAC 2H
.0500.
11. Noncompliance Notification:
The Permittee shall report by telephone to the Mooresville Regional Office, telephone number (704) 663-
1699, as soon as possible, but in no case more than 24 hours or on the next working day, following the
occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility incapable of
adequate wastewater transport, such as mechanical or electrical failures of pumps, line blockage or
breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving waters without
treatment of all or any portion of the influent to such station or facility.
Persons reporting such occurrences by telephone shall also file a written report in letter form within five days
following first knowledge of the occurrence. This report must outline the actions taken or proposed to be
taken to ensure that the problem does not recur.
